Overuse Injuries More Common as We Age: TX Podiatrist
According to Kenneth Cornell, DPM, a podiatrist at Austin Regional Clinic in Austin, Texas, adults are generally more likely to have foot issues — including foot pain — as they get older. There can be several causes, but the most common is overuse and the way your feet change over time. "As we age, our feet tend to have less elasticity — our arches become weaker and tissue becomes less elastic," he says. "When there is less elasticity, you're more likely to suffer arthritis or overuse injury and, therefore, foot pain."

How to fix it: As with any type of persistent pain that's difficult to manage, talk to your doctor regardless of whether or not you've already been diagnosed with a foot injury or condition. You may ultimately need foot surgery if pain from overuse persists, but your doctor can help you figure out the right treatment plan.

5 Things to Consider When Selling Your Practice
4. Remain as an employed physician.
Part of an agreement to purchase your practice may be the preference of the buyer (particularly in sales to hospitals) to have you remain at the practice as an employed physician. There are now two negotiations to consider:
• The sale of the practice
• The employment agreement.
When the hospital becomes the owner of your practice, there is no guarantee that the future performance of the practice will provide you with the same rewards as when you were the owner. We have witnessed hospitals doing a weak job of billing and collections, resulting in reduced collections and weakened net revenues. We suggest negotiating for a compensation plan based on your performance not the performance of the new owners. The model for a fair compensation plan is one based on relative value units (RVUs).

Query: ICD-10 and Liver Function Testing
When ordering liver enzyme studies prior to prescribing Lamisil or after prescribing that drug, what diagnosis code do we give the lab?

Response: When ordering liver function tests (i.e. LFTs) prior to prescribing oral terbinafine, the most appropriate ICD-10-CM code to provide the laboratory is B35.1 (onychomycosis). After all, that is the reason why you are planning to prescribe terbinafine and that is what specifically terbinafine is indicated for. However, a secondary ICD-10-CM code that would be indicated is Z13.818 which is defined as: Encounter for screening for other digestive system disorders. The reason that LFTs are ordered prior to prescribing oral terbinafine is to rule out the presence of any issues in the liver which could possibly be compounded by the terbinafine.
As far as ordering LFTs once the patient has been taking terbinafine regularly, the primary ICD-10-CM code would once again be B35.1. The most appropriate secondary ICD-10-CM code would be R94.5 which is defined as: Abnormal results of liver function studies. That is the reason why the subsequent LFTs are being ordered.

In his recent post, my esteemed podiatric colleague, Dr. Simmons, asks a wonderful and very simple question about what podiatrists (and other healthcare providers) are supposed to do when record inflation hits us from all sides and we cannot collect more from Medicare and other health insurance payors. Fortunately, this simple question has a simple answer. You can choose to opt out of Medicare and other health insurance programs and increase your fees when your expenses increase the way most businesses do. I did this 10 years ago and never looked back! Of course, you must follow certain laws and rules to do this legally and properly if indeed you elect to take this step.
I saw a wonderful lecture some time ago by a neurologist named Lawrence Huntoon, MD, who began his talk on opting out of Medicare by asking two excellent questions: 1 Have you had enough yet? 2. Do you believe that Medicare is like The Hotel California (where you can check in but never leave)?
Dr. Huntoon ended his lecture with the following words: “There’s a certain comfort in dealing with the devil, you know. The unknown is uncertain, so you embrace the status quo. But those who are brave and principled at heart, will receive FREEDOM as their reward, and that’s very smart.”

Our practice (7 DPMs) uses Practice Fusion for EMR and Medent for management. This involves extra work for staff as we maintain 2 separate scheduling softwares but it works for us.
We avoided what we believed were less expensive EMRs (although Practice Fusion was free at the beginning but is very reasonable today) and looked at what we thought would be around in 10 years after switching to Medent in 2000 when our previous software company did not have a reasonable plan for Y2K. This has worked for us since 2013 with only minor issues.
