Toddlers Should Only Wear Shoes Outside: NY Podiatrist
The word toddler itself actually refers to the first stage of walking. New walkers adorably toddle along on their own two legs, which may make you think it's time to get them some shoes. And have you seen those little shoes! So cute! But as they begin to walk, should they really have shoes on their feet? In fact, toddlers should be barefoot as much as possible, said William Spielfogel, DPM, chief of podiatry in the Department of Orthopedic Surgery and director of the Podiatric Medicine and Surgery Residency Program at Lenox Hill Hospital-Northwell Health.
|
|
Dr. William Spielfogel |
"Walking barefoot can help not only to strengthen the arch of the foot but it can help their muscles, tendons, and ligaments in the foot develop," Spielfogel told POPSUGAR. "It can also help toddlers improve their coordination and proprioception, which is extremely beneficial for their motor development." Dr. Spielfogel said that although staying barefoot is ideal, sometimes it's more important to protect little feet. "Toddlers should wear shoes [when outside] to help protect feet from injury from the elements such as sharp objects and rough surfaces on the ground," he said.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Six Ways to Motivate Your Team for Free - Part 5
Fortunately, for practice managers, often the best motivators are relatively easy, inexpensive, and often a whole lot of fun. Try a few of these ideas at your practice, and see if your team is more willing to go the extra mile.
5. Not Just the Boss
It's great to be recognized by the boss, but Christy Good, CMPE, Senior Industry Advisor at MGMA points out that the practice shouldn’t stop at the top. “Encourage employees to recognize, thank, and support each other,” she says. “Build that culture of support and kindness.”

Query: Late Night and Weekend Hour Paging Options
Is there a newer simpler and less expensive way of being paged after hours for consults and emergencies without a complete telephone system overhaul? This may sound like a dumb question, but our pagers are attached to our smart phones via our phone system. As technology continues to evolve, our phone system is rapidly becoming obsolete and will cause a lot of money to overhaul. Does anyone know of a simpler, less expensive way of receiving and answering after-hours calls without breaking the bank. I am starting to long for the days of digital pagers!
Joseph Grillo, DPM, Ft. Myers, FL

From: Justin Sussner, DPM
It is sad that prior to COVID-19, most of us weren't allowed to give vaccinations. But that has changed in many states and on the national level. But according to the PM News poll, only 4% of us have become vaccinators, with a majority 65% saying they won't! Why not?
Yes, it may take some time away from your practice, but the benefits are many. Besides being the part of the solution, helping society, volunteering improves your name and that of your profession. Since joining my county's Medical Reserve Corps, I have met many other medical professionals and increased my referral base. It is win/win/win! Please consider reaching out to your local DOH (state, county) or hospital to see how you can help.
Justin Sussner, DPM, Suffern, NY

| RESPONSES/COMMENTS (NEWS STORIES) - PART 1A | |
From: Peter J. Bregman, DPM, Brian Kiel, DPM
Dr. Cox, I am glad that you are doing the procedure although you’re not doing the open more definitive procedure. It also seems from your response that you were concerned about reimbursement. However, our goal is to give the patient the best result and while an endoscopic-type procedure is very good, I am a proponent of the open procedure using the appropriate microsurgical techniques.
Peter J. Bregman, DPM, Las Vegas, NV
In-office release of DTL and osteotomies of 2 metatarsals with anecdotal success is a recipe for malpractice. Our obligation is to do the best for the patient, not what is best for us, whether it be our time or pocketbook. If surgical excision is the "best" treatment for a particular patient, then you are obligated to do that procedure or refer them to someone who will do it. I have maybe 3-4 neuroma surgeries per year as we perform sclerosing injections, a minimum of 6 every week or two weeks, with excellent results. We tell the patient that the usual course is 6-8 injections but occasionally there are 1 or 2 more.
Brian Kiel, DPM, Memphis, TN

|
| RESPONSES/COMMENTS (NEWS STORIES) - PART 1B | |
From: Ivar E. Roth, DPM, MPH
I have to agree wholeheartedly with Dr. Bregman concerning neuroma surgery. RARELY does there exist a reason to do a neurectomy. It has been at least ten years since I have removed a neuroma surgically. Between steroid, alcohol injections, and orthotics, rarely is anything else necessary. However, recently I had one very stubborn case and I did a release only, with full success.
Just like it is now accepted that we only do a fascial release for heel spurs, meaning we no longer remove the bone, the same situation exists with neuromas; they just do NOT need to be removed. The standard of care should be NON-removal for us and for the benefit of the patient. That should be a point of distinction between DPMs and the MDs.
Ivar E. Roth, DPM, MPH, Newport Beach, CA
