CA Podiatrist Develops Telephone App for African-Americans
Bill Releford, DPM, a podiatrist and lead developer of the Black Doctor 24/7 app, is hoping telemedicine with a network of about 20 doctors will help Black men nationwide feel more comfortable about seeking preventative care from home.
|
|
Dr. Bill Releford |
"With this particular application, we’re able to at least address one of the barriers to healthcare, and that’s called providers," said Dr. Releford. "Many studies have actually documented the fact that African-Americans do get better care when they are treated by an African-American healthcare provider."

Wide Toe Box Can Prevent Some Commin Foot Problems: UK Podiatrist
"Podiatrists see a wide range of foot and lower-limb concerns," explains Emma McConnachie, podiatrist and spokesperson for The College of Podiatry. "If you suffer from problems with your toes, such as corns or ingrown toenails, then avoid a narrow toe box," McConnachie says.
|
|
Emma McConnachie |
"Yes, your toes really will slide forward into the pointy parts of your shoes! Even slight compression can cause problems, and I often see patients in my clinic suffering as a result of wearing shoes that are narrower than their forefoot." If this is the case, try wide-fit shoes. But as McConnachie points out, a 'wide fit' is simply wider than other shoes in a particular range and doesn't necessarily mean they will fit if you have a wider foot, so having your feet measured properly is important.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Six Ways to Motivate Your Team for Free - Part 3
Fortunately, for practice managers, often the best motivators are relatively easy, inexpensive, and often a whole lot of fun. Try a few of these ideas at your practice, and see if your team is more willing to go the extra mile.
3. Pat Backs
When your employees do a good job, make sure they know that you know. You can acknowledge the work of a team or point out individuals who’ve gone above and beyond. Just make sure they get the memo. Don’t assume they know you value you them.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
From: Stephen Musser, DPM
This can be a difficult situation, but sometimes you have to be politely rude and tell the patient, "Mr./Ms./Mrs., if I have answered all your medical questions, I need to excuse myself and go on to my next patient so I don't get too far behind. If you think of any more questions, please send me a message through the EHR portal."

| RESPONSES/COMMENTS (NEWS STORIES) - PART 1 | |
From: Peter J. Bregman, DPM
I read the news item highlighting the coverage of Morton’s neuroma by Dr. Kushner, and not surprisingly he mentions neurectomy as a end-point solution. When will this profession understand and adopt decompression as the first surgical option for a patient? It has a very high success rate when done properly and has zero chance of stump neuromas. I have treated too many stump neuromas from neurectomies, which can ruin people’s lives for a long time.
I don’t think our profession understands the damage that can be done by performing a neurectomy. I recommend everyone look deeper into doing decompression instead, and receive proper training if necessary and certainly the Association of Extremity Nerve Surgeons would be the place to do that. Having performed over 400 nerve decompressions with around 90% success and zero stump neuromas, I cannot advocate neurectomy. I hope there will be an end to neurectomies as a first line surgical treatment.
Peter J. Bregman, DPM, Las Vegas, NV

| RESPONSES/COMMENTS (NEWS STORIES) - PART 3A | |
From: Amol Saxena, DPM, Patricia Kirk, DPM
Two articles by colleagues on sexual harassment and gender discrimination in our profession, both appeared in last year's Journal of Foot & Ankle Surgery: Article 1, Article 2. I encourage those commenting to read the above, and if they haven't already, the book Caste by Isabelle Wilkerson.
Amol Saxena, DPM, Palo Alto, CA
Here are a few articles: Article 3, Article 4, Article 5. I am hoping that this is enough to satisfy Dr. Schneider's need for Dr. Mendoza to prove herself. Or would you be more impressed with her re-telling her hellish stories of inequalities to you? Shall I start with the guidance counselor who told me that I should marry a surgeon? How about the surgeon who would rub himself all over any female resident? The orthopedic surgeon who asked me every single day for months how many men I had had sex with? (The nursing staff alerted my director about this situation, because of course I had no "proof", so I took this garbage from this guy every day!). The sexual innuendos in every damn case? Or how many times was I told that I should just marry a doctor? I guarantee you that my quiet strength and the fact that I made it through don't need to be proved; it was there, it was always there! Asking her to substantiate this with all the literature available is exactly why it has gone on for so long.
Patricia Kirk, DPM, Lexington, TN

| RESPONSES/COMMENTS (NEWS STORIES) - PART 3B | |
From: Joan Oloff, DPM
After having read Dr. Mendoza’s comments on her experiences of being in a male dominated profession, then Dr. Schneider’s follow-up comments, I like to comment on the sheer absurdity of Dr. Schneider’s comments. Since Dr. Schneider might best relate to an analogy related to his male colleagues, I will offer this. Imagine if Dr. Mendoza was a male of color, describing his and his friend's experiences with racism in training. Would it be appropriate to respond that, in today’s climate of black lives matter, can this colleague’s claims be substantiated?
Yes, Dr. Schneider, I can assure you that gender discrimination exists in this profession. While I believe we have many strides past blatant gender discrimination, there is still much work to be done until we achieve full parity. Denial of the problem only serves to exacerbate it. As Dr. Mendoza mentioned, student doctors often network with other female residents to learn which programs are more gender neutral when applying.
Gender discrimination is something I personally endured throughout my career, even while having an academic appointment at the former California College of Podiatric Medicine. Let’s work together to truly have gender parity. Acceptance and awareness is a good beginning.
Joan Oloff, DPM, Los Gatos, CA

From: Jenneffer Pulapaka, DPM
For many women, it is simple. We are fed up. Sadly, our country has reached a level of violence and threats that include hate crimes towards women, against our friends, and ourselves. People now openly shoot women, threaten to beat women over customer service dilemmas, or verbally assault female medical team members. It has reached a level where oppositional critics rally for aggression while dismissing the request for assistance in preventing further violence.
Patient-perpetrated sexual harassment needs to be reported by physicians. Unfortunately, I'm not alone; many women are faced with inappropriate behavior, advances, or even harassment from their...
