WesternU Dean’s Lecture: Redefining the Role of Podiatric Medicine
Podiatric physicians and surgeons are trained alongside their allopathic and osteopathic peers, have proven to be equal in their ability to care for patients, and should be licensed accordingly. That was the message from Franklin J. Medio, PhD, keynote speaker for the fourth annual Western University of Health Sciences College of Podiatric Medicine Dean’s Distinguished Lecture March 13, 2019. Medio, who is president of Consulting Services for the Health Professions, presented “The Recognition of Podiatric Medicine as a Medical-Surgical Specialty: Breaking Down Barriers through Education and Training.” Medio said he does not consider podiatry a separate profession from allopathic and osteopathic physicians. Dentistry, pharmacy, and optometry have unique qualities and are different professions, he said. But podiatrists perform a complete history and physical examination of patients. They order lab tests. If a patient has a medical problem that’s outside of their scope, they call for a consultation. “They manage patients just like any other physician. I don’t see the difference.”

The main difference is that podiatric medical students do not take the United States Medical Licensing Examination (USMLE) or the Comprehensive Osteopathic Medical Licensing Examination of the United States (COMLEX-USA). Podiatric medicine and surgery is not recognized by the LCME (Liaison Committee on Medical Education), by COCA (Commission on Osteopathic College Accreditation), or ACGME (Accreditation Council for Graduate Medical Education) as a clinical rotation, Medio said.
WesternU CPM is already taking steps toward having its students earn a Physicians and Surgeons Certificate, which will require approval from the Medical Board of California. CPM students will sit for the Comprehensive Basic Science Examination beginning this year and for the next two years. “Then we will assess and evaluate it. That will help us determine their readiness to sit for the USMLE. Once we are able to demonstrate proficiency at the initial level, then we will be able to take it further,” said CPM Interim Dean Lester J. Jones, DPM, MS. “The whole key is that the four-year academic environment here at WesternU sets the stage for our students to have the knowledge, skills, and attitudes necessary to be able to sit and be successful at each of those stages of that type of testing structure.”

The Hatton Garden ringleader known as “Basil” has been jailed for 10 years – four years after carrying out the £14 million heist. Alarm specialist Michael Seed, 58, is believed to have let himself in to the building in London’s diamond district using a set of keys before defeating the security system.

Gait expert Dr. Gordon Burrow compared covert footage of Seed with CCTV images of Basil, disguised in a ginger wig, facemask, and hat as he carried a black bin bag to and from the scene to obscure his face from the cameras. The podiatrist told jurors the “unusual” limp offered “strong support” for the prosecution case that Seed was Basil.

Analysis Supports 5-10 mm Margins for Excision of Melanoma-in-Situ
A real-world analysis of the treatment of melanoma-in-situ supports the latest guidelines recommending 5 to 10 mm surgical excision margins but also highlights the role of clinical discretion in individual cases and the need for more research on this topic. The study was presented in a poster at the 2019 American Academy of Dermatology Annual Meeting, which took place from March 1 to 5.
The investigators analyzed margins for excision at < 5, 5 to 7.9, and ≥ 8 mm based on commonly used values in the literature. The primary endpoint of the study was histological clearance. Overall, 127 lesions from 117 patients were included in the analysis of lesions that were treated with surgical excision. Most (81.89%) were located on the trunk and extremities. Lesions treated with larger margins had lower rates of clearance. Clearance rates were 100% for lesions treated with < 5 mm margins, 96% with 5 to 7.9 mm margins, and 93% with > 8 mm margins.

Query: Is a DEA Number Always Necessary?
I just received notice that my DEA number is up for renewal. I have done research and have even asked some pharmacists about the need for having a DEA number. The short answer is that if you do not prescribe narcotics, a DEA number is not necessary. My NPI number is all that is really needed. I am semi-retired and do not have the need to prescribe pain medication. I do not do surgery anymore and only use NSAIDs or other non-narcotic medications for pain. What is the general consensus as to the need for having a DEA number? Is there anyone else who still practices and prescribes medications without a DEA number?

Query: Using a DPM Degree Outside of Podiatry
What else can I do with a “DPM” degree besides practice clinical medicine? Where else is the DPM recognized outside of our community? I have a bachelors degree in biology and then I went to podiatry school right out of undergrad. I’m feeling that practicing and seeing patients is perhaps not for me—at least full time. One of my true passions is to teach. I’m interested in seeking teaching opportunities at some of my local colleges and universities. Anyone else care to share their experience?

Editor's comment: You only go through life once, so you should always pursue your passion. Many academic institutions from high school level (NYC Public Schools, for example) on accept the DPM as post-graduate training.

I had a patient who liked to hug (never “grabbing” or “touching” my staff). I took it as his being a sweet old man and thought nothing of it. One day, this got brought up in conversation with a lawyer friend. He told me that if he makes them feel uncomfortable, I know it is going on and I do nothing to prevent it, that is sexual harassment. The next day, I asked my staff their opinion on the patient. It turns out, they all thought he was creepy and they tolerated him because he was a patient. Needless to say, he was discharged that day and my staff appreciated knowing that they and their well-being came before patients.
In your case, that patient needs to be discharged at the least and a police report filed, if warranted.
