Attempting to Run Barefoot Requires Transitioning: MN Podiatrist
Experts believe that cushioned shoes negatively affect balance, stability, and foot strength because the layer of foam between your feet and the ground dulls the messaging between your feet and your brain. Wearing cushioned shoes may also become a sort of crutch, making your feet extra sensitive and causing your ankles to weaken. Before you kick off your shoes for good, however, know that going barefoot isn’t actually the best answer (at least not for everyone). Living a sans-shoes lifestyle may strengthen your feet, but it doesn’t come without risks, says Paul Langer, DPM.

“Aside from the obvious — stepping on sharp objects — the biggest risks are making the transition from conventional shoes too quickly and not allowing for adaptation,” Dr. Langer says. This concept applies to any change in activity, he points out, not just going barefoot. “Just as you shouldn’t go from being sedentary to running five days a week, neither should you go from wearing cushioned, supportive shoes to barefoot or minimalist [shoes].” Instead, you should gradually build up to the scenario and allow your body to comfortably adapt to the new stress or loads. Attempting to forgo the crucial adaptation period can lead to severe soreness in your arches, heels, ankles, calves, knees, and even your hips. In a worst-case scenario, you might actually make your gait worse by trying to transition too quickly.

Recognizing Podiatry Will Cut Amputation Rate in Nigeria by 50%: Podiatrist
A consultant in podiatric medicine, Yahaya Nauzo, DPM, says unnecessary limb amputation has led to increased mortality in patients living with diabetes. Dr. Nauzo noted that effective management of leg ulcers by a podiatric doctor before it degenerates to the point of amputation can increase life expectancy of those living with the disease.
Nauzo lamented that lack of specialists in the field has led to avoidable deaths, as the country doesn’t have enough podiatric doctors to manage the increasing number of those living with diabetes that develop leg ulcers. He blamed the situation on the inability of the Medical and Dental Council of Nigeria to formally recognize podiatry as a branch of medicine. He noted that having a podiatric doctor that those with diabetes can see twice a year will prevent more than 50 per cent of amputations in the country.

TX Podiatrist Uses Newly Launched Plating System
Austin, Texas-based Leith Medical, LLC, a medical device startup, has received U.S. Food and Drug Administration (FDA) 510(k) clearance for its One Screw, One Step™ foot and ankle plating system with anti-backout technology. The lateral malleolar fracture plate system is designed for “fixation of a variety of distal fibular fracture patterns.”

The patented system has made its way into the operating room. Earlier this month, Arush Angirasa, DPM, a partner with Southwest Orthopaedic Group, performed the first patient surgery using the system. The initial product launch includes four plate systems. Surgeons will use the first tarsometatarsal, lesser tarsometatarsal, and first metatarsophalangeal joint systems in the forefoot and midfoot. The lateral malleolar fracture plate system is designed for “fixation of a variety of distal fibular fracture patterns.”

4. Be Specific
Acknowledging a job well done is most effective when the acknowledgment is specific.
“I spoke with a doctor recently,” Elizabeth Woodcock recalls, “who went to his scheduler and said, ‘Mr. Jones (not his real name), because you were able to get this patient in today, he will not have to go to the hospital. You helped give excellent care.’” This not only recognized the employee for his work, it recognized his role in the organization and its mission. “This is very powerful,” says Woodcock.

Query: History and Physical Examination in 2021
What does a medically appropriate history and physical examination consist of in the 2021 E/M protocol? Is this the same as a problem focused E/M? Auditors may have a differing opinion of when a history and physical is medically appropriate. It seems as though process is very subjective.

Response: Yes, it is somewhat subjective. There are no “bullet points” that have to be met as the code selection is based upon conditions-data-risk, not on history or exam. But… you still have to document information to support your claim for the code you selected. You can’t have a one sentence chart note that states “diabetic with foot ulcer, bill CPT 99214.”
Keep in mind your documentation also needs to be there for medical-legal reasons in case something goes terribly wrong. So you get to choose what to put into your chart note. In reality, it should be just as complete as before, except now you don’t have to worry if you had 7 versus 8 bullets or worry about other organ systems to reach the code level. This new system is not a charting shortcut. Be a doctor, complete and thorough. If you select “time” as the basis for your code level, that includes your charting time.

RE: Periungual Warts: Intralesional Antigen Immunotherapy

Derm World Weekly (March 10, 2021) reported on a study published in the Journal of Cutaneous Medicine and Surgery that explored the efficacy of intralesional antigen immunotherapy for the treatment of periungual warts, including the measles, mumps, rubella (MMR) vaccine, Candida antigen, and purified protein derivative (PPD). 150 patients were randomly assigned to receive one of the three immunotherapy treatments, each agent injected intralesionally at a dose of 0.1 mL into the largest wart at two-week intervals. This was continued until complete clearance or for a maximum of five sessions.
Clearance of the warts was achieved in 80% of the Candida antigen group, 74% of the MMR group, and 70% of the PPD group. The number of sessions required ranged from 1.88 (Candida) to 4.22 (MMR). No recurrence was noted in all treatment groups after 6 months.

Recently, I opened our local paper and there was a picture of a pharmacy student giving a vaccine shot. I know the laws have been updated but nothing burns my toast more than the former exclusions of podiatrists. An IM injection can be taught to a teenager. My dad gave thousands of them during WW2 as a technical sergeant. I think back to all the nerve blocks, joint injections, and aspirations I did and have to laugh. My friend, an oral surgeon and professor at a major institution, just shakes his head.

RE: FL Podiatrist Discusses Morton's Neuroma (Peter J. Bregman, DPM)

I read the response from Peter J. Bregman, DPM and agree I would love to never do another neurectomy. That said, I have tried first cutting the deep transverse ligament (DTL) with an #18 needle with poor success. Then I went to an MIS technique of using a #67 or #64 blade and hugging the metatarsal and cutting the ligament with only slightly better success. More recently, I have been doing about a 1.5 cm incision, then using a Freer elevator to locate the DTL. I then use a tenotomy scissor to cut the ligament, and use a hemostat to spread the interspace, with better success.
With this type of procedure, there is very little post-op pain and no bleeding. I use a few simple sutures and it is quickly done; however, there have been some of those procedures that have...

I simply asked that Dr. Mendoza’s claim be substantiated. Dr. Kiel said that “Dr. Mendoza, as a woman, is much more able to validly claim discrimination without written substantiation (which of course does not exist).” That opens a door that we actually do not want to go through. All claims of any type of discrimination should be verified lest we become a different type of society.
