MI Podiatrist Discusses Heel Spurs
According to Conway McLean, DPM, "Spurs are common but need not be a source of difficulty or discomfort. A bump of bone can sometimes be a tumor, but that’s a wholly different discussion. The most productive approach, when symptomatic, will generally be utilized by a physician specializing in care of that structure or region of the body. But there are going to be a plethora of different methods and varying benefits."
|
|
Dr. Conway McLean |
"Surgical removal can be a simple matter, depending on the location and type of spur, often performed through a tiny incision utilizing minimally invasive techniques. In contrast, when encased by the Achilles, removal means moving the tendon aside to get to the spur, demanding a more rigorous post-operative recovery. Operative care should be more about correcting the biomechanical issue with spur removal of lesser importance," says Dr. McLean.

Effectiveness of a Multidisciplinary Limb Preservation Program in Reducing Regional Hospitalization Rates for Patients with Diabetes-Related Foot Complications
This study evaluated the toe and flow model (TFM), a limb preservation program led by podiatric surgeons in Alberta, Canada, for its impact on hospitalization rates and length of stay (LOS) in patients with diabetic foot complication (DFC). Diabetes, a leading cause of non-traumatic lower extremity amputations (LEAs) in Canada, often results in diabetic foot ulcers (DFUs), a major cause of infection, amputation, and hospitalization. TFM has reportedly reduced amputation rates by 39-56%.
Conclusion - Despite similar demographics and healthcare systems, the TFM region benefited from a dedicated multidisciplinary program and comprehensive limb preservation services. The study shows that TFM effectively reduces hospitalizations and LOS for DFCs, with significantly better outcomes in the TFM region than in standard of care (SOC) regions.

6 Strategies for Back Office Billing
The front desk staff is essential for checking in patients, collecting co-pays, and verifying insurance. But there’s more to the story, and out of sight doesn’t mean out of mind. Much of the heavy lifting for billing and collections is done in the back office, so it’s even more important to have a deep bench of talent, says Tammie Olson, manager and coding and compliance strategist at Management Resource Group.
2. Reconcile encounter forms and bill claims daily
If there are any questions about the services rendered, seek clarification from the provider. Your billers should be knowledgeable about appropriate modifiers and when to use them. Make sure your billers are submitting clean claims. Clean claims get paid the first time around and stand up to a potential audit.

RE: The Passing of Allan Spencer, DPM
Allan Spencer, DPM passed away peacefully at the age of 93 on March 17th surrounded by his family. Dr. Spencer served in the United States Army during the Korean conflict. He then attended the Ohio College of Podiatric Medicine graduating in 1958. He went on to private practice and to teach at OCPM for 39 years, giving his students an understanding of biomechanics and his uniquely dry sense of humor.

Dr. Allan Spencer |
Dr. Spencer served as President of the American College of Podiatric Orthopedics (now ACPM). He lectured at a myriad of seminars on biomechanics of the foot and ankle throughout his career and influenced thousands of podiatrists.

From: Paul Kesselman, DPM
I read this recent article and had to chuckle a bit. Several years ago, I invited a representative of Noridian who was in charge of the CERT auditors to an APMA CAC meeting. At a pre-meeting the night before the CAC meeting, I spoke with her about her past work history and what prepared her for her current position at Noridian. He responded that she started as a customer service rep and worked her way up the totem pole to her current position. I asked her what in life prepared her the best for her position and she remarked that she went to culinary school and was a chef, but the demands of family life were just too difficult to work in the restaurant business. While most of the others involved in the conversation laughed, I was the only one who saw that her previous career was the perfect preparation for her current position.
As my wife, who is an excellent cook and baker, will tell you, preparation, logistics, and precision cannot be compromised when it comes to many aspects of cooking, but this is especially true in baking. The secret sauce of reimbursement is in documentation and ensuring that those who read our notes understand them in a sequential form and in a language that they understand. The notes are for them and not for us. Using your own shortcuts and skipping steps that are proven will ensure your cake or cookies will not rise. The same is true for documentation. So who is laughing now?

From: Amol Saxena, DPM, MPH
It sounds like next steps are to discuss things with ABFAS. I have previously written that a good immediate option is to have ABFAS issue the CAQ in the surgical discipline. This would be analogous to the MDs in how they view and issue CAQs: baseline board certification first and then sub-specialty (i.e., CAQ, which is in ABPM's bylaws).
In my lectures on "Podiatry, Prejudice & Possible Solutions" at various meetings and on PRESENT, I discuss options and show how American Indians think 7 generations ahead. My proposal above is for the short-term. Other options similar to dentistry, ophthalmology, and even cardiology exist but are never discussed in depth.
Since the AMA considers "the case closed" as to DPMs ever taking the USMLE and movement towards allowing physician and surgeon designation to be held only by MDs and DOs, our schools need to adapt and make podiatric medicine under the MD or DO umbrella. Foot and ankle surgery can then become a specialty like ENT or oral surgery with additional training. I’m happy to help bring stakeholders to the table and facilitate the discussion.
Amol Saxena, DPM, MPH, Palo Alto, CA
