Avoid Cheese Grater-Type Callus Removers: NY Podiatrist
 
Many salons offer callus remover procedures, often using cheese grater-like devices (or the real things!), but this can be very dangerous, warns Jacqueline Sutera, DPM, a podiatrist in New York City. "When these go too deep and are used on patients with thin skin, poor circulation, or diabetes, it can cause burns and terrible wounds or infections," she says.

To avoid this, she recommends using creams specially made for thicker foot skin. "These creams have gentler ingredients like urea and lactic acid and can be used several times a week to keep skin smooth and thin out calluses in little time," she adds.

At first blush, heels and comfort are rarely synonymous. In fact, designer styles can be some of the worst culprits of wreaking havoc on feet. Najwa Javed, DPM, however, is on a mission to shake up the luxury heel market with her line of supportive heels. “A majority of women’s designer shoes have narrow toe boxes and minimal to no toe depth, which causes blistering and cramping of the toes,” she explains. “This, along with a very steep medial arch incline, causes significant ball of foot trauma.”

Called E’mar, Javed’s brand aims to marry high style with medical innovation backed by her 15 years of expertise as a foot and ankle surgeon in Silicon Valley. The idea was sparked 10 years ago after Javed noticed a lack of wearable yet chic footwear options for her patients with foot pain. “We know from medical literature that heels higher than 65 mm create way too much pressure on the wearer’s forefoot,” said Javed. “With E’mar, the idea was to offer the sleek look of a higher heel, but in a style that offers the wearability of a 2-inch heel.”

Fortunately, for practice managers, often the best motivators are relatively easy, inexpensive, and often a whole lot of fun. Try a few of these ideas at your practice, and see if your team is more willing to go the extra mile.
 
6. Have Fun - "Work culture is important,” says Elizabeth Woodcock. “People want to work at a place that’s fun.” It’s easier than you might thing to make your practice a fun place to work. Cristy Good suggests offering simple amenities at work, such as free snacks and pleasant break rooms. Holiday parties are also a great way to build a nice office culture, Good says.
 
Woodcock suggests things like participating in charity events as an office, having a crazy sweater day, or a dinner swap. If you try, you can probably think of dozens of fun ideas to boost office culture. “It’s a good idea,” Good says, “to let employees take the lead on this; let them develop their own culture.”

Query: Neurologic Diagnosis Codes
 
I keep getting claims denied (generally Medicare) when I try to bill routine foot care procedure codes with neurological codes. An example would be CPT 11055 or CPT 11720 with G35 (MS) or E1142 (DM with neuropathy). The referring physician and date last seen are there. What am I doing wrong?

Response: Both E11.42 and G35 are asterisk diagnoses listed in the Novitas Local Coverage Article: Billing and Coding: Routine Foot Care (A52996). When either are listed as the primary diagnosis, that should not be a problem. You also said you had the referring physician and date last seen on the claim. You need to be sure the NPI for that physician was also on the claim box 17b or its electronic equivalent. The name of the referring physician goes in box 17 (or its electronic equivalent) with his/her full name and credentials. No periods or commas are allowed.
 
The one thing you did not mention in your question was the modifier appended to the CPT code. When billing either of the CPT codes listed, a “Qx” modifier must be appended to the code and the diagnostic pointer must refer to the primary diagnosis code. Remember that while the diagnosis codes you mentioned are neurologic, the Q modifiers refer mainly to vascular issues which must be documented in the medical record of that patient for each and every “Routine Foot Care” (at-risk) visit.

There is a very simple solution that we have been using for years with good success. We set up a separate phone and phone number for our "on-call phone." I think the add-on cost is like $10/month. When a patient calls our office number when we are not there, they are directed by the voice mail to call the "on-call phone" number with any emergencies (except 911 emergencies, of course). Depending on which doctor is on-call, these calls are call-forwarded to his/her personal cell phone (*73). 
 
The patient is then directly speaking to the on-call doctor. The patient doesn't see the doctor's personal number. The doctor knows it is a patient because he sees the "on-call phone" number. Every Monday morning, I cancel the call-forwarding (*72) and reassign the call-forwarding to the on-call doctor's personal phone number (*73). It is extremely simple and inexpensive and works great.

I would like to address the point by Dr. Cox of the adhesions or scar tissue that may be associated with the neuroma. There may be a solution that I have used in the MIS procedure over the past 20+ or so years since I gave up the practice of neurectomy. After using a small incision on the plantar surface near the webspace and releasing of the ligament with a #67 blade (thank you Al Brown, DPM), I insert a small hemostat and open and close it at the interspace and around the metatarsal heads involved. This is meant to free up any of the scar tissue present. This in-office MIS procedure has worked well for me. I hope this tip helps other DPMs.

I thought the article by Dr. Kobak was an informative piece about the National Practitioner Data Bank. There is—as you might imagine—another side to the coin of this organization. It is not complimentary.
 
The National Practitioner Data Bank is run by Health and Human Services and is audited on a somewhat regular basis by the General Accounting Office (GAO). To the best of my abilities, I’ve gathered what appears to be the (so far) three audits of the National Practitioner Data Bank by the GAO. In all three of those audits, the GAO concludes that 40% of the information contained in the databank is wrong. As of this date, I’ve seen no response from...
