Stanley Joseph Gurney, Jr., 81, of Meadville, died Friday, March 11, 2022 at Meadville Medical Center surrounded by his loving wife and daughter. Dr. Gurney was a graduate of the Ohio College of Podiatric Medicine.

Dr. Gurney was a podiatrist in Meadville for 32 years and a member of the American Podiatric Medical Association, and past president of the N/W PA division. He was a diplomate of the American Board of Foot and Ankle Surgery and a fellow of the Academy of Ambulatory Foot and Ankle Surgery.

QUERIES (CLINICAL)
Query: Treatment of Osteomyelitis in a Distal Phalanx
 
I performed an arthroplasty on a patient’s 2nd toe to address a chronic diabetic wound. The patient developed cellulitis, and was given immediate IV antibiotics, but ultimately ended up with osteomyelitis at the end of the proximal phalangeal shaft and the base of middle phalanx. The patient is adamant about wanting to keep the toe. Has anybody had success resecting bone, inserting a single vancomycin-impregnated bone cement pellet on a K-wire, and leaving it for spacing?

QUERIES (NON-CLINICAL)
Query: Softening Solution for Calluses and Nails
 
For many years, patients being seen in my office for routine care were given whirlpools to soften thickened toenails, corns, and calluses. I have discontinued whirlpools over the past two years and instead use 3-WEA solution diluted in water and applied to the foot using polygauze non-woven 4 ply 4x4s. Although 3-WEA has been the long time industry standard for this foot prep, I find it isn't always providing enough softening of the skin and nails. The directions call for one gallon of water to 1/4 bottle of 3-WEA. Any mixture more concentrated leaves the foot slippery and difficult to handle. 
 
For those of you still doing routine care on your patients, I would be interested to hear of other skin and nail softening products you are using for this treatment. Also describe how you are applying this solution to your patients' feet. Any input would be appreciated.

RE: Anesthetizing an Infected Toe

After 50 years of anesthetizing toes, I have never had to re-enter for a second injection. I recall school instructors injecting adjacent to the toenail or across the extensor or flexor tendons. This is not only unnecessary but can cause further tissue damage, not to mention additional pain for the patient. I inject straight down from dorsal to plantar at the level of the concavity of the proximal phalanx; one injection at the lateral and one at the medial. This blocks out all four proper digital nerves - two dorsal and two plantar. This is far away from any infected tissue. I always use 2% carbocaine without epinepherine.
 
Like a bad tooth which heals after extraction, once the offending portion of nail is excised, the infected tissue will heal itself without antibiotics. The only time I might use an antibiotic is with brittle diabetics or an otherwise compromised patient. After injection, about ten minutes should be allowed for the mediation to work. The toe will be numb about two hours afterward.

Drs. Udell and Chaskin are clear in their assertions that “important podiatry seminars should be streamed” and be “allow(ed) to offer live steaming”. Such hybrid events are certainly allowed and some have been done, but the word “should” concerns me here, as there are practical concerns. As CEO of the multimodal education company PRESENT e-Learning Systems, that transitioned from live conferences to virtual in early 2020 and has run 12 successful multi-day live virtual/streaming conferences, I can add some clarity to this discussion. 
 
We have intentionally avoided running hybrid conferences that offer both live in-person education while also streaming it to a virtual audience. Why? Because it’s very hard to do a good job at running both an in-person and virtual conference at the same time. We studied carefully what resources we would have to put in place to run a true high quality hybrid event where attendees at both were happy and well served, and we concluded that we would have to hire at least four and likely five new people to properly set up and service this much more complex event. Streaming a live conference isn’t just a simple add-on, and that’s why we’ve chosen to keep our in-person and virtual events separate.

The American Board of Podiatric Orthopedics is no more! If memory serves me correctly, the name was changed to the American Board of Podiatric Orthopedics and Primary Podiatric Medicine.  We then dropped the orthopedic component, completely changing the name to the American Board of Podiatric Medicine. Perhaps those name changes, mirror the profession's decline in podiatric orthopedics and biomechanics.

Doctors! Medicare has made it clear that if you bill for these injections, be ready to pay back everything you were paid and go through the hassle, etc. Why not just tell your patients it is NOT a covered service and charge them cash up front. That is the way to go without worrying about big brother at your back.

I’m on the ethics committee and Director for Podiatry for the American Academy of Stem Cell Physicians (AASCP). We have put out statements to help MDs, DOs, and DPMs understand that regenerative medicine therapies, including amniotic injections, are not yet recognized by the FDA for approval and should not be billed to insurance carriers. This topic was also discussed with the director of the FDA, Dr. Peter Marks, at the AASCP conference. Even with successful cases in trials with investigational new drugs in place, as well as many well published articles, there are still bad actors in the industry. 
 
Most of the blame falls on reps stating that these are allografts and are reimbursed under such codes to push the product. If you are getting good success and document properly and are not offering guarantees, and your patients can afford these adjunct regenerative therapies, this will remain as a viable treatment option in the area of podiatry. Most of our liability insurance companies (such as Medpro) will also cover such therapies offered by doctors, if you use sound judgment. This should be considered as “non-covered“ services by carriers as with any other service we offer.
