During this event, students were celebrated for their outstanding efforts and achievements. Many of the industry’s brightest podiatry students from each of the four classes were honored with various awards and scholarships. A 3rd-year student, Bryan Levay, was selected to receive the Blaine Labs Endowed Scholarship based on his overall GPA, public service, leadership potential, and extracurricular activities.

AAPPM Honors Two Podiatrists at Its Midwinter Seminar
The American Academy of Podiatric Practice Management (AAPPM) hosted yet another terrific Midwinter Seminar earlier this month in Pittsburgh, Pennsylvania. Over 200 attendees were presented with the most up-to-date information to help catapult their practices toward success. The 2013 Midwinter Seminar was praised as one of the most energetic and smoothest run conferences.

In the midst of the excitement, learning, and energy, the AAPPM also took the opportunity in Pittsburgh to present two awards to two different and influential podiatrists. Hal Ornstein, DPM, the AAPPM Chairman, was presented with the very prized and rarely awarded B.C. Egeter Award for his leadership and for the efforts he continues to make to teach the next generation of podiatrists to lead the profession. Eric Polansky, DPM, was presented with the 2013 President’s Award for his leadership and support for every facet of the profession.

Car Crashes into CA Podiatry Office
A woman in a Subaru crashed into the large front window of a podiatrist's office on Fairmount Avenue in El Cerrito Monday, the office said. No one appeared injured in the crash, said Deanne Giacomelli, who handles medical billing for the Fairmount Podiatry Group.

Dr. Darlene Fahmie said the car smashed into the glass front of the office about 2:45 p.m. "Thank God nobody was sitting in those chairs," Giacomelli said. The office has chairs for waiting patients just inside the window, she said.

Former FL Podiatry Manager Sentenced
The Pensacola woman charged with stealing nearly $44,000 from her job at Gulf Coast Podiatry has been sentenced. Circuit Judge Ross Goodman sentenced Ellen Renee Garrett, 55, to 24 months in state prison followed by 8 years probation, Tuesday. Garrett pleaded no contest to one count of grand theft from her employer for embezzling money from the practice during her time as office manager.
Garrett, who began working for the office on North Ninth Avenue in 2002, would post fraudulent transactions to cover up the stolen money. She did this over the course of more than four years, according to the State Attorney’s Office. Her actions were discovered during an audit, police said. In addition to prison time and probation, Garrett must pay restitution to her former physician employer.

This is certainly a complex case and likely needs staging of some sort. The osseous problems need to be addressed in the rear foot and any equinus as well. For the record, and as an AENS fellow using Semmes-Weinstein filaments for any type of real neurologic exam, is not acceptable as anything less than crude.
Having said that, I think it is important for a good neurologic consultation. This would rule out the presence of any progressive neurologic disorder. If there is a true peripheral nerve entrapment, then certainly a referral to a peripheral nerve surgeon would be appropriate. Just looking at the x-ray, the deep peroneal nerve may be impinged by the dislocating talo-navicular joint.

The surgical plan you have sounds rational. One other thought might be a plan to simply address the chief complaint on this 80+ y/o and improve the other issues (other lesser digits and hallux varus) with some more conservative ideas. There is one saying that I have found true, especially in the elderly: The enemy of good is perfect.

I was very surprised to read some of the replies to this query. I expected to read about fixated metatarsal osteotomies and fusions of lesser toes, but I really didn't expect podiatrists advocating amputation of the second toe and a fusion of the first MPJ, a non-painful joint.
I have a large geriatric practice and I often encounter similar feet. The hallux varus was what I would consider to be...

RE: Drug-Seeking Patients (Bruce Lebowitz, DPM)

When a non-surgical patient asks for drugs, my answer is "NO". I don't give, nor do I need to give, any explanation and I am certainly not uncomfortable saying this. "NO" is self-explanatory. As for post-op requests, you are the best judge of when to reduce or cut-off drugs. My answer to those patients is that I can no longer give them narcotics and they need to take Aleve or ibuprofen.

First, you need to tell the patient you cannot advise them to take a spouse's pain medication. Second, inform patients that there is concern that their pain will be more difficult to manage post-operative if they start taking strong pain relievers now. Third, if their symptoms are that severe, there is nothing wrong with referring them to a pain clinic. Fourth, you may want to mention that the pharmacy/DEA frowns/watches physicians who prescribe schedule pain medication to patients who are not under the care of a pain management physician or being treated post-operatively. I would add, "and you don't want to get me in trouble, right?" (they always agree).
You cannot deny that a patient is having pain. If there are no contraindications, consider NSAIDs or Ultram, along with a referral to pain management.

RE: 104 Applicants Not Matched for Residency Positions

My suggestion to partially solve the problem would be to double the existing residency positions at each program and divide the salaries between the residents. If we all remember, about 15 to 20 years ago, podiatry residency positions did not pay even half of what they do now. Since the standard of living expenses has not increased by 50% from then, it makes some sense.

We are ignoring the gorilla in the room. That is the RIGHT to practice without a residency. We have forgotten that we are not a surgical specialty. We are a general specialty for the foot (and leg) which has the ability to sub-specialize, should the practitioner desire.
Any graduate DPM who has passed the national boards has the knowledge and skills to go directly into general practice. Evidently, not every graduate can get a post-graduate program. That may be something we have to live with for a while. The demand for dermatology and ophthalmology residencies is greater than the supply. So the unsuccessful applicants go into another specialty or into general practice. The same situation exists in dentistry. But I do not believe any MD or DDS is totally blocked from practice. And if they so choose, they can wait another year and apply again. BUT IT IS THEIR CHOICE.
To have graduate DPMs totally prevented from practicing because so many states have a post-graduate training requirement seems, to me, to be at least unethical, if not illegal. It is time for the APMA to get involved, with all the power and influence it can muster, to get the states to relax or modify that particular requirement.

I implore private physicians out in practice to take in a preceptor. I have done this through the AAPPM for the past two years. Preceptorships offer a great opportunity to help our recently graduated doctors while they wait out this morass and continue with their educational process. The preceptor experience has been a very pleasant and enjoyable experience these past two years. I will be offering two positions this year with a $2,000 per month stipend. I ask that each practitioner reading this post consider contacting AAPPM and hiring a preceptor.

The recent PM Quick Poll demonstrating that greater than 40% of podiatrists do not favor the training of an assistant for "routine foot care" suggests an answer to the residency shortage. Gather all future graduates in a room. Have all those interested in nail and callus care stand off to the left. Have those on the right given a residency. Have those on the left given a chiropody degree. There, problem solved.
Dental assistants, PAs, NPs, EMPs, all render independent or loosely supervised care sans residency. Let's be realistic. Other than for the purpose of licensure requirements, why would you require a residency for "routine care"? Here is podiatry's dirty little secret; there are many podiatrists for whom "routine care" is the overwhelming substance of practice and income. Or are you telling me that the schools are incapable of teaching this basic skill? I just can't wait to read the intellectualization clarifying to me the complexities of "routine care."

RE: The Passing of Jack Morris, DPM
"You got to know your anatomy!" probably was Dr. Morris' best mantra. One time, while I was in biomechanics class at CCPM, he said plantar fasciitis could be fixed with a ball-peen hammer. 15 years later we had ESWT- a ball-peen hammer is a lot cheaper, but insurance companies would not be involved. Podiatry has lost a biomechanics genius.
