PODIATRISTS IN THE NEWS
Foot and Ankle Concerns Often Overlooked: MI Podiatrist

“Foot and ankle concerns are too often overlooked by the general public,” said Jodie Sengstock, DPM, Michigan Podiatric Medical Association (MPMA) director of professional relations. “Our feet are the foundation of our body. One of the goals of the MPMA is to reach out to the public to let them know that annual foot exams can lead to a better quality of life and also may help detect the onset of other life-threatening diseases,” says Dr. Sengstock.

Dr. Jodie Sengstock

A podiatrist's examination of one's feet can help identify early symptoms of serious disorders such as vascular disease and diabetes. Foot issues can also cause other problems throughout our body.

Brown Stripe on Nail May be Sign of Melanoma: NY Podiatrist

A brown or dark stripe or path on the nail, or browning onto the cuticle or skin around the nail may be a sign of melanoma and should be taken seriously, especially if it's spreading to the skin.

Dr. Miguel Cunha

If the brown strip or discoloration extends into your toenail bed or skin, see a podiatrist or doctor right away. In addition to melanoma, brown stripes or patches on toenails could also be a sign of HIV or lupus, podiatrist Miguel Cunha, DPM said.

PRACTICE MANAGEMENT TIP OF THE DAY
Selling Your Practice? Clean up the Coding First - Part 3

As more and more practices are courted for acquisition, firms are being engaged by both buyers and sellers to evaluate the accuracy of coding and documentation, and the efficiency of the revenue cycle. The results of these audits indicate that in many practices, coding and billing has gone unsupervised and unmonitored for quite some time. If your practice is considering a sale, now is a good time to have an independent expert, qualified and experienced in your specialty, review your records and systems. If you don’t identify the coding issues, know that your potential buyer will. Here are some issues that our auditors find with striking regularity.

3.  Unsigned notes. - If an audit uncovers unsigned notes, that’s a problem. We have found provider notes in multiple practices that were never signed. Again, it’s a harbinger of other likely non-compliance issues and a signal for an auditor to dig deeper. Anything that raises the eyebrows of a potential buyer does not bode well for the practice.

CODINGLINE CORNER
Query: Date Last Seen Becoming an Issue

We have experienced this year that more doctors are only seeing their diabetic patients once a year. Patients are on anticoagulants are getting their international normalized ratios (INRs) done and their doctor is adjusting their blood thinner without a face-to-face visit. Is anyone else experiencing this? We all know Medicare’s rules regarding the date last seen. Should we be asking our patients to see their doctors even though technically they don’t need to per their other doctor’s protocols? Is there something we can do to reach out and have these rules looked at? It seems a bit far fetched, but worth asking! Any input is greatly appreciated.

Scott Kurecki, DPM, North Port, FL

Response: Tell your patients that Medicare has a 6-month rule for “Covered” at-risk foot care. If they can’t provide you with a date last seen within 6 months, provide them an ABN because this is otherwise covered. Ask them to pay cash that day as it will not be a covered service. Explain this is out of your hands as this is the Medicare rule.

RESPONSES/COMMENTS (PM ARTICLES) - PART 1A
From: Robert Scott Steinberg, DPM

Sorry, Dr. Trench, but I can't agree with your ideas! I own the foot. You want me to only own part of the foot. Added to that, we will spend the next 50 years explaining our fractured-in-half profession to patients and physicians. That's not going to happen. The residency program at Norwegian American Hospital, headed by Louis Santangelo, DPM, teaches both podiatric medicine and surgery. We have a three times a week foot and ankle clinic at the hospital. Residents also rotate through attendings' offices. In addition, Norwegian American Hospital hosts a twelve-slot family practice residency program, giving our podiatric residents further immersion in medicine.

So, be careful before you break something.

RESPONSES/COMMENTS (PM ARTICLES) - PART 1B
From: Daniel Chaskin, DPM

Regarding Jarad Shapiro’s article, I respectfully disagree. Non-surgical programs lead to the path of being denied an ankle permit to medically treat the ankle in certain states. In NY, a PMSR 24 is not accepted regarding an ankle permit. Connecticut, NY, etc. only recognize a PMSR/RR regarding granting an ankle permit to give medical and surgical treatment. There is currently a shortage of PMSR/RR positions for any of the unmatched to apply to.

RESPONSES/COMMENTS (NEWS STORIES)
From: Robert Scott Steinberg, DPM

If tied properly, you should not be able to slip-off quality stability or motion control shoes. I know I can't slip-off my New Balance 990s without using a lot of force, because they are lace-locked. Loosely tied shoes allow the heel to lift off the base of the shoe, adding deforming force down through the midfoot.
