TLC, the channel that brought you Dr. Pimple Popper has recently introduced a new series called My Feet are Killing me. In this series, patients of Brad Schaeffer, DPM and Ebonie Vincent, DPM undergo surgery to have their feet radically transformed and their lives changed forever.

Dr. Schaeffer who practices in Piscataway and Hillsborough, NJ was recently featured on the Titan Games, where he became a Titan and reached the semi-finals. Dr. Vincent, who practices in Orange County, CA, graduated from Des Moines University College of Podiatric Medicine. She went on to complete a 3-year residency program at Inspira Health network in Vineland, NJ.

ON THE LECTURE CIRCUIT
Inaugural Copeland-Meier Visiting Professorship Held
 
Recently, the University of Cincinnati held its inaugural Copeland-Meier Visiting Professor lecture with Dr. David Armstrong as the featured lecturer. This is only the 11th Visiting Professor lecture named at UC since its inception, and the first for podiatry at a major academic medical center. The lecture was established to recognize the successful partnership between podiatry and vascular surgery, which includes collaborative residency programs.

The Copeland-Meier Visiting Professor lectureship is the first for podiatry at a major academic medical center. In 2007, Dr. George Meier, Director of Vascular Surgery asked Cary Copeland, DPM to establish inpatient podiatric services at UC and work in conjunction with vascular surgery. Dr. Copeland recruited 9 community podiatrists, but also requested that a residency program be established which subsequently obtained CPME accreditation in 2010. Today, the University employs 4 full-time podiatrists with a robust residency program accredited for 2:2:2 of PGY-1, PGY-2 and PGY-3 in addition to an externship program taking up to 3 students per month.

Query: Diagnosis Codes and CPT 11042
 
We are getting denials for CPT 11042 for toe pressure ulcers. I cannot find a diagnosis code that is appropriate. Anyone have any insight?

Response: In a non-diabetic, the appropriate code for a pressure ulcer of a toe should be selected from this list, depending on the Grade of the ulcer:
 
L89.890 Pressure ulcer of other site, unstageable
L89.891 Pressure ulcer of other site, stage 1
L89.892 Pressure ulcer of other site, stage 2
L89.893 Pressure ulcer of other site, stage 3
L89.894 Pressure ulcer of other site, stage 4
 
If the patient has diabetes, you should code it as a diabetic foot ulcer, even if pressure is contributing.

RE: The Passing of J. David Cartwright Sr., DPM
 
Dr. Cartwright earned his degree of Doctor of Podiatric Medicine at the California College  of Podiatric Medicine in San Francisco. A respected podiatrist, Dr. Cartwright practiced in Los Angeles from 1969 to 1982. At one time, he had four podiatry clinics in the L.A. area, and he also mentored and trained other podiatrists in surgical techniques.

He bought a podiatry practice in Big Bear in 1976, and as a private pilot, commuted in his Cessna from the San Fernando Valley. He moved to Big Bear full-time in 1979 with his wife Diane and their two-year-old son, David Jr. “Big Bear’s Podiatrist”, Dr. Cartwright practiced in the valley until the mid-’90s, at which point he became a realtor and focused on real estate sales and investing.

"Dr. Tony" practiced podiatry in the Lovejoy and Orchard Park areas for 45+ years. He was very devoted to his practice and his patients and started the Podiatry Residency Program in Western New York.

I assume you do not practice as part of a large group or hospital. If you do, your group or hospital would have a sexual harassment policy in place. It would detail what your exact steps would be.
 
If you’re not part of a large group or hospital and do not have a sexual harassment policy in place for your office (I assume you don’t or you wouldn’t be asking), your employee can call the police and file charges. You should memorialize your conversation with her in writing and also note the day, time, location and any witnesses. Different states have different reporting requirements. In New York, we have the Division of Human Rights. On a federal level, the 1964 Federal Civil Rights Act (codified as 42 U.S.C. § 2000e et seq.) covers this, but you must have at least 15 employees to be covered by the EEOC.
 
As far as your patient is concerned, I’d speak first with your attorney (many malpractice carries will allow you legal advice to protect you and advise on your actions) before discharging the patient based upon the allegation of sexual harassment. On the smallest level possible, a frank conversation with your patient letting him know his behavior will not be tolerated would be in order, and your patient’s response should also be incorporated on the written memo of the event. Chances are he won’t feel comfortable returning after that and you would avoid having to discharge him from your practice.

One of your primary responsibilities as a professional business owner is the safety of your employees. Inappropriate behavior, such as language, physical activities, or any actions by patients, or anyone else, which you or your staff deem inappropriate is immediate cause for dismissal from the practice. My office manager is an ex-ER nurse and she does not tolerate anything, even over the phone, that falls into this category.
 
There is a fine template available from PICA Insurance for dismissing patients. We use the term "inappropriate action" in the letter to explain the reason for being let go. There is no second chance, and I have had a few call backs to request an opportunity to make amends to no avail. Your staff must feel safe in your work environment. Fortunately, this does not happen that often. I recommend you take immediate action in this matter.

RE: NIH/NIDDK Forms New Diabetic Foot Consortium (DFC)

Diabetic wounds are a significant source of morbidity and cost the U.S. healthcare system more than $13 billion dollars a year. In recognition of the urgent need, NIH/NIDDK has stated an initiative to form the new Diabetic Foot Consortium (DFC) for multi-site clinical research to validate biomedical markers for diabetic foot ulcers that can predict healing outcomes, guide treatment, and monitor healing and response to treatment. The long-term goal is to lay the foundation for a clinical research network to test therapies that can improve healing and prevent amputations.
