Diabetics Need to Check Feet Daily: OH Podiatrist
According to Matthew Reiner, DPM, at ProMedica Physicians Podiatry, the human foot is also a barometer for overall health. Diabetics are advised to pay close attention to their feet to watch for potential health problems. “With long standing diabetes, if it’s not controlled, you can lose feeling in your foot,” said Dr. Reiner. “It can lead to open sores and to acceleration of the development of some foot deformities like bunions and hammertoes.

Diabetes can also affect the blood vessels of the entire body like the heart but also the foot, where the blood vessels aren’t getting the proper circulation and that can lead to problems. “If you have certain medical problems like diabetes or heart disease, I think you should check your feet every day to make sure there’s no open wounds.”

Query: Peripheral Nerve Stimulators
I have been paid on CPT 64555 but wondered about code CPT 64590. CPT 64590 is not even listed in the APMA Coding Resource Center. It would be great to get some input on these codes.

Response: CPT® 64590-INSERTION OR REPLACEMENT OF PERIPHERAL OR GASTRIC NEUROSTIMULATOR PULSE GENERATOR OR RECEIVER, DIRECT OR INDUCTIVE COUPLING.
The APMA coding resource center provides AMA CPT coding that is done by podiatrists. This is a CPT code used for gastric neurostimulators for sacral nerve stimulation for urinary and fecal incontinence. I do not see the connection to a foot and ankle medical reason you would be doing this. Do you? I did a search and could not find any relevant foot and ankle reason to be using this CPT code.

This is an interesting topic. First of all, we should all know that if we call a patient from a cell phone, they will most likely automatically have your cell number. Here is the problem: "Mr. and Mrs. John Jones" now has your cell phone number because you called them to see how they were doing post-op or you gave them the cell number to call in the event of a post-op emergency. They consider themselves "special" because they no longer have to go through your office staff to reach you.
Two months later, you are driving on the expressway and they call you on the cell phone, not for an emergency but to schedule a routine appointment and you have no way of documenting it as you are driving 65 miles an hour with both hands on the steering wheel. Another scenario is that you might be at a concert, a party, or someplace where you have to put the phone on mute and someone calls to schedule a routine nail clipping. They are upset because you did not pick up. Yes, in cases where I want a patient to be able to reach me in a hurry, I give them my cell number, but if they start calling for routine matters, I gently ask them to call the office during business hours.
