For the ninth consecutive year, 100% of Western University of Health Sciences College of Podiatric Medicine (WesternU CPM) students matched with a residency program. Like in previous years, the Class of 2022 will be spread throughout the country, including some of the top programs. Three students matched at Scripps Mercy Hospital in San Diego and others are headed to prestigious programs, including Cedars Sinai Medical Center, John Peter Smith Hospital in Fort Worth, TX, Ochsner Clinic Foundation in New Orleans, LA, Scott & White Medical Center in Temple, TX, and St. Mary's Medical Center in San Francisco, CA.

IL Podiatrist Writes Children's Coloring Book About Feet
 
Doctors' offices across the country can soon buy a children's coloring book written by a local podiatrist who wanted to help kids feel less scared when they face common foot problems. Carl Cortese, DPM, who has an office in Normal, IL as well as offices in Clinton and Pontiac, wrote Sammy Sole Saver: The Foot Caring and Coloring Book, set for release next month.

The book includes some of the common foot ailments children may face, like warts, fungus, and heel spurs. Each is represented by a cartoon antagonist, or “Arch Enemies,” waiting for children to color them in. Opposite the coloring page is a page with some of the details about the ailment, such as causes and preventative measures.

Usually, not always, I will do an I&D of a paronychia/onychia and prescribe antibiotics and have the patient come back for a matrixectomy. I don't like to add insult to injury.

I’ve never understood the case of an infection lessening the effects of anesthetics. I’ve never failed to numb a toe in 20 years. I inject dorsal to plantar both medially, and later at the level of the mid-shaft of the proximal phalanx. 10% of the time, I will have to inject one across the plantar aspect of the toe in the same area. After that, don’t wait to apply the tourniquet. Two minutes later, the toe will be numb.

Many years ago, and the day before my son's Bar Mitzvah, he came to me with a badly infected ingrown nail. I immediately took him to my office, blocked the toe with 2% Xylocaine, but achieved ineffective anesthesia. I was quite surprised, and my son, needlessly uncomfortable.
 
The next week, I discussed this with the 3 other podiatrists with whom I worked. They had also experienced a lack of anesthesia, and had dismissed it as a problem with their own technique. We collectively realized we had a bad batch of Xylocaine. Switching to a new package solved the problem immediately.

Dr. Chaskin is right on target. Important podiatry seminars should be streamed so that participants do not miss important lectures. I remember lecturing at a major seminar, given at a beach resort, where out of 600 attendees, only 4 attended my lecture and the following speaker had only one attendee present, me. Things have improved somewhat. Most states require that attendance be taken three times a day; however, most conventions have exhibition halls and many attendees will take breaks from the lectures in order to visit the booths. 
 
Another factor that applies to many doctors is the entire issue of COVID-19 and restrictions on those of us who are immunocompromised being told not to attend events with large crowds. True, this is thankfully a minority but we have always respected the needs of minorities and by allowing all seminars to offer live streaming, we would meet the needs of all of our colleagues.

Today's lower extremity surgeon is well aware of the principles of biomechanics, as taught in the Compendium of Podiatric Biomechanics (Root, Weed, Sgarlato, and Orien). This has evolved into an understanding of limb alignment. This, combined with imaging and clinical exams, means that so much has changed and continues to change. Get on board.

It appears that CMS coverage for the use of amniotic fluid injection is questionable at best and that billing CMS for same may potentially be considered fraud based on the statement below:
 
"CMS places itself on the side of skeptics, and, as a result, most amniotic fluid injections are considered non-reimbursable under Medicare regulations. If a healthcare providers bills Medicare for amniotic fluid injections, they are at an increased risk of being the target of a federal Medicare fraud investigation."
 
As with any other "questionable therapy", it is advisable to check with one's insurance carrier before billing for a product independent of recommendations from specific manufacturers. These regulations are confusing and put providers at a disadvantage because historically even though the service is paid that does not constitute that CMS may require reimbursement for the "non-covered service."
