ACFAOM Grants Honorary Member Status to UT Podiatrist
On Friday March 15, Dr. David Edwards received a certificate memorializing that the American College of Foot & Ankle Orthopedics & Medicine (ACFAOM) Board of Directors had unanimously voted to extend Honorary Membership on his ascension to the presidency of the APMA.

Jack, I faced the same dilemma about eight years ago. I contacted our State of Florida Podiatric Licensing Board and was told it was not necessary. I contacted my professional liability insurance carrier and they were more than enthusiastic that it was not necessary. So, I have not had an active DEA for about 8 years. I could not tell you the last time prior to then that I wrote for a narcotic, but I do know that since I discontinued my DEA license that I have no longer been called by patients in acute pain that could “only be resolved by Oxycontin, etc.” Save yourself time and money, but for peace of mind call the Licensing Board and your insurance carrier to get their 2019 opinion.

On the surface it may appear that unless you write prescriptions for narcotics, spending close to a thousand dollars to renew a DEA number seems unnecessary. The problem is that certain states will require a DEA number in order to prescribe other non-narcotic medications. At the SAM meeting, one of the speakers said that in the state of Florida, one must have a DEA number in order to prescribe Lyrica. Double check this if you practice in Florida. In New York, Ketamin trochies require that the prescriber have a valid DEA number. The bottom line is that you should survey what medications you prescribe and then ask the local pharmacy whether you need a DEA number to prescribe them in your particular state.

This actually happened to me and one of my staff members many years ago. There was this elderly man who engaged in inappropriate touching with a female assistant, particularly in a small whirlpool room where she offered him assistance removing his shoes and socks. She came to me after a number of incidents and while it made her uncomfortable, she tolerated it because she did not want to lose a patient for me. I was pleased that she brought it to my attention and I told her so. My immediate response was "I have many patients, but good assistants are hard to find. So, if one of you has to go, it has to be him."
I gave her two options: 1) Since she was mature enough and self-confident enough to speak for herself, I would allow her to speak to him directly and let him know that his behavior was unacceptable and would no longer be tolerated. 2) Alternatively, if she wanted me to speak with him, I would do so and transmit the same message.
In either case, if the patient was lost, so be it. My memory is that she chose the first option and I don't accurately remember whether the patient remained in the practice, but I think he did.

I love what I do and look forward to going to work every day as a podiatrist. Some may find otherwise as this writer has. Just to toss this out, I have a friend, a podiatrist, who teaches anatomy part-time at a local city college to medical assistant students and nursing students. I know he could go full-time if he wanted.
I am sure the opportunities are, perhaps not endless, but numerous. I agree with the advice from Dr. Block to “follow your passion”. Let your imagination flow free and I am sure there are many places who would need you to teach some subject or other that you are already partially or well versed in.

CODINGLINE CORNER
Query: Denial L1970
We recently received a denial on a claim for L1970 stating that documentation does not include measurements that custom casting was performed. We have never included this documentation when ordering or billing for this brace. The CPT Description the payor (UHC) put on their denial for L1970 was “MOLDED TO MODEL ANKLE J” and for the L2330 was “LACER MOLDED TO PATIENT” which of course does not match the CPT code description in our system or other referencing tools on these codes.
Can anyone explain why they might be requesting these measurements? Our molds are a cast that is formed to the patients foot/leg.

Response: Perhaps they wanted you to include a copy of the lab order (in Medicare parlance this might be a detailed written order). Typically this would include certain measurements such as height from floor to fibular head, height of brace, width of forefoot, circumference of the ankle width at malleoli and circumference at the top of the brace. While these would be reflected in the cast, at times the cast may not extend proximal enough or it may extend too far proximal. Your casting note should also include information on how you casted (e.g. wt bearing, semi wtbearing, etc.).
