PODIATRISTS IN THE NEWS
FL Podiatrist Discusses Morton's Neuroma

According to Stephen Kushner, DPM, "Morton’s neuroma is often caused by shoes that are too tight or that have high heels. These shoes can cause the nerves in your feet to become compressed or irritated. The irritated nerve thickens and gradually becomes more painful as a result of the pressure on it. Another possible cause of Morton’s neuroma is a foot or gait abnormality, which can lead to instability and can also put pressure on a nerve in your foot. Many common foot deformities like flat feet, bunions, and hammertoes are often present when Morton’s neuroma is diagnosed."

Dr. Stephen Kushner

"If your pain persists, your doctor may try injections of corticosteroids or anti-inflammatory drugs into the area of pain. A local anesthetic injection may also be used to numb the affected nerve. That may help relieve your pain temporarily. Alcohol sclerosing injections are another remedy that may provide short-term pain relief. And finally surgical excision is an option, however not the primary treatment," says Dr. Kushner.

PRACTICE MANAGEMENT TIP OF THE DAY
Six Ways to Motivate Your Team for Free - Part 2

Fortunately, for practice managers, often the best motivators are relatively easy, inexpensive, and often a whole lot of fun. Try a few of these ideas at your practice, and see if your team is more willing to go the extra mile.

2. Stay on Mission

Make sure your team is aware of the mission,” says Cristy Good, CMPE, Senior Industry Advisor at MGMA. When your team is aware of the larger goal that their work is a part of, they’ll feel an investment in the organization and have a reason to work hard to achieve that organization’s goals.

QUERIES (NON-CLINICAL)
Query: Covid Talkaholics?

Has anyone experienced this issue related to the COVID-19 pandemic? Some patients who have not been anywhere for many months and their only excursions outside of their homes are to the doctors have come down with this new addiction I like to call, "Talkaholic". They just want and need to talk and ramble extensively because they have a need for some form of social interaction that has been lacking. If so, how do you  handle this?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: By the Year 2025: A Commentary on Podiatric Medicine in the Foreseeable Future
From: Leonard A. Levy, DPM, MPH

By the year 2025, when currently enrolled podiatric medical students will be at the beginning of their residency, the need for podiatric physicians will be ultra-acute with one of every five Americans being 65 years of age and older. Each of these people has an average of five chronic diseases, most with disorders having pedal manifestations, stressing podiatric medicine as well as the entire healthcare system. The current epidemic of diabetes, for example, with its devastating complications in the foot will be paled by the much wider distribution of that disease in 2025. Degenerative arthritis, especially prevalent among the elderly, will run rampant, so frequently a prime cause of podiatric medical problems.

In addition, any suggestion to shorten the required period of residency will be retrogressive, the antithesis of achieving parity with allopathic and...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Ivar E. Roth DPM, MPH

As one of America’s first podiatrists to go fully concierge, I think I can weigh in on this topic with authority. Additionally, as a pioneer in concierge medicine, I was also the chapter president of the American Academy of Private Practice Physicians in Orange County, which recognized me as an equal to the MDs. The first three years after I dumped all insurances including Medicare, my income dropped around 70%. Those were trying years but I knew I had to do what it took to break away from the insurance addiction. After about 5 years, my income was back to the previous level. The key to going concierge is to excel at what you do and offer excellent customer service. But that is NOT enough. I ended up reinventing myself and my services a few times.

Now I offer unique services, including Saturdays, that patients cannot receive anywhere else. There is a good demand for my services and the demand is growing rapidly. The future is bright for the concierge model and those who are willing to take the risk and benefit from the reward. It was not an easy road but at the end of the day, I would not do it any other way. I am my own boss, I depend on no one, and NO ONE dictates to me how to see patients or run my office. Podiatry is great and wonderful and I hope my colleagues will someday embrace this model.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2A
From: Brian Kiel, DPM

There may be a "climate" of gender equality but that does not mean there is equality. There is a "climate" of racial equality but climate does not mean equal treatment. Dr. Mendoza, as a woman, is much more able to validly claim discrimination without written substantiation (which of course does not exist). There is unequal treatment of people due to gender, race, or religion. To deny that this occurs is a result of the "privilege" of being a member of a class that does not have to be concerned about discrimination.

RESPONSES/COMMENTS (NEWS STORIES)- 2B
From: Billie Bondar, DPM

During a residency interview, I remember a doctor handing me a sheet containing sexually "humored" questioned. I sat and read them and then gave it back, saying “this is not okay.” I then walked out.

RESPONSES/COMMENTS (NEWS STORIES) - PART 3
From: Ben Pearl, DPM

I encourage those that want to see the full context of what Golden conveyed in the interview to watch excerpts that have been posted on YouTube. Golden was not satisfied by the analysis of shape of footprint in the sand as a way of determining running shoes selection. He also was not satisfied with using the filming of a runner from behind done at many running stores as a method of running analysis. In the interview, he expressed the need for side and front views as well.

As far as shoes go, there are some that have higher durometer insoles such as the Provision and Paradigm. The more “minimalist” Altra is the Escalante. Two main differences in the Altra shoes are the fore/aft relationship as there is no elevation (0 drop), and the extremely wide forefoot and toe box areas. The best way to check these shoes out is to try them on at workshops and demo sessions at an AAPSM meeting, (once they cease being virtual), or try them at local running stores to feel the differences in the line and models yourself. As Benno Nigg, PhD has suggested, many times the best shoe and or orthotic combination is the one that feels best after repeatedly running over several miles.
