AT THE COLLEGES
NUI Changes Designation from Podiatry to Podiatric Medicine

Earlier this year, The National University of Ireland (NUI), Galway announced a change in title in its discipline of undergraduate and post-graduate podiatric education from podiatry to “Podiatric Medicine;” the latter is increasingly being employed across the globe, so it is appropriate that Ireland use the term. This designation better reflects the significant advances that the profession has made, particularly regarding extended scope of practice, podiatric surgery, and independent prescribing.

Dean Trepal (5th from left) and President Levine (5th from right) flank Caroline McIntosh, Professor at NUI Galway, as they pose with NUI Galway faculty at Superbones East.

NUI enjoys a strong, dynamic affiliation with NYCPM, and is one of many international affiliations started by NYCPM President Louis L. Levine and Vice President for Academic Affairs Michael Trepal, DPM. The NUI affiliation has led to a number of joint initiatives, including research collaborations, the establishment of the Transatlantic Podiatric Medicine international conference series, and student exchange programs. The change in title to podiatric medicine will undoubtedly raise their profile, strengthen their position internationally, and increase opportunities for further international collaborations.

PODIATRISTS AND SPORTS MEDICINE
Simple Stretches Can Make a Difference: TX Podiatrist

“Imbalance or weakness of the muscles can lead to limited motion and can affect each step while walking and running,” says Leslie Campbell, DPM, a podiatrist in Allen, Texas. “If severe enough, this can lead to overuse injuries or ankle, knee, and back pain.” Some simple stretches can make a world of difference.

Dr. Leslie Campbell

“Although it’s often forgotten in the greater scheme of exercise priorities, daily foot stretching is vital to unclench the connective tissue in your feet; since tendons and ligaments have no memory,” Campbell says.

RELEVANT RESEARCH
Genetically Engineered Maggots Clean Non-Healing Wounds + Promote Cell Growth

According to the Daily Mail, researchers have discovered that “genetically engineered maggots can clean non-healing wounds and promote cell growth.” The maggots do this by secreting “human platelet derived growth factor-BB” as they remove dead tissue. In a treatment called “maggot debridement therapy (MDT), the sterile larvae are raised in a lab and applied to” non-healing wounds, such as diabetic foot ulcers. The Food and Drug Administration has approved MDT.

MEDICARE NEWS
CMS Tests Waters for Enhanced Medicare Advantage Audits

The federal government is examining how it can beef up medical record auditing of Medicare Advantage insurers, which have been accused of exaggerating patient diagnoses to pad their payments. New procedures outlined by the feds will lead to heftier payment recoveries, perhaps tens of millions of dollars more compared with past audits.

The latest federal notice comes a few months after the CMS outlined a plan to expand Medicare's Recovery Audit Program to Medicare Advantage, the private, managed-care version that competes with traditional Medicare. Last week, the CMS released a request for information that targeted Medicare Advantage risk-adjustment data validation (RADV) audits. RADV audits serve as the federal government's primary policing tactic to find and recover overpayments made under Medicare Advantage.

QUERIES (NON-CLINICAL)
Query: Source for Silicon Toe Caps

Many years ago, there was a company in West Orange, NJ that fabricated custom silicone toe caps with dispersion pads. Does anyone know if they are still in business and does anyone have any contact information?

CODINGLINE CORNER
Query: CPT 20600 Injection

I am looking for the code to use for an injection of an arthritic joint. I am proposing CPT 20600, however, no LCD exists for the code. Any thoughts?

Donald Carlson, DPM, Hermiston, OR

Response: For injection of an arthritic interphalangeal joint or metatarsal-phalangeal joint, I suggest CPT 20600. For injection of a midtarsal, subtlalar, or ankle joint, I suggest CPT 20605.

Pick the diagnosis code(s) that best describes the pathology and qualifies the therapeutic injection.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Jeff Kittay, DPM

Ordinarily, I agree with our colleague Dr. Udell on most everything he posts here on PM News, but I must take exception with his statement that "our teaching institutions are under no obligation to ensure that each graduate gets a residency." The schools most certainly ARE responsible for ensuring that EVERY graduate has the right and ability to practice. If that means diminishing the number of students per class so that all will have a slot, so be it.

This is not a crapshoot where some unlucky students get to find out, after the fact, that they now have unlivable debt and no way to pay it back. And telling them beforehand that they might not be able to practice is no solution.  The schools have a moral obligation to provide or create residency slots so that every single graduate can meet any state's requirement for practice. The "residency shortage" we have all been reading about for decades must be solved once and for all by the schools.

RESPONSES/COMMENTS (NEWS STORIES)
From: Michael L. Brody, DPM

One of the major advantages of DR over CR is the reduced dose of radiation necessary to obtain a high quality image. This can be a significant advantage when you think of radiation dosage for chest x-rays, hip x-rays, and other imaging studies. When it comes to foot studies, the radiation dosage is already so low, a 50% reduction in dosage is not as significant. That being said, when regulations are written, the regulation is based upon the entire industry. As a result, DR is viewed as providing a significant safety benefit over CR.

A second major advantage of DR over CR is speed of image. A DR image is immediate and there is a ...
