NY Podiatrist Completes NYC Half Marathon for Cancer Charity
 
Rob Conenello, DPMpast president of the American Academy of Podiatric Sports Medicine (AAPSM) completed the United NYC Half Marathon held on March 18th. "This was a special event for me," Dr. Conenello said. "I was running for team Aktiv Against Cancer, whose mission is to use goal-oriented exercise as medicine for those struggling with the wrath of cancer."

ON THE INTERNATIONAL LECTURE CIRCUIT
CA Podiatrist Lectures at Biomechanics Seminar in New Zealand
 
Kevin Kirby, DPM lectured to approximately 100 New Zealand podiatrists at a podiatric biomechanics and sports medicine seminar this weekend at the Auckland University of Technology. Lectures included the topics of "Tissue Stress Theory", "Running and Running Shoe Biomechanics", "Longitudinal Arch Load-Sharing System", and "Pediatric and Adult Flatfoot Deformity". Workshops were also given by Dr. Kirby on modern podiatric examination techniques and gait examination techniques.

Dr. Kade Paterson (Australia) also gave a lecture on his fascinating research on "Patellofemoral Biomechanics and Foot Pronation" at the seminar. Even though New Zealand has only about 300 podiatrists, they are very enthusiastic about being on the cutting-edge of the latest research and theories on foot biomechanics and sports medicine and custom foot orthosis therapy techniques.

Drive Down Expenses at Your Medical Practice - Part 1
 
Start with the easy stuff first. First, determine is you are you part of a buying group (group purchasing organization or GPO). If you are not, you are likely paying much more than you need to on items such as medical and office supplies, medications, vaccines, and other services. There are many GPOs to choose from so start with your medical association or local chapter to determine which one(s) they recommend for your specialty.

Query: Billing a fracture treatment code and Unna boot or fiberglass cast application
 
When billing a fracture treatment code, are you able to also bill for an Unna boot or casting procedure? For example, a patient came in and the doctor treated the fracture and applied a fiberglass cast - Codes: 28475 and 29405 along with the casting material, both CPT codes with the same fracture diagnosis code.

Response: In my experience, an Unna boot is only covered for venous vascular conditions. I  never recommend using an Unna boot for fracture care because it most likely will never be covered for this situation. If you want to use it on the first treatment, it would be included in the fracture care.
 
CPT 29405 is a column 2 edit to CPT 28475, so on the first casting, technically, you are not to bill the casting as it is considered part of the treatment of CPT 28475 - Closed treatment of metatarsal fracture; with manipulation, each. On a subsequent visit, you can bill CPT 29405 along with the casting material.

RE: The Passing of Carl Kirschner, DPM
 
My dear classmate, Carl Kirschner, DPM, Class of 1960 at the New York College of Podiatric Medicine, died on November 6, 2017 at the age of 86. Carl was beloved by all of his classmates. With his robust sense of humor, he kept us all laughing throughout our four years.
Carl was fortunate to have been selected for the only hospital-based surgical residency available in the country at that time at Detroit Civic Hospital headed by Dr. Earl Kaplan, its founder.

Carl remained in the Detroit area creating a successful multi-practitioner surgical practice which he regretted having to give up due to debilitating back problems. Always with a positive view on life, Carl brought joy, love, and humor to all of those fortunate to have known him. He will be missed by us all.

QUERIES (EHR)
Query: Creating Your Own EHR
 
With Practice Fusion going to a fee subscription model, I am curious if it is feasible and legal to create your own EHR by creating templates for documentation and storing them on some secure cloud server if you are not interested in meaningful use and MIPS, etc. I like the neatness and accessibility of an on-line medical record but am otherwise dismayed by the clunkiness and slow speed of Practice Fusion, and don't feel I want to commit to a contract on promises of improved features. Any thoughts and opinions?

Dr. Williams, I must sorely disagree with your premise statement with respect to the theme of this article and how the patient was “victimized” by the physician assistant and the pharmacy. This is nothing more than the “drive by media” producing a sensational “fake news” story on how big pharma is destroying healthcare by “overcharging” patients for medication that they may or may not actually need. 
 
The author of this article makes the reader believe that this patient who seemingly is a well-to-do retiree based on her aforementioned resident geographic locale on “Capitol Hill” was taken advantage of without...

The story as referenced by Dr. Williams is not about a compounded drug, but is about  Kerydin, which is a brand name drug widely available for the treatment of onychomycosis. It is applied topically and it became available at roughly the same time as Jublia, another topical antifungal. These medications are not only expensive but if used properly, only last a month. What is even worse is that the clinical success data provided by these companies does not rate them as panaceas for the treatment of  fungal nails. They are by no means gold standards. 
 
In our practice, after fungal testing, we might prescribe these medications, but only if the patient's insurance company combined with company incentives make them affordable. If the patients are going to have to pay over a thousand dollars for a month's supply, we will offer them a choice of several new antifungals which not only can be dispensed from the office but have been shown to be clinically effective.

While I was one who did grind mycotic nails and did not have any ill effects, I totally agree with Dr. Udell. I often dispel such false arguments with the theoretical tale of "I always cross the street without looking and I haven't been hit by a car yet." The lack of a disastrous outcome in any given case does not negate the need for reasonable caution.

I too grind nail and have done so for 39 years. I do use a vacuum system and after each debridement, I wipe the nails with alcohol. I now find if I wipe the nails down with alcohol before I grind them, there is hardly any dust.
