CA Podiatrist Honored with ACFAS’ Distinguished Service Award
Foot and ankle surgeon Roya Mirmiran, DPM, FACFAS, of Sacramento, CA, is the recipient of the 2018 American College of Foot and Ankle Surgeon's (ACFAS) Distinguished Service Award, one of the College's highest honors. Dr. Mirmiran is the fifteenth recipient of this distinguished honor and was presented with the award at ACFAS' Annual Scientific Conference in Nashville, TN, on March 23, 2018.

Watertown Regional Medical Center and the Lake Mills Clinic has welcomed back podiatrist Brent Yaeggi, DPM. A Midwest native, Dr. Yaeggi has practiced in the area for 23 years.

Dr. Yaeggi graduated from the Scholl College of Podiatric Medicine and completed his residency at Covenant Medical Center in Waterloo IA. He is board cerified by the American Board of Foot and Ankle Surgery and is also affiliated with Fort Memorial Hospital.

REMEMBERING HISTORIC PODIATRISTS |
Fundraising Efforts Underway to Honor Historic KS Podiatrist
Fundraising efforts are underway for Dr. Annie Roark, the first African-American female doctor to open a practice in Wichita Falls. Dr. Roark practiced podiatry in town for nearly 60 years, even offering her services to white patients, something unheard of during segregation. Dr. Roark was extremely active in the east-side community, operating the Isis theater on Flood Street, where a historical marker honoring her now stands.

Dr. Roark passed away in 1984 but was buried in an unmarked grave in Lakeview Cemetery behind Kiwanis Park. To right the wrong of the unmarked grave, the Museum of North Texas History, the Wichita County Historical Commission, and the North Texas Troubadour Show are having a fundraiser next week in order to give Dr. Roark a grave marker.

FL Podiatrist Ordered to Attend DUI School
A Villages podiatrist has been ordered to attend DUI school and to stay away from alcohol as the result of a sentencing hearing earlier this month in Lake County. Dr. Felix Esarey, owner of Tri-County Podiatry in The Villages, had been arrested on a charge of driving under the influence on Thanksgiving. Esarey’s attorney, Jaimie Washo Spivey, was successful in bargaining the charge down from driving under the influence to reckless driving. Esarey has been placed on probation for one year.and has been ordered to participate in a Victim Awareness Program, attend DUI school and seek recommended treatment, perform 50 hours of community service, and to stay away from alcohol.

Once you get the hang of reviewing pricing, terms, and negotiations, look at every expense item on your profit and loss sheet, and apply the same methods to each of them. If you set up a system whereby you know what contracts and pricing you have with each vendor, and routinely review those rates annually and/or upon renewal, you will save yourself thousands of dollars in expenses every year.

Yes, there are those who will respond well to OTC arch supports, heel cushions, arch pads, change of shoes, etc. and don't need Rx. foot orthotics. But, for those who have biomechanical faults and especially those with chronic (several months or more) "plantar heel pain", then the "proper" functional foot orthotics are essential for long-term PHP/PF relief.
For those who would benefit from Rx orthotics, the keys to getting the best fitting orthotics with the best chance of eliminating PHP/PF are: 1) a complete biomechanical exam to evaluate for leg-length discrepancy, scoliosis, pelvic tilt, muscle imbalance, along with analysis of stance & gait; 2) Careful evaluation of the orthotic impressions to make sure the feet were held in the proper neutral position as the lab will make the orthotics from whatever impression they are given; 3) The proper prescription (shell material, depth of heel cup, postings, etc.) for that individual patient's pathology, weight, shoes, etc.
The owner of the major orthotic lab that I use told me recently that most of the very large clinics that he makes orthotics for will check off exactly the same things on the prescription form for every pair of orthotics ordered and that it's surprising how many podiatrists take orthotic impressions with the patients standing and think the lab will know what adjustments to make for the orthotics to realign their patients' feet. I would be interested to see the results of this study if the above factors are taken into consideration.

Dr. Young comments without reading the paper. Dr. Udell presumes podiatry research will prove the value of our orthotics versus shams or OTC products. In fact, the meta-analysis conceived from the nil included orthotics developed eight (8) different ways. More fact: Subtalar joint neutral cast orthotics don’t place subjects into subtalar joint neutral weight-bearing. That’s because collapsing feet cast STJ neutral produce devices that are collapsed.
In 2012, Lee, et al. proved that the subtalar position has little to do with controlling arch height or protrusion. They cast non-weight-bearing, a cohort with subtalar joints aligned at 4° eversion, 2° eversion, neutral, and 2° inversion. Digital scanning analyzed the shape of each negative. They found that the changes in arch height, navicular height, and protrusion were insignificant and very small between groups.
There is no (anecdotal) doubt that podiatry $200-1200 custom cast foot orthotics work and make patients happy. Dr. Richard Schuster said, “well placed tissue paper in a shoe will work and make patients happy.” To produce peer-reviewed literature, you must research from the nil. Most important, you must have cohorts, products, and services that can be proven valuable and applicable in practice. In the case of podiatric custom foot orthotics, my opinion is that we do not.
