APMA Announces A5513 and K0903 Victory!
 
The American Podiatric Medical Association (APMA) has announced that after much advocacy by APMA and the American Orthotic and Prosthetic Association (AOPA), diabetic shoe inserts fabricated using scanning devices will be reimbursed the same as those inserts made over a positive model of the patient’s foot, effective April 1, using a temporary HCPCS code, K0903.

After a lengthy multi-pronged advocacy approach by APMA and partners, CMS reached the decision that as the processes were essentially equivalent, the fee needed to also be identical. APMA is grateful for the partnership with AOPA, the O&P Alliance, and the Amputee Coalition, as well as indebted to the congressional assistance of Rep. Brad Wenstrup, DPM (R-OH), to reach this successful resolution. As this situation does not represent the only area in which medical technology has evolved, APMA is hopeful that it will serve as the correct precedent as to how other technological advancements will be addressed prospectively.

PA Podiatrist Discusses Treatment of Heel Pain
 
Heel pain that is present for more than four weeks and has not responded to changes in support or anti-inflammatory medications are signs it is time to seek help. According to Jason Miller, DPM, FACFAS, a Pennsylvania foot and ankle surgeon and Fellow member of ACFAS, if the heel pain is interfering with normal daily activities, work, or recreational activities, then it's time to see a foot and ankle surgeon. When visiting a foot and ankle surgeon, patients need to be honest about how their pain developed, their level of activity, what type of shoes they wear and what type of treatment they've completed, if any.

Foot and ankle surgeons will look to conservative treatments first – such as stretching exercises, icing, modifying footgear and activity and sometimes physical therapy. They may also recommend using a night splint or a cortisone injection for patients who experience chronic pain in the morning. "A lot of patients will get better with conservative therapy if done correctly," said Dr. Miller. "The good news is that, for patients who don't respond well to conservative approaches, we have more treatment options than ever before."

PA Podiatrist Receives Lifetime Achievement Award
 
Richard DiBacco, DPM, an Erie native, received a Lifetime Achievement Award from the Greater PA Education Foundation. DiBacco, who serves as director of Podiatry Associates Foot and Ankle Center, which has offices in Erie, Meadville and Conneaut, Ohio, earned a bachelor’s degree from Gannon University and his medical degree from Ohio College of Podiatric Medicine. He went on to complete his residency at the Foot and Ankle Clinic of Youngstown Osteopathic Hospital.

He is the founding director of the podiatric surgical residency program at Millcreek Community Hospital and has served as chief of podiatric surgery at multiple hospitals in northwestern Pennsylvania. Past honors include Doctor of the Year at Union City Memorial Hospital. He currently serves as president of the Kent State University Foot and Ankle Board of Directors.

Drive Down Expenses at Your Medical Practice - Part 3
 
3.  Take a look at services.
 
How much are you paying for your bank charges? For credit card processing? For phone and Internet? Shop around for the best deals. Simply asking your bank for a better deal can produce nice savings. A recent client – single location, four-physician group - saved $4,500 a year in fees by switching to a new bank, but also gained process efficiencies with free remote deposit (no staff time wasted going to the bank every day) and online access to all their accounts through a single sign on.
 
Shop around for better credit card rates too. You can also approach your current vendor and ask them to reduce their fees — there is not much they can do on the credit card company rates (Amex, Visa, MC), but they can reduce monthly fees and per transaction costs. Call your phone company and ask them what special offers they have. If they are not willing to extend those deals to existing customers, tell them that you will be happy to switch to a new company for a better deal. Every time we’ve used that approach, we’ve received a better offer from the carrier.

It was probably a specialty pharmacy which would deal with Novartis to make sure the prior approvals are expedited. Compounding pharmacies are a completely different story. But most likely, even if the patient got the prescription through their own Medicare part D plan, the situation would have been the same.
 
I assume all of us in practice understand that no matter what the drug rep says about minimal cost to the patient, the true medication cost is between high and outrageous. The rep will tell us, "If your patient can't afford the price of the medicine, we have a special discount so they can get the product for $50." BTW, that doesn't apply to Medicare patients because of the machinations of Congress. I've never understood how the drug company can afford to give away a $1,000 product for $50. Unless...! 
 
Yes, we should discuss the possible cost of the medications we prescribe, and maybe decide we will not prescribe those medications. But we can also offer alternatives to the prescription topical medications for nails. The OTC nail preparations that we can dispense from our office would cost the patient $40-$50 out-of-pocket for a reasonable size bottle. I can't believe that the efficacy of those preparations could be worse than the 10% quoted for the prescription medication.

I don't necessarily disagree with the conclusions (without reading the study), but the study can be misleading. What were the biomechanical and pathophysiologic diagnoses of the PHP, for prescribing the orthotics. Not all PHPs are the same and respond to orthotics.
 
My empirical clinical experience indicates combined therapy treatment seems to be a reliable path to treat PHP - custom-molded orthotics in combination with other modalities i.e.: NSAIDs, physiotherapy, stretching, etc. Unitherapy modalities, in my clinical experiences, are not as effective for treating PHP.

I'm assuming that PHP refers to plantar fasciitis. In 1976, I developed severe “PHP”. I saw a podiatrist who prescribed functional foot orthotics. I walked into his office on my toes to pick up the devices and walked out without heel pain. This actually led to my going back to school to become a podiatrist at the age of 42. In the years since, I have prescribed thousands.... yes thousands of orthotics to treat biomechanical issues... mostly plantar fasciitis.
 
I'm retired now (kind of). I still get positive feedback from patients whom I treated years ago. I am pretty certain that the majority of podiatrists who have treated “PHP” would agree that functional foot orthotics, done by professionals, have produced positive results in many if not most patients. Is it possible that the orthotics used in this “sham study” were not fabricated by true professionals?

As referenced on sterilization options from previous posts, I have to wonder how these burrs being re-used are cleansed and sterilized for reuse. 
 
In the end, this is a lot of discussion on a $32 or less procedure, which does not really sustain a profitable practice on today’s environment.

It is my hope that the discussion of nail grinding has resulted in, I pray, at least one podiatrist to stop grinding mycotic nails. We, as physicians, should treat the infection of nail fungus, not perpetuate it, and thus place ourselves, staff, and patients at risk. 
 
To those who think they are not at risk, a recent study reported on MSN.com, revealed the incidence of lung infections in dentists as an occupational hazard to tooth grinding. Dentists glove, mask, and take precautions with far more respect for the dangers they encounter in their work than we do, and yet, they too are at risk.

No Socks, No Entry at Royal Ascot
 
Men who wear shoes without socks may be refused entry to the Royal Ascot this summer under new rules. For decades, Ascot has been renowned for its strict dress code, and this year, male attendees have been warned that they could be unceremoniously turned away if they arrive without socks.

Last year, the College of Podiatry warned that they were seeing a rise in the number of 18-25 year-old men who were suffering from fungal infections. This was directly linked to the increase in young men wearing shoes without socks and ill-fitting shoes. Not wearing socks causes sweat to seep into the shoe's leather, which then turns the inside of the boot into a breeding ground for bacteria.
