APMWA Announces 32nd Annual Student Writing Competition
DEADLINE APRIL 1, 2018
The American Podiatric Medical Writers Association has announced its 32nd Annual Student Writing Competition.

All papers MUST be non-technical in nature. Appropriate subjects include practice management, ethics, law, education, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

NY Podiatrist Stresses Importance of Strength Training
"I have always said that a healthy runner is a strong runner. Most runners are quite good about putting in the miles, but actually concentrating on strength training is out of their comfort zone. The runner's gait is a kinetic chain...meaning that what happens in one area of the body has a profound effect on another part," says Robert M. Conenello, DPM.

"I screen all my athletes by challenging them with simple tests. One such is a single leg squat. Running is a series of single leg squats. Many of these well-conditioned athletes are unable to perform this simple task. They may need to use their upper body to gain balance or they exhibit a drifting of the bent knee inward on the bent leg. These are indications of poor core strength as well as weak hip abductors. Simple exercises such as planks and clamshells can be performed to increase strength in these areas," says Dr. Conenello.

MN Swimming Champion Prepares for Podiatry School
Danielle Nack can't remember the last time she took two weeks off from swimming. Nack, who was an all-state swimmer at Mankato West before becoming an All-American with the Gophers, will graduate in May with a degree in psychology. She will relax and get a job around Mankato this summer and attend podiatry school in Des Moines, Iowa, in the fall.

Nack will long be remembered for her swimming, dominating as a prep star by winning state championships, putting her name on several school records, and setting the standard at many pools in the Big Nine Conference. Last weekend, she earned All-American honors with the Gophers, swimming on three top-8 relays and setting school records in the 800 freestyle relay and 400 medley relay. At the Big Ten championships a month earlier, she set personal-best times in the 50 freestyle, 100 butterfly, and 200 butterfly, taking second in each event.

1. Cut Back on Supplies

Query: A5512 Returns
The LCD for A5512 states that heat moldable inserts cannot be used in other than diabetic shoes. If a patient decides to return the shoes after dispensing & billing, we know we need to reimburse the insurance company for the shoe payment (A5500). The question we have is, what do we do with the payment of the 6 units of the A5512 after they have been heat molded to the patient? The manufacturer will not accept the heat molded inserts after they have been heat molded to the patient's foot.

Response: This is a tricky situation because, in theory, the shoe and inserts have to be dispensed as a unit. Medicare will not reimburse for a shoe alone or an insert alone. You did not mention why the shoe was returned. Possibly, it could be exchanged for a shoe that fit better? You may return the shoe for some credit but obviously not the inserts.
The next question is refunding Medicare the money since the shoe was returned. You should also refund Medicare for the inserts. The patient could pay you cash for the inserts with an ABN as they technically are not covered in this scenario.

I have used DOX EMR for about 5 years now. This system is podiatry-specific. It has every diagnosis and treatment we use in podiatry pre-programmed into the system from the start. Drs./users do not need to build any templates – building templates and entering details we need in podiatry can be frustrating and very time-consuming. None of that is required with DOX – they have done all that work for you. The system is ready for use from day one. Easy to learn – a few nuances, but very intuitive. DOX is extremely good in their handling of Meaningful Use/ MIPS – with only a little extra work selecting PQRS measures. It is a very good system – I highly recommend it.

RE: Where Did that Foot Skeleton Come From?

Today, they're probably all plastic replicas, but back when I was in podiatry school, we got real bone specimens. I still have a whole wired foot ,a disarticulated foot strung on nylon cord, and a whole leg with half of a pelvis. I've always wondered where the bones came from and I just came across an interesting NPR video that sheds a bit of light.

It's hard for me to believe that this issue is still being addressed for more than 20+ years. I'm not sure that I understand the purpose of regurgitating studies that compare these devices for treatment of foot pain. Most studies on this topic have obvious flaws, such as a person's weight, foot type, and structure, activities, footgear, and primary weight-bearing surface. Besides all those variables, it's a waste of time to compare the two. Either you believe in abnormal foot function as a cause of deformity and pain or you don't.
As a profession, I hope we believe the former. However, we will never educate the public or the insurance companies on the superior benefit of custom foot orthotics to OTC arch supports, so why bother. Insurance companies will never capitulate to reimbursing for custom foot orthotics and there will always be people looking for simple solutions; that's their choice. That's not to say that every person with foot pain won't benefit from OTC arch supports; they may. For those that do, great; for those that don't, well thought-out and well made custom orthotics should benefit them if their problem is due to abnormal foot function - period.

What I have found problematic in most studies on the role of various orthotic devices in plantar fasciopathy is that they all treat this disease like it is a homogeneous entity. Future investigations would be more robust by specifying the factors placing the patient at risk for plantar fasciopathy. It seems intuitive to me that plantar fasciopathy secondary to sagittal plane deformities (e.g. equinus) will do fine no matter what you place under the patient's foot, since a heel lift is probably all they needed.
On the other hand, I believe heel pain with severe coronal plane deformities will benefit from an appropriately posted customized device. This paper, while limited in its methodology, hints that there may be some substance to this approach.
