IA Podiatrist Uses NanoScope for In-Office Arthroscopy
 
Using a flexible camera smaller than a pencil, Kosta Antonopoulos, DPM, a podiatrist at UnityPoint Clinic Foot and Ankle, was able to treat Tanya McPeek's painful ankle in November in a procedure room right in his office. Rather than go back into the operating room for a second surgery, Antonopoulos suggested that McPeek have a minimally invasive procedure performed with the NanoScope, an operative arthroscopy imaging system.

Dr. Antonopoulos said he has been doing procedures with the NanoScope in the operating room for several years. But, in the last year, he said the procedure was granted in-office approval. He said the procedure room is made to be a sterile environment, just like the operating room. "The equipment's a lot smaller. In the next year, it's going to get even smaller," Antonopoulos said of the reason why such a procedure can now be done in the office. "Patients do so much better when it's in the office. They're not having to undergo anesthesia."

Medicare Claws Back Money for Amniotic Stem Cell Shots
 
Medicare is clawing back money it paid doctors for dubious birth tissue injections -- a move that likely could put some clinics out of business, an orthopedic surgeon told MedPage Today. In late February, seven U.S. regional Medicare Administrative Contractors (MACs) announced that they will seek to recover losses from claims going back more than 2 years, for shots that cost some $2,000 apiece. For clinics that injected patients in multiple joints with at least two doses per joint, that could total thousands of dollars per day.
 
"Any claims processed and paid for dates of service on or after Dec. 6, 2019 will be adjusted and payment will be recouped," stated the announcement from WPS Government Health Administrators, which covers the Midwest region. "Impacted providers will receive an overpayment demand letter identifying the amount of overpayment." The notice applies to amniotic, placental, and umbilical cord injectables, which the WPS notice describes as "experimental exosome biologic products that have not proven to be safe and effective for any medical use."

RE: The Passing of Mose A. Galluzzo, DPM
 
Dr. Mose A. Galluzzo, age 95½, of Rockford, passed away Sunday, March 13, 2022. He was a Veteran of WWII serving the United States Army in the Philippines. He received his Doctorate degree at the Illinois College of Podiatric Medicine. He had a private practice as a podiatrist for 45 years.

Dr. Galluzzo was instrumental in a landmark decision that established that orthopedists could not testify against podiatrists in malpractice cases in Illinois. His son Paul Galluzzo, DPM practices in Rockford, IL.

QUERIES (SPORTS MEDICINE)
Query: Achilles Tendon Injuries from Pickle Ball
 
As pickle ball becomes extremely popular, I am seeing more patients with injuries relating to the sport. Besides the typical ankle sprains, I have recently seen three patients with Achilles tendon injuries. MRIs revealed partial longitudinal tears in the tendon on all three patients. While waiting to get their MRIs, I have been dispensing 1/2" heel lifts bilaterally for them to use until I get the diagnostic results. If I have a patient with an Achilles tear, I refer them out to other podiatrists or orthopedic surgeons for continued care. Being semi-retired, I cannot do the proper follow-up needed for treatment of this condition.
 
Normal conservative treatment would be a CAM walker with heel lifts that are gradually reduced in a timely manner. I would appreciate some advice with a treatment plan that my colleagues are using to treat their patients with this type of injury. Please include treatment plans, time intervals especially with follow-up MRIs, and criteria followed if surgical intervention is needed.

Dr. Levy raises the question of peripheral neuropathy as a manifestation of dementia. Gait abnormalities may occur as an early manifestation of dementia and may precede the actual diagnosis of dementia. Typically, decline in executive function and gate fluidity are parallel. Noting a combined decrease in memory and gait speed are predictive of dementia. Patients with dementia are at 2–3 times the risk of falling and, when evaluating a patient with known dementia, it is important for the podiatric physician to perform a gait analysis and risk assessment for falling.
 
There is a spectrum of cognitive and mobility decline with neurodegeneration and aging. A simple get up and go test, or duel task testing, together with an easily performed mini-mental examination, can alert the patient or caregivers about early dementia, regardless of ideology.

There is no new thing under the Sun (Ecclesiastes 1:9). The person who invented the idea and the instrumentation for fat transplantation to the ball of the foot was Dr. Edward Chairman.  It is of note that NONE of the individuals who are making this out to be revolutionary have mentioned the person who originated the procedure...or that he abandoned it six months or so after its inception.
 
Dr. Chairman (one of my attendings when I was a resident at The Graduate Hospital in Philadelphia under Dr. Harold Schoenhaus) explained to me that once the fat cell is liberated from the panniculus, the blood flow is disrupted and that transplanted fat is quickly resorbed. None of the people who are currently pushing this have addressed the issue of disruption of the blood supply once violated from the panniculus; that is also curious.
 
Anyway, you are correct Dr. Roth. I don't see this having any better long-term results than those experienced by Dr. Chairman.

The lack of attention to this posting speaks clearly to where our profession has unfortunately placed itself. 15 or more years ago, this posting would have had a landslide of postings; today hardly a whisper. It would have been met with citations from papers presented at both local and national meetings. Unfortunately, that is no longer the case.
 
A few years ago, right after the APMA annual meeting, I posted a letter which addressed why at best fifty or so of the 1,000+ attendees came to a workshop hosted by several prominent DPMs dedicated to advancing biomechanics in the 21st century. Most of those attending this meeting were...
