PA Podiatrist Recommends Annual ABI Test for Diabetics
 
Poorly controlled diabetes can spawn a host of medical problems that can lead to amputations, but generally, a triad of issues tend to be present: poor circulation, neuropathy, and a diminished immune response. Circulation problems may interfere with wound healing, which, in turn, can lead to sepsis, or overwhelming infections.

Early signs of circulation problems include cramps in lower legs or thighs while walking, or a loss of hair on legs or toes. To combat this, Dr. Ronald Renzi, an Abington podiatrist, recommends a simple once-a-year blood pressure test on the ankle to check blood flow.

Gov. Mike Pence has re-appointed William K. Oliver III, DPM to serve a three-year term through March 7, 2019 on the Board of Podiatric Medicine.

Steve Westenhofer, Mimbres Memorial Hospital, CEO, has announced the opening of Deming Podiatry Services and the addition of podiatrist Angela Fields, DPM, to the team of specialists.

Canadian Foot Docs Volunteer in Winter Special Olympics
 
The Canadian Winter Games of Special Olympics was recently held in Corner Brook Newfoundland. Over 700 athletes from the 10 provinces and three territories attended. The winners of these events will then compete in the World Winter Games to held in Austria during the winter of 2017.

Specialized Podiatry recently opened in Vernon Hills. Dr. Igor Dukarevich has been in private practice of podiatry since 2011. Prior to that, he graduated from the Scholl College of Podiatric Medicine and completed a three-year residency at Rush University Medical Center.

Query: Met Osteotomy & Plantar Plate Repair
 
Can someone please advise me of the appropriate CPT for a plantar plate repair? I will be doing this procedure in conjunction with a Weil-type osteotomy.

Response: There is no specific code for plantar plate repair of the lesser metatarsal joints. I recommend using CPT 28200 (repair, flexor tendon, foot, single), and recommend also billing for the Weil osteotomy using code CPT 28308. 
 
Your other coding option for the flexor plate repair would be to bill CPT 28899 (unlisted foot/toe procedure). If you choose this, you would need to submit an operative report with a manual claim and request peer review.

RE: The Passing of Herbert Blustein, DPM
 
With much sadness, I note the passing of my colleague and friend of many years, Herbert Blustein, DPM, on Saturday, March 12th.  Since graduating NYCPM in the 1950s, Herb was an active member of Queens Division, and had recently retired as a Life Member of the NYSPMA. He was admired, respected, and loved by his patients and fellow podiatrists, and will be sorely missed. Condolences to his devoted wife, Miriam, and their family (including his podiatrist son, Steven). R.I.P.  Herb.

As a resident, I was sent on a consult. As I approached the patient’s bed, I discovered that the patient was dead. I sheepishly returned to the attending to relay the news. He was upset that I did NOT get a copy of the face sheet and insurance information so he could bill the consult.
 
The examples you cite are not that over-the-top, but as others have said, they are learning experiences. It’s good to see that you are developing a good sense of stewardship for podiatry.

In the grand scheme of things, x-rays are an imperative part of a podiatric medical practice and generate a tremendous amount of revenue. This comes on top of the pending Medicare Incentive Payment System in 2019, which may reduce fees paid by Medicare nearly 20% if a low quality and high cost profile is generated by the Value-Based Modifier (VBM). It looks to me that with these fee constraints in place, along with ACOs and other fee reducers like Meaningful Use Attestation and PQRS (which are both set to expire next year), the profession of medicine is no longer a financially viable profession from here on. 
 
I am in my waning years and look to finish out my career in the next 10-15 years, but the new graduates and those just entering the profession may have a long road ahead of them financially to make back the money paid and/or borrowed for tuition. What are the schools doing to inform current students about these future fee reductions? What are the state affiliates doing to educate their members on this issue?
 
Yes, most of the recently graduated profession will most likely be employed and make a set salary, but I believe based on these fee reductions, the days of making hundreds of thousands of dollars a year may be long gone. It looks to me that medicine is turning into a "punch clock" profession. How sad this is. The government wins and we lose.
