UTRGV School of Podiatric Medicine Coming to TX
 
The University of Texas Rio Grande Valley School of Podiatric Medicine will reside in the University’s medical campus in Harlingen, according to city Mayor Chris Boswell. "This will be the first school of podiatry in the state of Texas and will allow students to pursue careers in the medical and surgical care of the foot and ankle,” Boswell said, "That will allow students to complete two years of university education prior to graduation and complete selected four-year degrees in the same building without having to leave home."

Flip-Flops Tied to Surge in Teenage Heel Pain: WV Podiatrist
 
Many are welcoming the warmer weather sporting flip-flop sandals; however, their popularity among teens and young adults is responsible for a growing epidemic of heel pain in this population, Stefan Lorincz, DPM, a member of the American College of Foot and Ankle Surgeons said.

“We’re seeing more heel pain than ever in patients 15 to 25 years old, a group that usually doesn’t have this problem,” Lorincz said. “A major contributor is wearing flip-flop sandals with paper-thin soles every day to school. Flip-flops have no arch support and can accentuate any abnormal biomechanics in foot motion, and this eventually brings pain and inflammation.” Dr. Lorincz recommends wearing sandals with reasonably strong soles and arch support.

Query: Two New Patients in One
 
I have a new patient scheduled who was originally due to see me for an injury to their ankle that occured at work. The injury is a Workers Compensation case. The interesting part of this is that the patient told the scheduling staff that she would also like to have an “ingrown nail taken care of” at the same visit. The patient is quick to say that the ingrown nail does not have to do with the work comp case and she would like to use her regular insurance for that problem.
 
I am planning to use the EMR system to create two encounters that day: one for the Workers Comp injury and the other for the unrelated onychocryptosis. If both encounters involve a separate and independent E/M with appropriate documentation, would they both be considered new patient visits? They both can’t be new patient visits… right?

Response: Workers Compensation is a unique entity unto itself. If the primary, major reason that the patient is coming to the office is for the injury to the ankle that occurred at work while the patient was on the job, I would bring the patient in as a new patient for an initial encounter dedicated specifically for the work-related injury. This way an appropriate diagnosis and a treatment plan can be determined and submitted to the proper health insurance carrier or perhaps the place of employment itself depending upon what entity is responsible for any pre-certification and approval for the proposed treatment to be allowed and reimbursed. The last thing that you need is additional documentation in the medical record for the date of service in question related to a situation or additional problem that has absolutely nothing to do with the work related encounter.
 
I would bring the patient back at another date of service or perhaps, if possible, on the same date of service for the issue with the ingrown toenail/onychocryptosis that needs to be addressed. These two encounters would obviously not be initial patient encounters despite the fact that two different, separate health insurance concerns are involved. In the scenario that I described, the Workers Compensation encounter/visit would qualify as the “New Patient Visit.” The ingrown toenail encounter/visit would qualify as an Established Patient Encounter.

I also put these patients on Bromelain, MSM, and Curcumin along with adequate hydration. I’m waiting to see some literature on BPC157 peptide injections for the same.

I would agree with the commentary by Drs. Ivar Roth and David Secord regarding the recent interest/revolutionary concept of fat transplant to supplement fat pad atrophy in the infracalcaneal and sub-metatarsal regions. Although Edward Chaiman, DPM may have developed instrumentation to facilitate this procedure, he certainly did not invent the idea. Raymond J. Suppan, DPM, the Chairman of the Department of Surgery, Ohio College of Podiatric Medicine in 1975 had already contemplated the concept, performed his own clinical trials, and determined the procedure unsatisfactory due to migration and atrophy of the fat transplant.

I am pleased to have read Dr. Paul Kesselman's posting regarding the sorry state of podiatric biomechanics. I want to commend him for raising this topic and hopefully some practitioners will chime in. In previous papers that have been published, many authors have suggested that successes in reducing pedal pathological symptoms are directly related to custom functional foot orthotics. No long-term controlled clinical studies have been published that have substantiated the verifiable benefits of custom devices. Further, most studies have never specified specifically how the exact position the foot was to be maintained during obtaining a negative model nor if a weight-bearing or non-weight bearing impression provides greater control of the abnormal foot motion.
 
I harken to my biomechanical instruction many years ago and how positioning of the foot, while obtaining a negative cast, was the key to achieving the optimal...
