Changing Socks Often Can Improve Foot Health: NY Podiatrist
“People typically think about the shoes they’re going to wear if they have a long day on their feet ahead of them, and their choice in sock is not usually the first thing that comes to mind,” says Brad Schaeffer, DPM, board-certified podiatrist and foot surgeon at Central Park SOLE, and star of TLC’s My Feet Are Killing Me. “However, socks can have a substantial impact on pain relief when they provide breathability, cushioning, and compression.”

According to Dr. Schaeffer changing your socks out every day is typically enough to ward off athlete’s foot, blisters, and foot odor that can come from wearing socks too long. If you’re prone to sweaty feet, are on your feet all day, or use compression socks for health reasons, however, he recommends changing them more frequently.

PA Podiatrist Discusses 3-D Orthotic Printing
Bryan Kentner, DPM recently discussed 3-D orthotic printing in a biomechanics installment of Ben Pearl, DPM's podcast Fit Foot U. Dr. Kentner reviewed the ways to change the density of the orthotics with various printer settings. He also spoke about options for different types of thermoplastic polyurethane (TPU) materials, including how to determine if you can use materials that vary from the original materials provided by the 3-D orthotic company.

Dr. Kentner stated that while 3-D orthotics are covered by insurance, Medicare does not yet cover 3-D printed orthotics for diabetics. Kentner continues to be on the leading edge of this evolving 3-D print technology.

Query: Evans Calcaneal Osteotomy
The procedure code for Evans calcaneal osteotomy is CPT 28300 (I think). The diagnosis code for pes planus is M21.42. The last three Evans calcaneal osteotomies that I have done have been denied by Blue Cross. I have another such case on schedule in two weeks that needs an Evans procedure as part of her surgery. Blue Cross has been paying for the other associated procedures that I do with flat foot repair, just not an Evans calcaneal osteotomy. Blue Cross is denying this as “not a covered service.” Does anybody have an idea regarding what is going on? Any suggestions besides just stop doing Evans calcaneal osteotomies?

Response: CPT 28300 is defined as the following: Osteotomy; calcaneus (e.g. Dwyer or Chambers type procedure), with or without internal fixation. Based upon the fact that an Evans calcaneal osteotomy is indicated for the correction of a deformity whereby the front and mid part of the foot is splayed to the outside (abducted), this deformity is often seen in a flatfoot deformity. You would then think that ICD-10-CM code M21.42 which is defined as flat foot (pes planus) (acquired), left foot would be an appropriate ICD-10-CM code to qualify CPT 28300 for reimbursement. Unfortunately, in the above post, it is not a covered diagnosis. The health insurance carrier that is involved in the above scenario is Blue Cross Blue Shield. It is possible that Blue Cross Blue Shield does not want to reimburse for CPT 28300 when it is linked to ICD-10-CM code M21.42 based upon their opinion that having a “flat foot” is not significant enough to justify billing for the procedure that was performed.
What other options are there for an ICD-10-CM code that would qualify CPT 28300 for reimbursement and at the same time describe the presenting pathology in a fashion that would not vary from how it is described based upon the physical examination and the x-ray findings?
Here are some options:
M21.072 Valgus deformity, not elsewhere classified, left ankle
M21.6×2 Other acquired deformities of left foot
Q66.6 Other congenital valgus deformities of feet

This post was written by me three years ago, answering a PM News Quick Poll question. It is interesting that we are at it again. The bottom line is that podiatry is a specialty that includes many subspecialties. They all can be included in everyday practice. The podiatrist who solely chooses to be a surgeon does so just because that is what they want to do.
Not everyone wants to be a foot and ankle surgeon and that’s okay, too. We have done this to ourselves and will continue to do so until we can meet on common ground. Unifying the ABPM and ABFAS is not the answer. We are fine just the way we are until we are not.

For years, I have sat silent reading the debate of my colleagues over board certification. It is amazing that a profession as limited as podiatry, with limitation on license and scope of practice, lack of job availability, competition from PAs, NPs, orthopods, we find more ways to hinder our profession to get ahead. Board certification is yet another self-imposed limitation placed by us on our members.
I unfortunately did not have the chance to become board certified. I did a RPR-PSR-12 residency when there was fierce competition for residency slots and there were no structures in place for a 3-year residency. By being a RPR resident, I am automatically disqualified by APBM. The CPME, the body that came up with RPRs, PORs, PPMRs, PSRs, for some reason decided that RPR was not good enough to qualify for ABPM. There is absolutely no rhyme...
