2025 APMA House of Delegates Convenes in Washington, DC
The 2025 APMA House of Delegates (HOD)convened in Washington, DC, on Saturday morning. The HPD is the governing and legislative body of the organization with representatives from each of APMA's 53 component associations. Saturday's session began with a special message from Karen Santi, the wife of APMA President Larry Santi, DPM. Dr. Santi was unable to attend the house due to a recent stroke. The entire APMA community sends its well wishes to Dr. Santi and his family during his continued recovery.

Brooke Bisbee, DPM, was installed as the 99th APMA president by Past President Lloyd Smith, DPM. Dr. Bisbee acknowledged several issues facing the Association, including a declining membership, and challenged the members present to be part of the solution. "I ask that every member join me and be part of the solution," she said. In her first state of the Association address to the House of Delegates, APMA CEO and Executive Director Meghan McClelland, MBA, outlined her vision for the association and moving podiatry into the future. Rob Frimmel, DPM, a long-time APMA and Florida Podiatric Medical Association member, was honored with the Award of Excellence on Saturday.

NY Podiatrist Receives Distinguished Service Citation
On Sunday, Barry Block, DPM, JD, was awarded APMA’s highest honor, the Distinguished Service Citation. This award is given in recognition of outstanding sustained accomplishments on the national level in scientific, professional, or civic endeavors. Dr. Block has fostered an unparalleled platform for conversation and collaboration within the profession. For more than 30 years, PM News has published the news, debates, and discussions that have shaped our community. The PM Hall of Fame contributed hundreds of thousands of dollars to the APMA Educational Foundation and student scholarships. As editor of PM News and Podiatry Management magazine, Dr. Block has further distinguished an already noteworthy career.

Dr. Block served in the APMA House of Delegates for more than three decades, sitting on the Resolutions Committee and Resolutions Review Subcommittee for many years. He also served APMA’s Membership Committee and was general editor of the Journal of the American Podiatric Medical Association. Dr. Block is the founder of the APMA affiliate, the American Podiatric Medical Writers’ Association. In nominating Dr. Block for this award, Bret Ribotsky, DPM, wrote, “his commitment to advancing podiatric medicine is evident in every aspect of his work and has left an indelible mark on our community.”

Podiatrists Prominent on Newly Formed ASMST Board
Amol Saxena, DPM, MPH and Marc Lederman, DPM were elected Secretary/Treasurer and Executive Director of the new American Society for Medical Shockwave Therapy, a multidisciplinary society which had its inaugural meeting in Boston, MA March 7-9, 2025 in conjunction with the International Society for Medical Shockwave Therapy.

The new board also includes Jay Spector, DPM and Liz Bondi, DPM. Ludger Gerdesmeyer, MD, and Adam Tenforde, MD also serve on the board. Attendees from the meeting were allowed to take the ISMST Certification exam which is the same for all practitioners, regardless of degree.

Congratulations to my friend Paul Kesselman. Paul is a quiet, non-assuming professional who doesn't brag about his accomplishments. He simply continues his passion of informing podiatrists of the many different aspects and the changing environment that constantly occurs with billing - coding and related aspects.
As pointed out nicely in the post by Bruce Kaczander, DPM, Dr. Kesselman has undoubtedly helped countless numbers of podiatrists across the country for many years in this important aspect of medical practice. He has, in my opinion, been under-recognized for his contribution to our field. The readers of PM magazine got it right in giving him this award for the Hall of Fame. Well deserved. Expect to see more to come. With gratitude and a strong congratulations.

The question of who is responsible for thromboembolic episodes in the podiatry patient is an important one, with reference to patient safety and secondarily malpractice allegations. DVT and pulmonary embolism are a major cause for alleged medical negligence against podiatric physicians. Dr. Chaskin posits, and Dr. Udell comments, on the suggestion that the physician who "cleared the patient" is responsible for the thromboembolism. I believe the problem is a bit more complex.
Many physicians maintain the position that they do not "clear" a patient for surgery, but rather "risk stratify" the patient, as the need for surgery and the decision to proceed with surgery are generally that of the surgeon. Under certain circumstances, in theory, an argument could be advanced that the "clearing physician" may be partially to totally liable for an episode of DVT/pulmonary embolism. As a hypothetical, you might refer a patient with known Factor V Leiden mutation or...

The on-going dispute by a small number of podiatrists seeking the ability to perform their own medical clearance for surgical intervention in hospitals has always baffled me. I fully support increasing education and increasing the diagnostic ability of our graduates to diagnose systemic diseases affecting the lower extremity and having knowledge as to how to manage them as well. But at the end of the day, treating these medical conditions is out of our scope of practice... and by all means should be. We do not have the medical education that an allopathic physician has.
Until that day arrives, we should not be performing medical clearance for conditions we are not licensed to treat. Additionally, there is a concern with the business aspect of providing medical clearance for our surgical patients, as this increases our malpractice risk and raises the cost for all of us trying to provide good quality medicine in the challenging business environment. We certainly do not need increased cost in this regard.
At the end of the day, it is best for the patient and best for our practice to have medical clearance provided by allopathic physicians who are licensed to treat and manage these problems when they arise. It is for that reason that they also assume the medical liability in providing this clearance for our patients--and we should be thankful for them for doing this.
