IL Podiatrist Discusses Flat Feet
If you have flat feet, the entire sole of your foot touches the floor, meaning you need all the support you can get to prevent discomfort and injury. Although Patrick McEneaney, DPM said kids with flat feet typically don’t experience pain because their feet are still flexible and malleable, complications can arise for adults who don’t wear the proper shoes for their flat feet.

“With adults, we tend to start seeing joint pain more often, a lot of tendinitis, especially with a tendon called the posterior tibial tendon, which is kind of on the inside of your arch. That tendon does a lot of the work of holding up the arch,” he said. “What happens is that after all those years of trying to hold it up, the tendon just can’t do it anymore. The tendon becomes weak and, over time, it can develop scar tissue or even tearing within the tendon itself, and that can lead to pain and problems in that area.”

NY Podiatry Group Adds Two New Offices
The Manhasset-based Advanced Podiatry medical and surgical group is expanding with the purchase of a longstanding practice with two Nassau County offices. The group last month added the Mineola and Williston Park offices of Country Foot Care to its existing locations in Manhasset, Plainview, Huntington, Coram, and the Maspeth section of Queens. Advanced Podiatry's 23 physicians now include Steven Brooks, DPM, the founder of Country Foot Care, which provided podiatric care on Long Island for nearly 40 years.

Also joining Advanced Podiatry in the Mineola and Williston Park offices are Drs. Heather Leeb, Lenny Nguyen, Chris Chung, and Basem Hakim. SA Needs More Podiatrists to Stem Diabetes-Related Amputations
A proper podiatric program has the potential to reduce the number of diabetic patients who face diabetic ulceration and amputation. However, South Africa currently has a shortage of podiatrists. There are about 300 podiatrists registered with the Health Professions Council of South Africa, of which around a sixth work in the public health sector, according to Lucas Breedt, national executive secretary of the Podiatry Association of South Africa.

“This is the case not because podiatrists ‘choose’ to be in the private sector rather than the public sector, but because the public sector has yet to make adequate provisions for the profession,” said Breedt.

Majority of Providers Frustrated With Lofty Patient Expectations For Virtual Visits
Fifty-five percent of healthcare providers are frustrated with managing patient expectations for telehealth visits, according to research released March 15 by UnitedHealth Group. Researchers from UnitedHealth Group subsidiary Optum surveyed healthcare providers from Oct. 25 to Nov. 2. About 75 percent of respondents practiced primary care, 18 percent practiced specialty care, and 4 percent urgent care.
Here are three more findings for the survey:
1. Fifty-eight percent of respondents reported feeling frustrated by the quality of care they can provide via telehealth appointments.
2. Half of respondents reported feeling frustrated by the technical details that come with navigating telehealth.
3. Ninety percent of respondents said they feel telehealth increases convenience for patients, and 52 percent said telehealth makes it easier for patients to find appointments.

Query: Anesthetizing an Infected Toe
For over 30 years, I have seen patients for ingrown nail surgery who presented with a localized infection and associated glomus tumors. While we were taught in school that an infection could “wall off” the effect of the local anesthetic, I’ve never had a problem with a hallux block at its base with the use of 2% Xylocaine plain in order to perform a partial nail border removal and matricectomy in the presence of infection. In the odd case over the years, I would also have to inject a few drops of Xylocaine proximal to the “corner” of the matrix and that usually did the trick. Perhaps it is the acidosis of the infected or inflamed tissue that lessens the effect of Xylocaine.
Today, for the first time in my practice, I was unable to anesthetize a 17-year-old’s toe in the presence of a medial and lateral ingrown nail infection —despite twice having added a few more drops of Xylocaine to the nail base area. Has anyone else had this problem? Do you use mepivicaine instead to better overcome the acidity? Or have I been mistaken all along in not having the patient wait a week with antibiotics before doing a partial nail border removal and matricectomy?

RE: State Societies Should Stream On-Demand Seminars

Every state society should stream conferences on demand as well as have them in person. If a subject matter is brought up, the listener can pause the on-demand lecture and go to their textbooks, online sources, and look up different opinions or concepts regarding the on-demand lecture.
Furthermore, if there are many podiatrists leaving the lecture rooms to visit exhibit halls, etc., information taught during lectures might be lost. On-demand lectures should be available for everyone who attends any state society conferences in-person.

Thank you Dr. Marino for enlightening us on the legalities concerning injectable biologics. The question that hopefully some of our legal minds can shed light on is how the use of biologics that are not approved differ from the use of any other off-label medication that physicians are allowed to prescribe. To be blunt, is it illegal to inject a patient with an amniotic product for arthritis of the foot or do we have to tell the patient that it is an off-label use of the product?

I have been following with interest this topic on “supination” here on PM News over the past week. Since I have been writing and lecturing on foot and lower extremity biomechanics and custom foot orthosis topics now for the past 37 years, I wanted to make a few important points. First of all, just because a number of podiatrists do not immediately respond to a post on PM News regarding the subject of foot and lower extremity biomechanics and foot orthosis therapy does not also mean that many other podiatrists don’t “care” about the subject.
Many of us who are truly interested in biomechanics and foot orthosis therapy do not feel the immediate need to use their valuable time to comment on every PM News posting. That being said, I am quite happy to ...
