PODIATRISTS AND FOOTWEAR
NY Podiatrist Discusses How to Buy a Comfortable, Yet Fashionable Shoe

When it comes to footwear, it used to be you had to forfeit comfort for beauty. Thankfully, designers are finally marrying the two, so you no longer have to walk around in boring “comfort shoes” when you need to look good and be on your feet all day. What to look for? New York City podiatrist Emily Splichal, DPM says, “If you’re someone who is standing all day, you should look for a slight heel, which seems counterintuitive, yet it off-loads the plantar fascia and Achilles tendon — very important for foot health. Standing is actually really hard on the body, so having the slight lift off-loads those areas that get stressed from prolonged standing.”

Dr. Emily Splichal

Dr. Splichal also advises looking for shoes with built-in cushions, especially when you’re on hard surfaces like marble and concrete, because they act as shock absorbers which will help with extra impact; otherwise you could get plantar fasciitis, Achilles tendonitis, or just plain stinging and foot fatigue, because your foot is working harder than it should. Built-in arch supports are always good— especially if you have flat feet, which get tired more easily.

IN THE COURTS
IL Podiatrist Sues St. Anthony’s Health Center Over Firing

The former chief of staff of St. Anthony’s Health Center has filed suit against the operators of the facility, claiming he was fired without a proper hearing. Dr. John E. Lindsay, a podiatrist, also claimed he was fired without consideration for his disability, which he describes in a suit as anxiety and depression.

The suit claims David Wells, vice president of the organization that took over the operation, summarily terminated him on Aug. 6, 2018. Two weeks before his termination, the hospital management accused him of improper conduct, but Lindsay denied any wrongdoing, the suit claims. The suit claims the management cited “outbursts” for his termination, but Lindsay claims the “outbursts” were a result of his symptoms of anxiety and depression. He said additional duties and an increased workload made his symptoms worse.

PRACTICE MANAGEMENT TIP OF THE DAY
Selling Your Practice? Clean up the Coding First - Part 4

As more and more practices are courted for acquisition, firms are being engaged by both buyers and sellers to evaluate the accuracy of coding and documentation, and the efficiency of the revenue cycle. The results of these audits indicate that in many practices, coding and billing has gone unsupervised and unmonitored for quite some time. If your practice is considering a sale, now is a good time to have an independent expert, qualified and experienced in your specialty, review your records and systems. If you don’t identify the coding issues, know that your potential buyer will. Here are some issues that our auditors find with striking regularity.

4. Improper use of modifiers. This runs the gamut and varies by specialty, but the key point here is that in the majority of our audits, modifiers are being used incorrectly. A few examples:

Modifier -25: Significant, separately identifiable E/M service by the same physician on the same day of the procedure or other service. Routinely billing an E/M code with a -25 modifier for established patients receiving regular injections for the same diagnosis and anatomic site is just plain wrong. Our reviews have uncovered a high volume of these.

Modifier -59: Distinct, procedural service. We find frequent incidences of appending modifier -59 when it is not required, due to a National Correct Coding Initiative edit. Overapplying modifier -59 attracts payer attention, and that can lead to an audit.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Tilden H Sokoloff, DPM, MD

Leonard you are spot on! The podiatric physician’s background and training should be no different than that of an ENT, dermatologist, orthopedic surgeon, etc. Our students and residents deserve better and so does the public. This has become a political football between the APMA, schools of podiatric medicine, and the accrediting body. Power and control. We should qualify our education by passing the USMLE or COMLEX and be qualified as MDs or DOs and move into the physicians and surgeons license process. We all work within our limitations.

As a DPM, MD, I can attest to the need to move on this issue now! It is way beyond backs, and limb salvage; it is being part of a delivery system that puts the podiatric medical physician at the same level as the MD and DO and all their recognized specialties. The 4/4/3 is now a 3/3/3 program in medicine and changing rapidly across all medical education. Too much wasted time in medical schools of all types and huge loans for no outcome differences. This conversion is very old and deserves action now!

RESPONSES/COMMENTS (NEWS STORIES)
From: Bret Ribotsky, DPM

It’s a great honor to welcome Sam Mendicino into the PM Podiatry Hall of Fame. I have respected him for decades. His contributions to our profession have been trajectory changing. For many years, his wisdom and class while working with our surgical board has started the incredible outcomes/progress of where we are today. He has stepped up proudly. He has been a bold bright leader and I know that our profession chose wisely by selecting him.

Sam is a very deserving lifetime achievement recipient, and each of us can benefit by emulating his service to our profession.  Congratulations for a well-deserved honor.
