NJ Podiatrist Discusses Orthotics
Orthotics are shoe inserts that provide comfort and relief from foot pain and help to prevent future injuries. According to Adam Kaplan, DPM, a podiatrist and CEO of Arcus Orthotics, "they are an affordable and non-invasive way to address common foot issues including heel pain, plantar fasciitis, bunions, flat feet, knee pain, back pain, and more."

Orthotics come in all shapes and sizes to fit any foot type, concern, activity, and shoe. They are made to be slipped into your shoes for a little extra cushioning or support, but not all orthotic insoles are created equally. Over-the-counter insoles that you can get at your local drugstore are a good place to start when trying them out, but there are also numerous benefits to investing in custom orthotics. "While over-the-counter orthotics are prefabricated, custom orthotics are designed for each individual and their specific issues and diagnoses. No two orthotics are alike," says Dr. Kaplan.

NY Podiatrist Comments on Using a Running Shoe for Walking
Is it ever okay to wear a running shoe for walking? While it’s not ideal, it makes sense for some people. So, if you have found that walking in running shoes works for you, take necessary precautions to avoid injury. You can add arch support to your running shoes; it may help prevent arch pain that develops as you walk over a period of time, says Casey Ann Pidich, DPM, a podiatrist at Park Avenue Podiatry Care in New York.

However, when it comes to wearing minimalist sneakers for walking, Dr. Pidich says it should be avoided at all costs. She believes these types of running shoes offer very little of the shock absorption you need for walking. “For example, Vibram Five Fingers or Merrell Vapor Gloves may work for runners who are forefoot strikers but will not offer enough support for basic walking,” she explains.

4. Do you qualify for the Qualified Business Income (QBI) deduction?
If you’re an independent contractor or a locum tenens physician, there are several things you can do to claim deductions against your income, one being the possible deduction for Qualified Business Income (QBI). As long as the total taxable income claimed on your 1099 is not derived from restricted income sources, you may be able to deduct 20% of your QBI from your total taxable income. There are a number of restrictions and qualifications that accompany this deduction, so you’ll want to consult with your tax professional and financial planner to see if this option applies to you.

Query: Random Bruising on Feet
A 30 year old female presented with excruciating pain to her heels that runs up her calves. The pain would keep her up at night crying. She is otherwise healthy but 2 years ago, she was having pains all over and was eventually diagnosed with a connective tissue disorder, non-specific, by her rheumatologist. He put her on plaquenil. Because of all the pain she is having, throughout the years, he sent her to 14 different specialists. She doesn't have pain like typical plantar fasciitis or tendinitis or nerve pain like tarsal tunnel. Her feet are cold and changes color. It was thought that she has Raynaud's but her rheumatologist later dismissed it because of the pics she just sent me of the random bruising she would get. These are not necessarily the painful areas. They just appear out of nowhere.

Initially, I prescribed her nitro paste which helped her cold feet, but not the pain. I prescribed her a steroid pack. She said that was the only thing that seemed to have helped her pain and was a great relief to her, but about a month later, the pains came back with a vengeance. Any ideas of what she might have that would give her random bruisings? I'm planning to give her another steroid pack but to consult with her rheumatologist on maybe a more long-term steroid regimen.

RE: President Biden to Include Podiatric Physicians Among COVID-19 Vaccinators

During his address on March 11, 2021, President Joe Biden said he will expand the number of people providing COVID-19 vaccinations in the United States including a wide swath of medical professionals. He said dentists, EMTs, midwives, optometrists, physician assistants, podiatrists, respiratory therapists, and veterinarians will become qualified to administer the three federally authorized COVID-19 vaccines.
The U.S. Department of Health and Human Services will establish a website to allow these clinicians to sign up to administer shots. Eligible vaccinators will also include medical and nursing students, the president said. While this recognition is welcome, DPMs are already qualified to administer intramuscular medication. In addition, podiatric medical students should also be eligible vaccinators.

In our practice, we see a fair amount of turf toe and hallux limitus with both younger patients and older patients. Over the years, we have tried to find a good solution for my patients but each individual has their own special circumstances. Currently, the best results we have found are with carbon plates, steel plates, or even a moleskin turf toe strap. These usually do the trick and give patients the comfort they hope to receive. These plates even work great on broken toes as they eliminate the motion in the forefoot. The plates stiffen the sole of the shoe.
My “go to” manufacturer is Dr. Jill’s Foot Pads as they make all of the above items. They have a new item, a turf toe sock, which seems to be working great which has a built-in splint.

I understand this has recently been discontinued due to an overwhelming amount of protest.
When I was in practice, I used to download the PDF to a folder in my computer and then could do anything I wanted with the file. Are those who are faced with this now also being charged simply to download the file? If so, that too is outrageous. I have only one Yiddish word to describe UHC (which owns Optum) and this is chazer (pig!). This brings to mind the old southern expression: Pigs get fed, but hogs get slaughtered.
If Optum thinks they are going to take over healthcare, I believe they will be faced with overwhelming protests (this recent one was just the tip of the iceberg), which could ultimately lead to their own destruction.
