ABPM Executive Director to Step Down
 
The ABPM Board of Directors announced that its Executive Director, Marc A. Benard, DPM, will be stepping down from the position effective December 31, 2018 and continuing thereafter as a special consultant to the Board of Directors. An Ad Hoc search committee has been put in place to conduct the process to identify appropriate candidates to assume the Executive Director position on January 1, 2019.

Dr. Benard became a diplomate of the ABPM in 1982 and the ABFAS in 1983. In 1985, he was elected to the ABPM Board of Directors. From 1986-1991, he was a member of the ABPM Examination Committee, serving as its Chair from 1987-91. In 1992, he became a special consultant to the Examinations Committee. In 1996, Benard assumed the role of Executive Director of the organization and has served in that capacity to the present time.

Pool Renamed for CT Podiatrist and English Channel Swimmer 
 
Marcella A. MacDonald, DPM, who has swum the English Channel 15 times, the most of any American, grew up on Manchester’s West Side. Although she moved to Andover, her name will live on in Manchester — the Board of Directors voted to rename the West Side Pool in her honor: the Marcy MacDonald Pool.

Dr. MacDonald, a podiatrist who has an office in town, said she was honored to hear the news, which she said was a 'total surprise.' “I’m still very shocked and pleased,” MacDonald said this week. “I’m very honored. It’s a nice, special thing.”

Even at a tech conference, Martin Pricco, MD, president of Gould Medical Group in Central Valley, CA isn't afraid to name one of the major culprits of physician burnout. At Gould Medical Group, Pricco wasn't content to let EHRs get in the way of physician contentment. The group looked at five areas where they could reduce frustrations with the EHR:

For any message that comes from a patient to the physician, make sure it's pooled to the correct resources. If nurses and medical assistants can read and answer that question before it gets to the doc, Pricco said this will go a long way in alleviating physician stress. Also set up protocols so all of the pertinent information is included and there's no back and forth between the doc and patient.

Query: Coding Severe Equinus Procedures
 
I need help with coding for a paralytic equinus procedure. I performed an Achilles tenotomy along with a posterior ankle capsulotomy/release along with a posterior subtalar joint/release. Any coding recommendations?

Response: I recommend one code which, I believe, is meant to cover these procedures. CPT 27612 arthrotomy, ankle, with posterior release, with or without Achilles lengthening.

This temporarily disabled podiatrist neglected to post in which state he practices. Workers Compensation laws vary from state to state. In the state of California, an injured worker has the right to request a qualified medical evaluation from one of a panel of three specialists in the relevant specialty. Once he is declared "permanent and stationary" by a QME, he may be allowed to return to his regular occupation with modification, which could include the use of double-action forceps. Future medical care may also be included.

Most important here is that state Medicaid plans are free to enforce their own rules regarding the need for Surety Bonding for DME Suppliers. Fortunately, physicians under Medicare were able at the Federal level to obtain an exclusion and do not require a Surety Bond under Medicare. That exclusion appears to not be in place in Texas. I urge you to contact your state component (TPMA) to:
 
1) Inquire if your state component as well as those representing MDs/DOs and orthotists/prosthetists are subject to the same exclusion rights we have under Medicare. Based on your scenario, shoes would not be the only exclusion. Acute care needs, such as CAM boots, crutches, surgical dressings, etc., would also be in the mix. The same rationale used for exclusion from Surety Bonding with Medicare at the Federal level should be invoked with your state Medicaid program. The state associations should contact their national associations for assistance.
 
2) Your state component should be able to provide you with guidance on whether you can charge a Medicaid patient for the payments they would otherwise be paying to a Medicaid provider. Each state has different rules on this.

In practical day to day experience, the idea that grinding nails can make you sick is absurd. Do I have a respiratory problem? Does the 71 year old who sold me my practice and never used a vacuum have a problem? Do I know of any podiatrist anywhere who has a respiratory problem? The answers are no. Perform all the analyses you like, but it is simply not a problem if you use basic common sense, which we all do. By the way, did you ever look closely when a mycotic nail is cut with a nail cutter? There is a spray of fine particles in all directions.

I agree with Dr. Hofacker. I too grind mycotic nails when applicable. When I can, I prefer terbinafine to clear them but there are that group of patients who have long-standing mycotic and dystrophic nails or simply dystrophic that cannot fit in a shoe and have a potential risk factor for ulceration and loss of toes. Don’t minimize that situation. How many older patients do we each have with PVD or other medical problems precluding removing 1 or 10 nails permanently and yet cannot fit in a shoe without risk.
 
I grind them. I do have a vacuum system. I use re-usable burrs. After each use, they are soaked in cold sterile solution, then ultra-sounded in more cold sterile solution, wire brushed if necessary, and individually packaged and autoclaved. They ARE sterile with each use. I ask Dr. Albright, what she does with that population of patients I have discussed above who cannot take terbinafine, or who cannot reasonably be expected to respond to terbinafine (not to mention the year-long process orals require), who cannot sustain an onychoplasty and yet, cannot fit in shoes.
 
In my practice population, I see a fair number of these patients. And I am trained to treat them well. Do some simply shrug their shoulders and turn these patients away? “Sorry Charlie”? It is a serious question that I feel those who will not grind should reply to as there is always another side to this equation.
