TX Podiatrist Expresses Fear and Concern About Anti-Asian Hate Crimes
After the Atlanta-area shootings that killed eight people, six of them Asian women, Thuy Ho-Ellsworth, DPM felt many emotions. "Oh, it was really hard, I mean, to see that because these were Asian American females, and it just hits home because you identify with them and when you feel that these are businesses that were specifically targeted for this type of demographic. It's a combination of devastation set in with fear and concern and just immense sadness," Dr. Ho-Ellsworth said.

Last month, Dr. Ho-Ellsworth joined healthcare workers across the nation and posted personal experiences of racism, raising awareness of hate crimes targeting the Asian-American community using #HCWFORASIANLIVES, a movement started by Dr. Jennifer Chung.

NY Podiatrists Discuss Bunion Correctors
"Bunion correctors can help in keeping the joint loose and limber and can even provide some temporary pain relief," says Pedram Hendizadeh, DPM, a podiatrist and foot surgeon in New York City. "Consistent use of some bunion correctors is thought to possibly slow down the progression of stiffness." Dr. Hendizadeh sometimes recommends bunion correctors to his patients to use as a conservative treatment for pain, or as post-operative treatment.

Keep in mind: A bunion corrector can only be helpful for a deformity that is flexible in nature and will not address the underlying issue that is causing the bunion in the first place. "For those who have an arthritic type of bunion, it may actually cause more pain to use the corrector since they already have limited motion in that joint," Pooja Shah, DPM says. Plus, Dr. Shah notes, the movement of the toe joint is only temporary: "Once the corrector is removed, the bunion will usually fall back into its original position."

Query: Post-Surgical Neuropathic Pain and COVID-19
In the last 3 months, we have noticed an increase in neuropathic pain in some of our patients who have undergone surgeries after receiving recent COVID-19 vaccinations. I assume this is an inflammatory response triggered by the vaccination. Has anyone else noticed this as well?

About three years ago, I did the overhaul of my phone system. I got rid of my outdated analog system and went with a PolyCom digital system provided by my Fiber Internet provider. Every prompt created and introduction message is done digitally. I hired via Fiverr, a voice actor to do my introduction message and background music from Audioblocks.
Every prompt for afterhours, billing, prescription refills, consultations, and emergency calls is handled via the digital system. Voicemails are transcribed and sent to my phone via MP3 format. I have no further need for an “answering service” as it is already built into my digital communications package that I pay monthly for. The package includes 4 phones, 4 lines, a dedicated Internet fax server, and 100MB dedicated fiber optic Internet.

Dr. Cox and Dr. Roth are right on track regarding neurectomy. I have been doing alcohol sclerosing injections since 2001. My father, Dr. M.A. Galluzzo, was doing these injections even before that. I haven't had to remove a neuroma in over 15+ years due to the high success rate of this treatment. I have done in-office DTL release only when the injections have failed which is less than 10 % of the time. I also have great success with neurolysis for stump neuromas. For those who say the success is anecdotal, I would like to know how many patients you have treated with alcohol injections.
When those patients who have undergone this treatment come back for other services and/or refer friends and family, I call that a success. If anecdotal equals success, then I'll keep providing this excellent level of care. And for those of you who don't do neurolysis with alcohol and may be located close to my practice and are thinking of neuroma excision for your patient, you can refer them to me. If it doesn't work, I'll refer them back to you for the procedure.

I have heard anecdotal information about this treatment for many years, very similar to how you described it. In fact there will be an article coming out shortly where I am on the opposite of the side of sclerosing injections. The reason being is if you read the literature, it takes a minimum of 20% alcohol to effectively cause chemical destruction of the nerve. So while I understand that there is a success rate for your injections, I would proffer that it is not the alcohol that is responsible for the success rate but it is the anesthetic itself.
I venture to say if you switched out the alcohol with 4 mg of Decadron, you might even get better results not having to do so many injections. It is conceivable that somehow the alcohol has some sort of chemical physiological effect that enhances the anesthetic or vice versa but I think it’s more likely than not that it’s just the anesthetic that’s doing the job. There needs to be a study done using saline, local anesthetic, local anesthetic with 4% alcohol, and local anesthetic with 4 mg of Decadron. This would answer the questions above.
I also think that the patients that you think were cured by your 7 to 10 injections probably went somewhere else for different treatment. This is likely about the same number who had a neurectomy and did not do well and went somewhere else, which caused the surgeon to believe it was a success.

A recent question was broached, “Should we reply back to his response?” My answer was swift. Yes, let me make this truly clear -- this is what epistemic entitlement looks like. Did Gina Mendoza ask you to validate her ordeals in residency? No. Do you have more relevant experience as a female resident? No. Are you inserting your disapproving opinion whereby downgrading her expertise as a female in her residency? Yes. You can now stop.
Gina Mendoza is a woman who need not defend her personal experience she thought to be challenging as a female. The fact this conversation continues demonstrates how mansplaining is institutional sexism that highlights an inequitable and imbalance of power in our society between men and women.
