CA Podiatrist Launches "Fashion with Function" Shoe Collection
Inspired by Victoria Beckham's recent announcement that she can no longer wear her signature sky-high heels, California-based podiatric surgeon Dr. Joan Oloff has decided it's time to start a true footwear movement for women, where the long held belief in "fashion over function" is replaced by a newfound belief in "fashion with function", and it has been a long time coming. In 2013, she founded Joan Oloff Shoes, a brand built upon the simple desire to improve women's lives and save their feet.

Dr. Oloff may not be the first person to have spoken out about the idea that women don't have to suffer for style, but she's certainly the most experienced. In fact, she has been surrounded by feet and footwear her entire life, having spent her formative years working in her father's shoe store before following her own path and becoming a renowned podiatrist and podiatric surgeon. As a doctor, Joan Oloff has treated countless women and an exceptional number of professional female athletes, whose careers and livelihoods are very much dependent on the state of their feet.

Jon Hultman: To get the MD degree, we would have to convert podiatry schools to medical schools and they would have to be accredited by LCME. None of our schools want to do this. The reality is that the end product of our education and training is indistinguishable; thus the license a DPM gets should be the same as the license an MD gets.
The barrier we hit with a plenary license today is not how medicine is practiced; each provider specializes in an area of medicine, and the CMA (California Medical Association) realizes this. What we want is our license to be equivalent regarding payers and other physicians; and that it's known that we have a license based on a review of strong educational parameters by credible educators who have looked at our training and have agreed that this should be our license.

Med Students Demand an End to Pricey Patient Care Skills Exam
Thousands of medical students across the country are demanding the elimination of a licensing exam that tests their patient care skills, arguing that it’s an expensive waste of time. Led by students from Harvard Medical School, the effort has drawn strong support nationwide. More than 6,000 students, residents, and faculty at 130 medical schools have signed a petition calling for the National Board of Medical Examiners to abolish the test.
The target of their anger is the Step 2 CS, a role-playing test that asks students to examine and diagnose patients or actors pretending to have specific conditions and fill out their medical charts. It’s meant to test their ability to interview patients, do physical exams, and explain their findings. The exam is only offered in five cities: Atlanta, Chicago, Houston, Los Angeles, and Philadelphia. That means many students need to spend hundreds of dollars on flights and hotel accommodations. The registration fee for the exam itself is $1,275.

RE: Surviving a Meaningful Use Audit

I have important information for subscribers participating in meaningful use. I am now in my second consecutive audit. I was told by Dr. Brody, who is representing me, that I may need proof that I contacted the immunization registry and the syndromic surveillance department and show rejection letters. When I called for help from syndromic surveillance, they told me absolutely not to attempt to set up an account if I will not be submitting data.
I didn't listen to them and signed up. Surprisingly, there are check off boxes to indicate which data you will be submitting. So, in our case that would be the specialty registries. For the immunization registry and syndromic surveillance, just state “exempt”. Go to the DOH website and sign in. For help, call 877-646-5410 option 3. Do not let them tell you not to apply. Just get help in registration. Then they will email you your confirmation, and that's your ticket. Both immunization and syndromic surveillance are both listed on the confirmation.
May you never be audited by these ____ auditors, but if you are, be prepared. They require information that is totally unnecessary. Don't they know we don't administer immunizations or treat the flu? Why would we require proof to that effect?

RE: Diabetic Shoe Denial

We have taken the time and effort to fight every therapeutic shoe denial from Medicare. They seem to "up the ante" with each denial. Every time we change our forms or get more signatures, they seem to want some other minute detail. It is obviously not about patient care! Recently, our second level denial was due to them not accepting a physician's note that was written by their PA and countersigned by the MD.
To quote "co-signing the document does not meet the policy article requirement for criterion 3. The in person visit must be preformed by the certifying physician." If a PA can do everything in an office, under the supervision of an MD, how does this make sense? Does anyone have any experience with this or feedback? These denials, for the most minor of issues, come after the service has been rendered and the costs can not be recouped. They are killing the benefit.

I respectfully disagree with Dr. Zlotoff's coding reply for a plantar plate repair. He opined for either using a flexor tendon repair code with the osteotomy code, or an unlisted code. In actually, when a CPR is correctly performed, one preserves the long flexor tendon. It is carefully retracted away from the plantar plate prior to the plate's incision. Therefore, that code is inappropriate. I'm unsure about my colleague's experiences using unlisted codes, but remuneration takes forever and only after multiple appeals. No thanks! I avoid them if a suitable code exists that reflects the work I perform.
So, when we think of the pedal pathology and the surgical repair, don't almost ALL plantar plate tears ultimately lead to...

These are the types of posts that the profession reads which lead to so-called illegal "unbundling" acts of the CPT-related codes. Is there a global code for these two procedures? What is the authoritative source which truly provides the answer, and not just a response to these related type questions? Why wait to find out until a federal court and another paid expert tell you that the answers are not so clear, or until you are the one "sitting in the hot seat".

Just as Labcorp does, we require all of our patients to put a credit card on file (CCOF) if we are to bill insurance for them. We have required this of all patients for just over two years. The only patients who do not put a credit card on file are those self-pay patients who pay in full at time of service.
Using a feature in our practice management system (Athenahealth), once each year, we swipe the patient’s credit card. Athena’s credit card partner (Elavon) stores the credit card information securely. The patient signs a one-year contract that gives us permission to bill that card for any outstanding balances. Dr. Ronald Renzi has some great and not so great advice for us regarding vascular testing. We use a hand-held Doppler unit from Summit that produces wave forms on select arteries. This, in addition to other history and physical examination maneuvers, gives us a better understanding of the arterial supply to the patient's foot.
However, please be aware that Medicare does not pay for screening exams. If you perform a vascular test on EVERY diabetic, it can be construed as a screening process. When I purchased a Doppler unit, I took a course in its use. They told us of a diabetic clinic which used in on all of their patients and were forced to return all funds. That is not to say you shouldn't use it on anyone you feel would benefit - just don't bill for it every time.
So, I agree with Dr. Renzi that this is a great tool, but if you bill for its use, remember:
1. Document the need (diabetes alone is not documentation)
2. Be sure your unit can produce a hard copy of the patient's wave forms.
