Diabetics Need to Seek Rapid Treatment for Foot Problems: IN Podiatrist
 
If you're an adult with an open, swollen, or painful foot wound, or a callus or bunion that doesn't seem normal, doctors say patients too often delay getting a professional opinion. TX Podiatrist Co-promotes for PhD Dissertation in the Netherlands
 
Lawrence Lavery, DPM, Professor and Director of Research in the Department of Plastic Surgery at the University of Texas Southwestern Medical Center, in Dallas, was the co-promoter for Dr. Suzanne van Asten’s PhD dissertation at the Free University in Amsterdam.

The title of Dr. Van Asten’s dissertation was titled "Biomarkers to Diagnose and Monitor Osteomyelitis in Diabetic Foot Osteomyelitis". Dr. van Asten was a research fellow in Dr. Lavery’s research group for two and a half years. She is the third Dutch physician to receive their PhD from work conducted with Dr. Lavery.

Opioids Unnecessary After Surgery in Some Cases
 
After carpal tunnel or stenosing tenosynovitis release surgery, acetaminophen, ibuprofen, and oxycodone work equally well for pain relief, researchers report. "It's become standard that if you have any procedure, you get opioids," said investigator Asif Ilyas, MD, from the Rothman Institute in Philadelphia. "We're trying to challenge that dogma."
 
"Orthopedic surgeons are one of the highest-volume prescribers of opioid medications," he told Medscape Medical News. "Our intentions are good, but there are a lot of unintended consequences." In fact, one study showed that about 6% of patients given opioids for the first time were still using them 90 days after a surgical procedure (JAMA Surg. 2017;152:e170504). Americans consume vastly more opioids per capita than people in the rest of the world, said Ilyas, and these drugs are now the leading cause of death among young people in the United States.

Even at a tech conference, Martin Pricco, MD, president of Gould Medical Group in Central Valley, CA isn't afraid to name one of the major culprits of physician burnout. At Gould Medical Group, Pricco wasn't content to let EHRs get in the way of physician contentment. The group looked at five areas where they could reduce frustrations with the EHR:

Since most of the EHR use time at Gould Medical Group was physicians documenting progress notes, this was an important area for improvement. The EHR was built with numerous methods for physicians to chart: dictation, voice recognition, template and keyboard, scribes, remote scribe technologies, or a combination. "We give everyone choice — you can do what you want, you can experiment with these various techniques and find the method that works best for you. We give people options. We don’t coerce them one way or another," he said.

QUERIES (MEDICAL-LEGAL)
Query: "Permanent and Stationary" Under Workers Comp 
 
I have been an employee for an HMO for over 30 years that banned nail grinders due to lung health risks. I have been debriding toenails with nail clippers exclusively for the past 4 years, at about 18-24 patients per day, 5 days a week. During the past 4 years, my right hand became so painful that I could no longer bend my thumb and developed a trigger index finger. I just had surgery about 9 weeks ago to release the tendon sheaths and I am supposed to go back to work in late March; however, my hand still swells and is stiff. 
 
I was told by the Work Comp doctor that at the next visit in late March if I am not better by then that I would be considered "permanent and stationary"--which means I will be...

RE: PM News’ "Top Podiatric Research Institutions"

I have read with considerable interest the very thoughtful commentary largely bemoaning the perceived lack of emphasis on research in our podiatric medical institutions, worldwide. Perhaps we within this specialty could take a hint from other institutes of higher learning and make a list - because everyone loves a list. I suggest a PM News "Top Podiatric Research Institutions" ranking much as U.S. News or others rate other institutes of higher education.
 
The metric might be based on self-reported annual: a) federal/not for profit research funds, b) industry-sponsored research funds, c) publications per faculty member, per calendar year, d) h-index or i10 index (a measure of per-publication impact) for faculty members. All of these could be collated by the editorial staff and reported. 
 
Not a week goes by that one of us reading this note gets a call from a prospective student asking "what's the top school in the specialty?" While this is always subjective, the fact of the matter is that we can't manage what we can't measure. This might be a nice start and I think it could give some friendly competition that could do nothing but improve education, research, philanthropy, and ultimately clinical care. 
 
I am sure that each of the deans of our specialty's institutions would be happy to report these findings. Bragging rights are often powerful motivators for countering institutional inertia. Schools that don't report would be glaringly evident. I am equally sure that we would love to see these rankings posted and increase over time.

We have banned all forms of grinding of nails. We use Miltex double-action nail nippers with straight jaws for debriding nails. These are very similar to double-action bone cutters. Patients are satisfied with this. You can get the thickness and the length with this method. You don't put yourself at risk of inhaling any irritants this way, and you can sterilize (with autoclave) your instruments when done. I published my instrument preparation protocol in PM News (and in Podiatry Management Magazine) a few years ago.

When literature out of England came out many years ago showing that podiatrists who grind mycotic nails were at risk of developing pulmonary problems, I took all of my drills and tossed them in the pail. Being an allergy sufferer, even the expensive vacuum systems did not prevent me from wheezing after doing a nail grinding.
 
How did my patients react to this change? Some left for other podiatrists, but most stayed. I explained to them that not only will they be breathing in their own mycotic nail dust, but they will be breathing in the infected dust from every other patient we saw in the last 12 hours.
 
I also believe that there are OSHA laws that require that a treatment room be left unused after grinding infected nails for a period of 8-12 hours. This makes sense and I do not regret not grinding toe nails. We all took the oath to help and do no harm, and grinding mycotic nails dances on the border of violating this oath.

I have done some study on aerosolization of nail particles from grinding of nails and presented it at the 2012 Meeting of the Council for Nail Disorders. Consider the following from a Great Britain study of 101 podiatrists Workplace Exposure to Bioaerosols in Podiatry Clinics Coggins, et al., Ann. Occup. Hyg., pp1-8 on behalf of the British Occupational Hygienic Society: 
 
32% of podiatrists reported respiratory conditions, 73.3% used gloves, 4.6% used respiratory masks, 16.8% used protective aprons, 15.8% used eye protection, 15.8% used room ventilation systems, 47.5% used nail drills with an exhaust system, 11% used nail drills with water spray dust  suppression. Recently there was a post by Dr. Blum about collecting cash from Medicaid beneficiaries. I have always thought that you cannot do this. If a patient has Medicaid and a physician doesn’t accept that Medicaid, that physician cannot bill the patient. 
 
I write this because I’m pretty sure it’s a federal statute and I don’t want my colleagues to think it’s okay to collect cash from Medicaid beneficiaries. I would also like to know if I am incorrect. Perhaps one of the more qualified billing experts can opine.
