PRACTICE MANAGEMENT TIP OF THE DAY
Six Ways to Motivate Your Team for Free - Part 1

Fortunately, for practice managers, often the best motivators are relatively easy, inexpensive, and often a whole lot of fun. Try a few of these ideas at your practice, and see if your team is more willing to go the extra mile.

1. Name Names - It may seem obvious, but many leaders miss it: Know everyone’s name. “One of the best ways to have a motivated team is when the doctor knows everyone’s name, and says thank you,” says Elizabeth Woodcock, president of Woodcock and Associates, an Atlanta-based physician practice consulting firm. Money talks; it’s true. But people are far more motivated to do their best work when they love their jobs, have fun at work, and know that the leadership notices and appreciates what they do. It’s not that hard to give them that.

CODINGLINE CORNER
Query: Clinically Diagnosed Stress Fracture

I clinically diagnosed a metatarsal stress fracture for a Medicare patient. There was no visible abnormality on x-rays. I dispensed a CAM walking boot as part of the treatment. Will this be covered or should this boot be self pay?

Troy Harris, DPM, Jacksonville, FL

Response: You can check with your carrier about what are the allowable conditions for the use of a CAM walker. Fractures are definitely covered but there are multiple reasons for immobilization which are also covered such as tendon or joint issues. Your chart note will defend the need for immobilization in an audit or appeal situation. Stress fractures are tricky but your chart note could indicate consideration for a repeat x-ray in 10-14 days or maybe an MRI. This goes into decision-making. It's also good medical-legally to have a battle plan laid out. Your clinical suspicions can change over time–there is nothing wrong with that.

RESPONSES/COMMENTS (CLINICAL)
From: Howard Dananberg, DPM

Both hallux limitus (HL) and turf toe (TT) do not necessarily have their etiology in direct joint damage, but instead to repetitive strain secondary to chronic inhibition of the peroneus longus. To those who think that this is not the case, I ask how often eccentric strength of the peroneals is examined as part of a comprehensive assessment of either HL or TT?

Muscles protect the joints about which they function. Without performing an evaluation to the muscular group known to resist 1st metatarsal dorsiflexion during the 2nd half of the stance phase of gait, differentiating the etiology from symptom is simply not possible. Muscular inhibition is a well described medical condition, but this knowledge has unfortunately...

RESPONSES/COMMENTS (NON-CLINICAL)
From: Robert Kornfeld, DPM

Dr. Bregman’s words will fall on deaf ears. I’ve been writing about this right here on this forum for 20 years. Even back then, doctors were being abused by insurance companies. The difference is that I did something about it. I dropped every contract I had and became a cash practice. I urged readers on this forum to unite against this abuse and do the same. Unfortunately, I became an army of one. The revolution he calls for isn’t going to come from the public.

As long as doctors participate (i.e. cooperate) there will continue to be more guidelines with lower fees. If doctors refuse to accept insurance, then and only then, will things change. If you make their product worthless, they cannot sell it. As you already know, bitching and moaning about it doesn’t create change. You’ve played into their hands and they’re laughing all the way to the bank.

Robert Kornfeld, DPM, New York, NY

In today’s climate of gender equality/equity, can Dr. Mendoza’s claims of gender discrimination in podiatry residencies be substantiated?

RESPONSES/COMMENTS (NEWS STORIES) - PART 2A
From: Eddie Davis, DPM

The running shoe industry has been able to save money on shoe manufacturing by moving away from structured shoes to shoes with soft single material midsoles. The industry goes through great lengths to justify why they have done so, basically telling us that what we know about biomechanics does not matter. Golden Harper, the creator of Altra shoes was interviewed by Ben Pearl, DPM. Harper stated, "Pronation, supination, and stability categories were not relevant in determining the proper running shoe." Harper described his shoes as being designed more for the natural way that we run.

What is the "natural way that we run?" Does placing a pillow under our feet not change the way we run? Are we to assume that the way each person runs is biomechanically efficient and does not lead to pain nor pathology? I think we know the answers to those questions. Hype from the companies selling overly soft, unsupportive, and unprotective shoes is not something we need accept.

Eddie Davis, DPM, San Antonio, TX

RESPONSES/COMMENTS (NEWS STORIES) - PART 2B
From: Ben Pearl, DPM

Regarding my recent Interview with Golden Harper, there was a question by Dr. Steinberg  as  to his degree so I feel obliged to respond. Sometimes people get caught up in life and adventure and do not update various profiles publicly available such as Linkedin. It is best to check more than one source as vigorous journalistic and scientific principles demand. In a 2015 article, Y Magazine, off the Brigham Young website, Golden Harper is listed as a BS (2009) in exercise physiology.

I was particularly impressed with Golden’s search for knowledge irrespective of what a study might say about the benefits of a particular concept related to his commercial interest of selling a running shoe.

The study of biomechanics continues to be at times an elusive and controversial area due to the inherent problems of isolating a particular variable, capturing longitudinal data, and repeating trials. One of the things I learned at NIH and from my father, who was a grant administrator there, was to line up your ducks before you cleared your potential grant applicant lest you both end up with egg on your hands.

RESPONSES/COMMENTS (OBITUARIES)

The passing of Morris Prigoff is very sad news as we hear our friends and family members pass away due to illness or even injury. Morris will always be remembered as a fun loving guy and the podiatrist who found lockers and garages as a unique hobby that also became somewhat of a business.

He was a dear 1966 classmate of mine at OCPM. We had many good times together over the years and it was very sad that he had such a bad illness that lasted so long. My deepest sympathy to his family.
