CT Podiatrist Discusses Causes and Prevention of VTEs
Blood clotting—also known as venous thromboembolism (VTE)—is a necessary function in the human body for wound healing and for stopping bleeding, but blood clots can also be dangerous if they form in the wrong place and don't dissolve. According to the CDC, 900,000 Americans a year are affected by blood clots, resulting in nearly 100,000 deaths. "Surgery is also one of the leading causes of blood clots in patients, which means those at highest risk need to be diligent in speaking to their surgeon about their elevated risks so preventive measures can be taken," says Peter Blume, DPM.
|
|
Dr. Peter Blume |
"The biggest recommendation you see in the airline magazines when you're flying is to move your legs to prevent DVT, because people who sit on a long flight have a high incidence of DVT," Dr. Blume explained. "Similarly, if you're sitting after surgery and the blood is not moving in the calf and you're not exercising, you could end up with a clot in your calf. Surgeons have learned over the years that getting people moving after surgery will reduce the risk of a clot causing a pulmonary embolism."

Avoid Stretching When You Have Acute Foot Pain: NJ Podiatrist
Telltale signs of flat feet are a lack of an arch and a wide footbed, both of which can make you more susceptible to pain. “They can occur from your genetics or from years of overuse in unsupportive shoes or from an abnormal gait, for example,” says Dana Canuso, DPM. But one way to prevent or alleviate foot pain is by performing stretches for flat feet.
|
|
Dr. Dana Canuso |
“Since the pain from flat feet can be due to an imbalance and soreness within the many muscles and tendons of the feet, it can often help to stretch these sore muscles or build up muscles that can help rebalance the foot,” says Dr. Canuso, who offers this caveat: “It is my opinion that if you are in acute or severe foot pain, you should avoid stretching the feet and stay as immobile as possible until your body has had a chance to calm down the inflammation a bit naturally.” In this case, it’s best to consult a doctor before beginning any type of intervention, even stretching, to ensure you’re not inadvertently causing more pain or damage.

House Calls a Gratifying Experience for Everyone: MI Podiatrist
When Gary Felhandler, DPM goes to work, he has two goals: to provide the best care possible and to ensure his patients feel comfortable in their setting. As a physician specializing in podiatry, he also delivers an increasingly rare service. Each week, Felhander travels to the homes of several of his patients, bringing his expertise on the foot to those who are often unable to step out of their dwelling.
|
|
Dr. Gary Felhandler |
“(My practice) tends to do a lot of house calls,” said the doctor. “A lot of our patients get to an age where it gets hard for them to get out. So we go to them — it’s a pretty gratifying experience for everyone.”

Arthrotomy for Septic Ankle Arthritis Had Higher Rates of Infection vs. Arthroscopy
Patients who underwent open arthrotomy for septic ankle arthritis had significantly higher odds of surgical site infection and hospital readmission compared with patients who underwent arthroscopy, according to published results. Using the Mariner dataset of the PearlDiver database, researchers obtained demographic and co-morbidity characteristics, including age, gender, infectious etiologies and Elixhauser co-morbidities, of patients who underwent irrigation and debridement through either arthroscopy (n=168) or arthrotomy (n=794) for septic ankle arthritis between 2015 and 2020.
Results showed patients who underwent open arthrotomy and patients who underwent arthroscopy had no significant differences in re-operation rates. However, researchers found patients in the open arthrotomy group had significantly higher rates of surgical site infection and hospital readmission compared with patients in the arthroscopy cohort.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
9 Challenges Funding Can Help Your Practice Overcome
Here are 9 common challenges faced by medical practices that alternative funding can help you overcome:
5. Size of practice
In private practice, operating costs are the responsibility of the sole owner. In a group practice, operating costs are shared between all owners. Funding can help you fill in cash flow gaps without straining your bottom line, regardless of how many owners you have, so you can stay focused on providing top quality care.

| RESPONSES/COMMENTS (PM NEWS QUICK POLLS) | |
From: Vince Marino, DPM
Although I have used them in the past, they are no longer available as the FDA quashed their use since they are injectable biologics. The FDA is requiring the companies go through more hoops. Here is an excerpt from the FDA notice sent to the public:“…warning to consumers in FDA’s July 2020 Consumer Alert:
•Regenerative medicine therapies have not been approved for the treatment or prevention of COVID-19, acute respiratory distress syndrome, or any other complication related to COVID-19.
• Regenerative medicine therapies have not been approved to treat autism, macular degeneration, blindness, chronic pain, or fatigue.
• Regenerative medicine therapies have not been approved to treat any neurological disorder, such as multiple sclerosis, amyotrophic lateral sclerosis (ALS; Lou Gehrig’s disease), Alzheimer’s disease, Parkinson’s disease, epilepsy, or stroke.
• Regenerative medicine therapies have not been approved for the treatment of any cardiovascular or pulmonary (lung) diseases, such as heart disease, emphysema, or COPD.
• Regenerative medicine therapies have not been approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain [emphasis added by poster].
FDA has repeatedly notified manufacturers, clinics, and healthcare practitioners of the need for Investigational New Drug applications (INDs) to legally administer these products and to ensure safety measures are in place prior to administration.” So the results of the poll may be skewed.

From: Robert Scott Steinberg, DPM
I agree one hundred percent with Dr. Stess. Almost every week, I see a new patient whose orthotics, produced from a scan, fail to address the complaint. These devices have many of the same things in common: Below 10mm heel cup; no external rearfoot post; arch height one half of what it should be. The plate is too flexible.
When I see these devices, I am embarrassed for our profession.
Robert Scott Steinberg, DPM, Schaumburg, IL
