Amputated Diabetics Have Worse Prospects Than Cancer Survivors: IN Podiatrist

A diabetic whose limb is amputated has a survival rate lower than that of most people who are in remission from cancer, Dr. Justin Hudson said. Hudson, a podiatrist who recently began practicing at Indiana University Health Morgan Hospital, wants to prevent the need to amputate feet and legs before it gets to that point.

“There are a lot of diabetics who don’t have access to proper wound care and they are the ones who end up having amputations,” Hudson said. “A lot of what we do is see diabetics before they have problems. If they are low risk, we like to see them once each year. If they are high risk, we want to see them every 60 days. We fit them with diabetic shoes with special inserts. If they have corns or calluses, we trim them down.”

NJ Podiatrist Performs Heimlich Maneuver to Save Woman's Life

It was just a Saturday night dinner for Maryland psychologist, Dr. Ellen Lent, until a piece of food got caught in her throat.  What had been an enjoyable dinner quickly became a life-threatening event. While her husband frantically tried slapping her on the back, Dr. Lent turned blue and someone yelled, "Is there a doctor in the house?"

Fortunately, Dr. Sharon Root, a New Jersey delegate to the APMA House of Delegates was dining nearby and rushed to the rescue. Root performed the Heimlich manuever, and the offending  food popped out. A grateful Lent wrote a thank-you note to Dr. Root saying, "Thank you from the bottom of my throat."

MEETING NEWS - PART 1

APMA House of Delegates Convenes in Washington

The House of Delegates (HOD) of the American Podiatric Medical Association formerly opened today on an emotional note with a tribure to the late Irvin O Kanat, DPM. For over half  a century, Dr. Kanat was an institution at the HOD. By resolution, Dr. Kanat will be forever remembered along with podiatry legends Drs. William Stickel and Seward Nyman in the silent prayer that concludes all HOD meetings.

Other highlights of the first day were guest speeches by Ray Stowers, DO, American Osteopathic Association president, Alison Wishart, Chairman of the Society of Chiropodists and Podiatrists, and Joseph Stern, DPM, President of the Canadian Podiatric Medical Association.  Awardees included: Distinguished Service Citation - Harvey Lederman, DPM, Award of Excellence - Stuart Wertheimer, DPM, and Rising Star Award - Alyssa Stephenson, DPM.

St. Louis Podiatry Seminar Celebrates its 25th Year

The St. Louis Podiatry Seminar celebrated its 25th year of podiatric education with a record attendance. The meeting, held in St. Louis annually, presented sessions with discussions on dermatology, forefoot surgery, musculoskeletal disorders, reconstructive ankle surgery, edema, pediatric disorders, and practice management.

QUERIES (CLINICAL)

Query: Long-Term Pain, Right Foot 

A 20 year-old patient reports that about 4 years ago, he sustained second degree burns (from a hot grease spill) which did heal. As a child, he  had surgeries on both feet for club feet. He currently reports a constant burning pain, deep pain, and shooting pain on the right foot. Sitting/lying down is not as bad, however, once he is on his feet for about 5 minutes or so, the discomfort becomes severe and he has great difficulty ambulating.  The pain is generalized, but mostly around the talonavicular joint. No radiating pain is noted. 

Chronic Pain in Right Foot

An EMG nerve conduction study revealed a right common peroneal nerve problem. The feet are very rigid and have a cavus deformity. ROM was 0 at the ankles and subtalar joints bilaterally. The great toes had minimal ROM. He had extremely atrophied musculature in the lower legs. Semmes-Weinstein 5.07 monofilament feeling was absent on the sub-third and sub-fifth metatarsal heads, lateral arch, and on the heel right foot. The patient had normal Semmes-Weinstein 5.07 monofilament on the left foot. Light touch was diminished on the right. No allodynia was noted even on the burn sites. He has been unsuccessfully treated with various pain medications, including narcotics. X-rays reveal an obvious dislocation of the navicular.  I’m looking for ideas on how to treat this patient.

RE: Forefoot Surgery on an 84 Year Old (Joseph Borreggine, DPM)

Although I feel the procedures you have chosen may be indicated, the likelihood of providing long-lasting relief is small weighed with the risks of performing that much surgery and the post-op recovery time on an 84 year old.

With a chief complaint of bilateral second hammertoes, I simply suggest disarticulating amputations at the second MPJs. Flexor tenotomies could be considered for the remaining lesser claw toes. Both feet can be done concurrently, ambulatory at once, and both of you will sleep better at night

Without seeing any x-rays, here are my thoughts, based on my experience. Check for equinus and fix it, if needed. There is no need to repair the plate if doing Weil (see article by L Yao, 1994 AJR). Then handle all else via minimal incision technique. Soft tissue rebalance as needed from lateral to medial, then address the hallux varus last, as needed, keeping in mind that fusion or tightrope technique are good options.

There are two things of importance biomechanically. There is a met adductus, reducing the true IM. That will prevent you from closing the IM adequately. There is periosteal thickening of the 2nd and 3rd mets, correlating to the flexible forefoot FFT. I wonder if her callus pattern confirms this (2 or 2-3)?

Consider “pushing” or “cheating” an Austin bunionectomy, and try to get as much plantarflexion from the osteotomy as possible. Consider shortening the 2nd or 2nd-3rd mets or MISing them with a post-orthotic, prn to adjust the met pressure parabola and prevent transfers.

Kevin Kirby, DPM recently shared an article with me in which he was interviewed by Andrew Tilin about minimalist and barefoot running for an article in Outside Magazine. Christopher McDougall, an advocate of minimalist or barefoot running was quoted in a section of the article which read: “I get adamant and foamy mouthed about this.” He says, “Podiatrists aren’t research scientists. They’re chiropractors for the feet. When you look at how the intellectual manpower is dividing up, the doctors and PhDs are on the side of minimalism. They’re the greatest minds in the business.”

Not only was I astounded by Mr. McDougall’s lack of knowledge about the scope of modern-day podiatry, I was surprised by his generalization about the status of opinion of the greater scientific and medical communities with respect to running. I was all the more taken aback by Mr. McDougall’s comments after reading the following near the top of the page about barefoot running on his website: “I can’t prove this, but I believe when my runners train barefoot, they run faster and suffer fewer injuries.”— Vin Lananna, Director of Track and Field for the University of Oregon and seven-time NCAA Coach of the Year. Needless to say, this wasn’t the scientific evidence I had expected to see given Mr. McDougall’s comments in Outside Magazine.

I offer my congratulations to CPME for making a serious effort to birth new residency positions for graduate podiatrists. I know this is not easy, as I was once a residency director (for an existing one) myself. But there is still the issue of defunct programs. Even if you have been in practice for years, and your program/preceptorship no longer exists, you can be seriously affected by this issue.

At the present time, there is no mechanism for dealing with defunct residencies/preceptorships. Also, since some of these programs have been historically run through the aegis of individual podiatrists, some people have been DENIED their certificate and have no possibility for redress. Before you think that this can't happen to you, I will give you...
