PODIATRISTS AND THE CORONAVIRUS PANDEMIC
Pandemic Has Created Foot Problems: NC Podiatrist
 
Women who’ve given up their high heels during the pandemic probably have happier feet. But people who constantly walk around the house barefoot or in socks risk developing foot problems like heel pain and tendinitis, said Jane Andersen, DPM, a podiatrist in North Carolina and a spokesperson for the American Podiatric Medical Association. Unchecked, simple issues such as tendinitis can develop into more-severe conditions like tendinosis, which is more difficult to treat, Andersen said.

Those with diabetes have more to worry about when it comes to foot issues during the pandemic and should be checking their feet every day, Dr. Andersen said. Wearing shoes at home to cushion the feet and provide arch support can help keep your feet healthy. House shoes also protect your feet against injuries like stubbed toes. Broken toes are something that Andersen and her colleagues have started seeing as a result of people being barefoot in their homes.

VA Podiatrist Interviews Golden Harper
 
Ben Pearl, DPM recently interviewed Golden Harper, a running expert and creator of Altra shoes, this week on his podcast Fit Foot U. Harper challenged the conventional process of running shoe selection. He stated that pronation, supination, and stability categories were not relevant in determining the proper running shoe. He described his shoes as being designed more for the natural way that we run.

Also discussed was the optimum running form, which he stated was best exemplified by Kenyan elite runners. This technique was described as not only being more efficient but easier on the knees. Students from Temple University’s sports medicine club participated in the podcast.

6 Questions to Answer Before Preparing 2020 Taxes - Part 5
 
As you begin to pull together your tax information for 2020, answer these 6 essential questions to potentially reduce your taxable income 2020 tax liability.
 
5.  Did you qualify to claim any tax credits?
 
A tax credit is a dollar-for-dollar reduction in your taxes due. Tax credits are not the same as deductions. In fact, they can be far more beneficial than deductions since deductions only reduce the amount of your taxable income while credits directly lower your total tax liability. A $20,000 tax credit, for example, would save you far more than a $20,000 reduction in your taxable income.

Query: Investigating Previously Dispensed DME
 
Does anyone know of a way to check to see if a patient has been dispensed a DME within the last five years? Specifically, if the DME was dispensed by another doctor?

Response: The only way I know of is to check the provider portal. This will show any DME that was provided by any provider.
 
The only possible glitch is if the DME was dispensed by a provider from a different MAC. So if a patient received a DME product in New York two years ago and then moved to Texas, that will not show up in your portal. But it will show up at the CMS portal so payment would be denied even if your portal gave you the all-clear.
 
Bottom line: when in doubt, get an ABN.

QUERIES (CLINICAL)
Query: Green Toenails
 
The patient is a 72 year old male who came to the office complaining of 10 green toenails. He has no history of new green interior shoes or green socks. He had been using topical Kerasal nail renewal 2x daily and states green color appeared subsequently. Certainly not pseudomonas nor candida. Not removable with alcohol swab. The green is easily removed on left foot with rotary sanding disc.

Before treatment, I suggested he wait until after St. Patrick's Day. Anyone else see this colorful side-effect? Kerasal’s main ingredients include propylene glycol, urea, lactic acid, sodium hydroxide EDTA. I personally use this product with no ill effect.

Considering the description of the patient, and the photos, I think that you may be dealing with erythema nodosum. You might want to try colchicine. The location on the foot is a little bit unusual but those lesions really look like erythema nodosum. This is probably a chronic case.

We tend to view hallux limitus and turf toe as being painful because it moves. I instead suggest that the joint hurts because it DOESN'T move when the motion demand is the greatest. It is also important to keep in mind that once the hallux makes ground contact, it is completely stationary until toe-off. Restriction to motion is in the metatarsal component of the joint, not the toe.
 
The use of carbon fiber plates and other stiffening devices can never resolve the symptoms in the long term. This will only serve to increase stiffness and perpetuate the underlying mechanics of the problem. They may be temporarily helpful for short-term relief of a true joint sprain, but otherwise...

RE: APMA House of Delegates Passes Emergency Resolution on CPME 320 Residency Training Standards

I’m proud of the APMA House of Delegates on passing Emergency Policy Proposition 5-21 by a supermajority of 83% and I applaud the California Podiatric Medical Association who sponsored the resolution. This is a very strong recommendation from the profession that CPME adopt residency training standards which bring us closer to parity with MD/DO standards.
 
The public comment period on CPME 320 ended on 2/15 and the major stakeholders have yet to see the those comments be released by CPME in order to complete their duties of collaborating on the standards. But I am hopeful CPME will listen to the many comments and review the multiple PM News quick polls in order to revise the draft to meet the needs of the public, the profession, and the medical community.
