ON THE INTERNATIONAL LECTURE CIRCUIT
CA Podiatrist Delivers Transatlantic Keynote

Dr. David G. Armstrong, Professor of Surgery and Director of the Southwestern Academic Limb Salvage Alliance (SALSA) at Keck School of Medicine of USC delivered a wide ranging lecture via transatlantic video link discussing current and next-generation strategies to treat people at high risk for diabetes-related amputation.

Dr. David Armstrong

The British Association of Prosthetists and Orthotists symposium (BAPO18), held at the Lancashire County Cricket Club in Manchester, featured several hundred clinicians from across the UK and Europe. Among a number of other efforts, Armstrong described new “wearable robots and exotendons” that mimic the body’s own musculoskeletal system that can help support and even augment strength and mobility.

RELEVANT RESEARCH
Foot Orthoses Not Superior for Improving Pain and Function in PHP

Plantar heel pain (PHP) is common. Foot orthoses are often applied as treatment for PHP, even though there is little evidence to support this. To investigate the effects of different orthoses on pain, function, and self-reported recovery in patients with PHP and compare them with other conservative interventions, a systematic literature search was conducted in MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science, CINAHL and Google Scholar up to January 2017.

Randomized controlled trials compared foot orthoses with a control (defined as no intervention, sham, or other type of conservative treatment) reporting on pain, function or self-reported recovery in patients with PHP. Conclusions: Foot orthoses are not superior for improving pain and function compared with sham or other conservative treatment in patients with PHP.

HEALTHCARE NEWS
Physician Pay: Gender Gap Widens

You probably caught the headline that more women than men enrolled in medical school for the first time in 2017. According to a new report released by Doximity, that hoopla around that shift can be tempered a bit: the gap between men and women when it comes to physician pay actually increased from 2016 to 2017.

In 2016, female physicians made $91,285 less than their male counterparts. In 2017, that gap grew to $105,000. More than half of metros saw the pay gap increase year over year, and the trend held in every metro area, among owners, contractors, and employees, and within every specialty.

PRACTICE MANAGEMENT TIP OF THE DAY
Drive Down Expenses at Your Medical Practice - Part 2

2. Move on to salary and benefits.

Have you considered using a professional employment organization (PEO)? These organizations become the employer of record; so while you maintain the management of your employees, the PEO bundles up payroll, HR services and compliance, workers compensation, employer liability insurance, healthcare benefits, and so on under its entity. You pay either a flat rate per employee per month (Justworks) or a percentage of payroll (ADP), and the PEO does the rest. While it might seem more expensive than what you pay for payroll cost alone, one of the biggest benefits (and cost savings) is access to healthcare benefits that are priced for a much larger entity than your practice could obtain alone. Additionally, there are often many ‘perks’ included, such as gym memberships and life insurance, optional short-term and long disability plans that employees can purchase, and so on.

You should also determine if you are routinely paying overtime, and shop around for new payroll companies to make sure that you are not overpaying for those services. Finding more efficient payroll systems can reduce time and create efficiencies too, so look for price plus efficiency when evaluating options.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Duane “Bud” Rowley, DPM

Duane “Bud” Rowley, DPM put himself second, never first. The beloved father, husband, podiatrist, and Lapeer-area soccer coach passed away Wednesday, March 14 after a battle with cancer. He was 52.

Dr. Duane “Bud” Rowley

Rowley earned his Doctorate of Podiatric Medicine from Ohio College of Podiatric Medicine. Rowley was diagnosed with Stage 4 colon cancer five years ago, but remained upbeat throughout the half-decade since, continuing in his role of father to his six children, coach to dozens of area youth, and doctor to many patients in the area.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Misinformation About Cartiva
From: Craig Breslauer, DPM

I have been performing Cartiva with positive results since its inception. I have performed over 20 thus far. Compared to most surgeries we do, it is technically easy and quick. The purpose of my post is to garner opinions regarding how to, or not, handle some of the comments I have encountered regarding podiatrists vs. foot orthos doing this procedure. I became aware of a Cartiva Facebook page and have read quite a bit. The majority of the posters have used orthos and many recommended only using a foot ortho. Furthermore, there is a foot ortho who acts as a moderator. I do not know if he is affiliated with Cartiva as a company or sanctioned as a moderator.

If the company supports the propagation of such misinformation, I think our profession needs to address it. I know I see poor outcomes/complications of both fellow podiatrists and foot orthos as I am certain they see some of mine.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Paul Kesselman, DPM, Simon Young, DPM

Thanks to Dr. Borreggine for pointing out it was a PA at the derm practice who wrote the Rx. He is absolutely correct that all prescribers should have some sense of the patient’s cost of their prescription regimens.

Paul Kesselman, DPM, Woodside, NY

I am incensed at the term “fake news”. It is a term to indicate intent on lying vs. ignorance. Furthermore, it’s a term used by leaders who are authoritarians and themselves are promoting false ideology and facts. Yes, Kerydin and Jublia are not compound pharmacy drugs, but they surely charge the same. The PA might have been misinformed or didn’t care. It's true that the patient did have documented onychomycosis, but was her health in grave risk? These abuses nevertheless should be exposed.

We should ask the Reps what are the costs of these medications. Anything products advertised or promoted by a Rep. are really, really expensive. It’s unconscionable to pay $1,500 for 10 ml Kerydin or 8 ml Jublia. Treatment can cost over $10,000 for let’s say 10 nails with a low cure rate, provided it’s involving the matrix. Anterior superficial, you can bump up  the cure rate to 26%.

Boutique pharmacies charge outrageous prices but we write the prescriptions and they laugh all the way to the bank, and insurances and patients blame and complain to the prescribers. If you have commercial insurance (NOT MEDICARE, MEDICAID, TRICARE), you can pay out- of-pocket for $100 in my office area. What’s wrong with that picture? This is an outrage which doesn’t only involve podiatric practitioners but all medicine and should be outlawed and regulated. This has little to do with R&D. R&D is a farce at this point.

Simon Young, DPM, NY, NY

RESPONSES/COMMENTS (PM NEWS QUICKPOLLS)
From: Janet McCormick

I am interested in knowing from those commenting their thoughts on the use of N95 masks when REDUCING mycotic nails? Do you feel they safeguard your breathing space enough, if the fit test is positive? What about the use of face shields, as they use them effectively in dentistry? BTW, I don't see "grind" as a word professionals should use when referring to electric files. Nor does "Dremel" reflect the use of professional equipment in many cases.

Janet McCormick, Frostproof, FL

We have a problem in our profession with grinding nails. We fortunately, or unfortunately, have become the experts of treating and debriding nails. It is in most of our DNA as podiatrists and surgeons to strive for excellence. I have not found a way to do a great job of debriding nails without using a grinder. To meet Medicare's requirement for a covered service mostly requires aggressive debridement. Most of the patients for whom we debride nails for cannot do it for themselves.

If it were my nails, I would want them grinded. If it was my parent's nails, I would want them grinded. We are unfortunately putting ourselves at personal health risk by doing so. We are not unique in this problem. Tell the coal miner not to go into the mine. Tell the radiology technologist not to take x-rays. What we have is a problem that needs solving. Our current nail dust extractors or vacuums are apparently not adequate to remove enough of the nail dust. We need a better mouse trap. Enterprising individuals or the companies that make and sell nail vacuums need to come up with a contraption that contains the dust better. There is a clearly a market for this. Our health and the quality of our work is worth it.
