ABFAS Presents Stone Award to Nine Graduating Students 
 
The American Board of Foot and Ankle Surgery (ABFAS) awarded nine graduating seniors the 2018 Michael L. Stone, DPM Outstanding Professional Conduct Award on March 22, 2018. ABFAS named the award in memory of former Board President Dr. Michael Stone. Dr. Stone was intensely committed to elevating the standards and quality of podiatric education. Each selection, determined by the professors and fellow students at their respective podiatric medical colleges, was awarded to “the individual who exemplified the highest standard of professionalism, deportment, and ethics in their interactions with peers, professors, and staff.” The award included a plaque and a $2,000 cash award.

The American College of Foot and Ankle Surgeons (ACFAS), a national surgical association of over 7,500 foot and ankle surgeons announced its 2018-2019 Board of Directors: The new board was installed March 23, 2018 at the annual business meeting conducted at the ACFAS Annual Scientific Conference in Nashville, TN.

NC Podiatrist Discusses Fall Prevention
 
March is Fall Prevention Month. As our elderly population increases, we see an increased incidence of falling. "More than likely, you have had a loved one fall and suffer from an injury. "As foot and ankle specialists, we see many patients who are at risk of falling because of their shoe selection. We believe in the right shoe for the right activity. Wearing bedroom slippers walking out to the mailbox? Not the right shoe. Going barefoot in the house to clean and cook? Put on shoes. Do you have a hard time putting on shoes and tying them up? Shoes need to be tied properly to ensure stability. Consult a podiatrist to help size, fit, and recommend the best shoe for you," says podiatrist Ryan Meredith, DPM.

"For severe cases, a customized balance brace may benefit a senior that is at risk of falling. Balance problems, history of falls or near falls, loss of feeling in the feet, or weakness are signs that balance braces may benefit the individual. These are customized, covered by insurances, and are accurately molded to the patient’s anatomy to provide maximum support. They are very lightweight and easy to apply and fit into most shoes easily. Ask your podiatrist for an assessment and evaluation to see if you’re eligible for this type of treatment," says Dr. Meredith.

4.  Multiyear Contracts
 
Review your multi-year contracts, such as those you have with EHR companies. While there may not be much that you can do to reduce the rates you pay during that term, know when your contracts are up for renewal and ask for better pricing for any renewals. Much like the phone carriers, they would prefer to work at a discount to keep you than lose you to a competitor. Even if you think you’d never switch EHR vendors, they don’t know that, right?

Query: Coding for correcting failed surgery by different Dr.
 
I recently saw a patient (new to me) who had surgery by another doctor in Sept 2017 for cyst removal and bone reconstruction. She has had continual pain since the surgery, but was basically told to get new inserts. I am treating her for a non-union fracture due to the procedure done last year. I do not know how I would go about coding this. Any assistance would be greatly appreciated.

Response: You would code for the non-union or mal-union of a fracture depending on which bones are involved because that is what you are treating. You could also use a secondary code for pain in the limb or painful hardware if that was an issue. No specific modifiers are needed since you were not the original surgeon and there are no global period issues to be concerned about.

Drs. Young and Schneider question the findings in the recent paper published in a British medical journal which challenged the efficacy of orthotics in the management of heel pain. Most of us would concur with the above doctors that one of the key armaments podiatrists use in combating heel pain are orthotics. We see the results from our daily clinical management of heel pain. 
 
So what is going on here? The authors of the article from Britain cited other studies. They basically "scooped the barrel" for already published studies which showed that orthotics don't help patients with heel pain. This paper and the conclusions of the authors are exactly what a recent thread on PM News was all about. We podiatrists are not doing the research and are not publishing the papers, and if we did, those authors from England may very well have read our papers and come to a different conclusion about the role of orthotics in the management of heel pain.

What I have found problematic in most studies on the role of various orthotic devices in plantar fasciopathy is that they all treat this disease like it is a homogeneous entity. Future investigations would be more robust by specifying the factors placing the patient at risk for plantar fasciopathy. It seems intuitive to me that plantar fasciopathy secondary to sagittal plane deformities (e.g. equinus) will do fine no matter what you place under the patient's foot, since a heel lift is probably all they needed. On the other hand, I believe heel pain with severe coronal plane deformities will benefit from an appropriately posted customized device. This paper, while limited in its methodology, hints that there may be some substance to this approach.

First let me state that I have not read this specific study, but I would like to state what will be obvious to all of our readers which is that all foot orthotics are not the same, all those who write prescriptions and order foot orthotics do not have the same high level of training and expertise and experience with orthotics, and many similar studies are flawed. 
 
An orthopedic surgeon, who specializes in foot and ankle surgery and now practices in my locality, wrote a similar article many years ago that gave a similar impression as this article. When I questioned him at a hospital staff meeting about foot orthotics, I asked him directly the following 4 questions to...

I cannot understand the practice at all. It is unsanitary and cannot be more comfortable. But I am fond of using anecdotes to bring my point across. I would ask my patient, "Would you consider taking a shower, cleaning yourself completely, and then putting on the same underwear you wore before, perhaps for many days? The answer was universally "No". This usually accompanied a level of 'cringing' when responded to by women. This was followed by "Don't you see that this is the same thing? You don't wash you shoes or otherwise sanitize them, but you are willing to put them back on your clean feet.
 
Generally, patients could see the comparison and agree. However, I don't think it changed behaviors in most cases. I think it comes from the perception that feet are generally not as clean anyway as the rest of the body, so it's okay... until something like a wound or infection presents.
