OH Podiatrist Provides Tips to Prevent Foot Pain When Working From Home
Foot pain is on the rise with more people working from home. One of the main reasons behind this trend is that a lot of people don’t think to wear shoes inside the house. So, what should you be wearing on your feet at home to prevent foot pain? “While working from home, a type of shoe you could wear would be an athletic shoe with an arch support and a deep heel counter, which prevents your foot from rolling to the inside when you’re walking. This support helps with what we call pronation so that your foot doesn’t fall to the inside when you walk. A slipper with arch support can also be a great option,” explained Joy Rowland, DPM, a podiatrist with Cleveland Clinic.

Dr. Rowland said it’s crucial to protect your feet from hard surfaces like wood and tile. People who opt to go barefoot at home are walking directly on their bone structure with no shock absorption, which can ultimately lead to foot pain. Rowland noted you could even wear out what’s called the fat pad on the bottom of your feet over time, possibly leading to long-term pain.

NY Podiatrist Discusses Specialized Socks for Diabetics
“Diabetic socks are primarily designed to protect the feet, usually with a low-profile seam to reduce the chances of disrupting the skin of the feet, a moisture-wicking material, and often a relatively loose-fitting design to prevent constriction,” says Bruce Pinker, DPM, of Progressive Foot Care.

Dr. Pinker recommends socks made at least partly from a synthetic fabric, like polyester, acrylic, lycra, or spandex, adding that some very well-designed diabetic socks contain antimicrobial qualities to further ward off bacteria.

NY Podiatrist Discusses Best Footwear for Teachers
Teachers are on their feet all day, and having the right shoes is essential to survive long days of grading homework assignments and quizzes, hands-on lessons, recess duty, and parent-teacher meetings. For those with plantar fasciitis, board-certified podiatrist Jacqueline Sutera, DPM recommends choosing a shoe with "arch support, cushioning, and a thicker, shock-absorbing sole."

Dr. Sutera notes that some other styles to avoid include flat, thin and worn-out shoes. "Also, it is a good idea to alternate shoes throughout the week and day, if necessary," she adds. Wearing the same shoes all day can cause injury and atrophy. Additionally, she suggests to "avoid walking barefoot when at home and wear supportive slippers or recovery slides" after a long day of standing and running. According to Sutera, walking barefoot can cause foot pain, inflammation, plantar fasciitis, back pain, tendonitis, and other ailments over time.

IA Podiatry Bill Spurs Pun-Filled Debate
It was an unusual feat — nineteen puns and corny references during three minutes of debate in the Iowa House. Representative Austin Harris of Moulton kicked it off on a bill dealing with the licensing of podiatrists. “To ensure podiatrists start off on the right foot, this bill would allow for podiatry students to apply for a license after two of their three years of residency,” Harris said. “This would put podiatry on the same standing as other medical doctors.” Representative Austin Baeth, a doctor from Des Moines, rattled off a dozen more puns. “This bill will ‘heel’ our lack of podiatric access in this state,” Baeth said. “I urge your ‘arch’ support,” Baeth said, as other members in the House laughed.
Speaker Pat Grassley jokingly intervened. “If they keep behaving like that, I believe Representative Windschitl is going to have a place for his foot moving forward,” Grassley said, prompting more laughter. Undeterred, Harris continued. “Thank you to my colleagues for not ‘de-feeting’ my amendment,” Harris said, to more laughter and some groans. “This is a terrific bill that gives podiatrists a foot in the door to starting their careers…I encourage my colleagues to follow in my foot steps in supporting House File 635.” The bill passed the House unanimously.

Query: ABNs
We are having a discussion in our group about ABNs and routine foot care (RFC). We are struggling to come to a consensus if or when they should be obtained. Is it suggested that we get an ABN at the first visit for RFC? Should we get an ABN once a year? Should we get an ABN at every encounter? And are ABNs needed for Medicare Advantage plans or do they apply only to traditional Medicare?

Response: ABNs are only for traditional Medicare, not Medicare Advantage or other payers. I find that many doctors overuse ABNs. They are not needed for qualifying routine foot care, typically. They are not needed for non-qualifying, routine foot care because such services are statutorily non-covered and ABNs are never needed for statutorily non-covered services. ABNs are appropriate for an otherwise covered service where such may be denied because Medicare deems it to be not medically necessary (note the difference between this and statutorily non-covered).
Example: your patient comes in for qualifying routine foot care, but comes in after only 45 days instead of the required 61. The service would normally be covered but after only 45 days, Medicare deems it to be not medically necessary. So you use the ABN to let the patient know that Medicare may deny payment and the patient could be responsible. You also put a GA modifier on the claim to let Medicare know there is an ABN signed.

Dr. William M. Davis, of Dawson Springs, passed away Mar. 4, 2023, at the age of 80. He worked in a variety of jobs in his younger years, in a silver mine and on a lake freighter, enlisting in the Army and serving in Viet Nam. Then he worked as a zookeeper at the Brookfield Zoo, and studied dragonflies and starlings.
