CSPM Hosts Health Fair for Special Olympics
The Special Olympics of Northern California Healthy Athletes Health Fair was recently held at at Samuel Merritt University in Oakland, CA. The event was headed by California Podiatric Medical Association (CPMA) member Tim Dutra, DPM, Clinical Director for SONC Fit Feet Program and California School of Podiatric Medical (CSPM) faculty member. He was assisted by 40 CSPM podiatry students who performed podiatric screenings on athletes.

These screenings included a biomechanics evaluation, foot and ankle exam, inspection of athletic footgear, measurement of feet, gait evaluation, and recommendations/referrals for podiatric care and treatment. The Fit Feet screening was done at the state-of-the-art Motion Analysis Research Center at Samuel Merritt University (SMU). SMU was presented with a plaque of appreciation for continued support of the Special Olympics Healthy Athletes Program.

MA Podiatrist Featured on World Down Syndrome Day
Louis J. DeCaro, DPM appeared on NBC March 21st to help to celebrate World Down Syndrome Day. Dr. DeCaro was featured to help create awareness and express the importance of early foot screenings in those with Down syndrome. According to DeCaro, "Individuals with Down syndrome typically have problems with collagen, which is the major protein that makes up ligaments, tendons, cartilage, bone, and the support structure of the skin. This creates significant laxity from the feet up, thus beginning at a young age the life long destruction of the kinetic chain."

"The resulting effect in 88% of the Down syndrome population is hypotonia, ligamentous laxity and/or hyper-mobility of the joints. The combination of this ligamentous laxity and low muscle tone contribute to orthopedic problems in people with Down syndrome,” says DeCaro.

Shoe Wear Can Indicate Your Foot Type: NY Podiatrist
Olga Shvets, DPM, a podiatrist in Mamaroneck, NY, recommends looking at your current sneaker for clues as to the right fitting shoe. She says that looking at the soles of a well-worn pair of sneakers to see where the sole is worn out will suggest where you are making most of your impact. “Knowing your foot type is important,” she says.

“You can be a pronator, supinator, or neutral. You can identify your pronation by paying attention to which part of your foot hits the ground first. If you find that you have flat feet, you are likely a pronator and the opposite can be said for high arched feet and supination,” says Dr. Shvets. A neutral foot type tends to wear most evenly in the back of their heel.

Selling Your Practice? Clean up the Coding First - Part 1
As more and more practices are courted for acquisition, firms are being engaged by both buyers and sellers to evaluate the accuracy of coding and documentation, and the efficiency of the revenue cycle. The results of these audits indicate that in many practices, coding and billing has gone unsupervised and unmonitored for quite some time. If your practice is considering a sale, now is a good time to have an independent expert, qualified and experienced in your specialty, review your records and systems. If you don’t identify the coding issues, know that your potential buyer will. Here are some issues that our auditors find with striking regularity.
1. Hidden take backs. - Savvy buyers don't want to assume a liability. They will deploy a steely-eyed auditor to review the documentation, supervision, and coding processes. What they are looking for are patterns of incorrect coding that could result in a payer take back. And, if the deal moves ahead, the purchase price will be adjusted to account for the risk.

Query: Is this a Locum Tenens Situation?
Can you please clarify if this would be a locum tenens situation? This starts as a two doctor practice. One doctor leaves permanently to take another position and the other stays, but overwhelmed with patients. The practice is presently looking for new, permanent doctor but needs someone to cover until that new doctor comes aboard. I understand that a locum tenens is only good for 60 days.

Response: I do not believe the situation described in the question qualifies as a reciprocal billing (locum tenens) arrangement. In this scenario, the remaining physician remains and brings in another physician to help. Based on Medicare MedLearn Matters (MM) 10090 dated June 13, 2017, the physician in the practice cannot remain in the practice while the other physician is providing services.
So if the practice brings in another physician to help out the remaining doctor, that additional doctor would need to be qualified and credentialed on the remaining doctor’s insurance plans and could not bill under the NPI or billing numbers of the remaining doctor. That would be considered abusive billing.

The Letters to the Editor in Podiatry Management (March 2019) from Bret Ribotsky, DPM and Gary Docks, DPM regarding the concern that certain forms of cancer may be associated with podiatric medical practice is indeed interesting. Dr. Docks also ends his letter with a statement that “it is time to call in epidemiology.” Epidemiology was an area of emphasis when I completed my degree in public health and I am responding using that background.
According to NIH National Cancer Institute (NCI), the lifetime probability of being diagnosed with cancer is 39.7% in men and 37.6% in women. In 2018 NCI estimated that in 2018 there would be more than 1.7 million new cases of cancer. Therefore, suggesting that DPMs may be more likely to acquire cancer as a result of occupational exposure requires much more than anecdotal conclusions provided by Dr. Docks.
Simplistically, one way to determine if DPMs are more likely to acquire cancer due to the exposure to various materials they use (e.g., fluoroscan, phenol, nail dust, adhesives, x-ray) is to acquire a large sample of people in the profession and a similar number who are not in the profession and follow them yearly over a prolonged period of time and determine what percent of people in both groups get cancer as well as what type of cancer they acquire by age, sex, and ethnicity. In the event that there is a statistically significant increase in the rate of cancer in DPMs compared to the non-DPM group, this would be at least initial evidence that there may be some environmental cause for such a difference. Other epidemiological studies also could take place as well.
