PA Podiatrist Provides Foot Care to the Homeless
As he tended to a patient with battered feet, Neal Kramer, DPM cracked jokes to help him relax while also providing him with the tools to fight a fungal infection. Thursday night, Kramer saw René, a resident of the Allentown Rescue Mission’s emergency shelter, who was battling a number of foot ailments. When René left the examination room, he had topical cream and petroleum jelly, clipped toenails, and a better understanding of how to take care of his feet.

For the last four years, Dr. Kramer, 74, has provided free treatment, consultations, and nail clippings to the men taken in by the Allentown Rescue Mission. Kramer, a Bethlehem resident and Lehigh Valley native, has worked as a podiatrist for about 40 years but said the time he spends at the mission as well as the semi-regular paid shifts he does with Lehigh Valley Health Network’s Street Medicine Program, are some of the most rewarding work he has done.

5 Things to Consider When Selling Your Practice
5. Provide traditional marketing services.
One option is to sell the practice and turn it over to the new owner, who has to introduce himself to his new patients and to other members of the medical community. As part of the sale, the established physician adds value to the sale if he/she stays on for 90 to 120 days, and introduces the new physician to the patients; and with the new physician, visits practices of referring physicians and other important members of the medical community, as well as key hospital administrators.

Jack M. Schreffler, 90, of Glen Mills, went home to be with his Lord on Sunday, February 20th, 2022, at Penn Medicine Hospice at Chester County Hospital in West Chester. He graduated in 1956 as a podiatrist from the Temple University School of Podiatric Medicine in Philadelphia.

He was a successful podiatrist in Springfield, PA for over 40 years and was once the president of the Pennsylvania Podiatric Society. Jack was the first podiatrist to practice at Fair Acres in Delaware County and was one of the founders of the Temple Foot and Ankle Institute in Philadelphia.

Query: PRP and Compounded Medications
Has anyone successfully billed insurance for PRP treatments of chronic wounds? My compounding pharmacy will only provide compounded medications to patients (but not the clinic) to be used on patients. Does anyone have recommendations on where to obtain compounded medications to be used in the clinic?

I switched to Sammy a couple of years ago. I was very satisfied. Modernizing Medicine has purchased Sammy and things have gone downhill. We do not get adequate support. We use Sammy for billing and their system stopped billing Highmark advantage programs correctly 6 months ago, and they still can't solve the problem. It seems that support is often clueless about many things we ask them.
As Dr. Morelli stated, you pay the full amount for each doctor but all doctors share the same software. In other words, the software is customized for each doctor but all the pre-built documents are shown on all users' software, leading to unnecessary huge menus to scroll through. It's really not a great program for a multi-physician office.

RE: Medicare Advantage Plans

I would like to share one of my experiences with a Medicare advantage plan. I received a request for a refund from one of these plans. It was a legitimate request, as 2 years ago I used an incorrect code on a hospitalized patient. I contacted my provider representative for this plan. I told her about this request.
I informed her that I would be happy to refund the money, if they would pay me the money for cutting toenails and calluses on the same visit. She asked me to send her examples of these denied claims. I sent her six claims. That was four months ago. I have not heard anything about this refund to date.

The measure of a successful doctor is NOT how hard he or she works or how many patients are seen daily. It is how smart you work! I left the insurance model and opted out of Medicare over 20 years ago and have been working direct pay ever since. The surprising reality is that I have decreased from 60 patients/day to 25 patients/week.
I have no stress, no staff, and still make enough to pay an ex-wife, all my bills, have the social life and traveling that I love and still have money to invest. I’m debt-free and I am not beholding to Medicare or any insurance company. Yes, it can be done.

Dennis, you’re not the only one. The company that pushes the subtalar artheroresis implant is so wrong when they say that orthotics will not help. Since using a specialized orthotic, my surgical load with treating pediatric flatfoot has dropped 98%.
While it is less financial remuneration for my practice, I know that my patients are being better served. More of our podiatric brethren need to re-examine the current world of orthotics.
