Last night, Charles Bradley, DPM, MA passed away. Dr. Bradley served as president of the APMA from 1983-4 and was instrumental in the founding of the podiatric section of the National Academies of Practice.

MA Podiatrist Discusses Plantar Fasciitis

Plantar fasciitis has become so ubiquitous that podiatrists can practically make the diagnosis before a patient even sets foot in their office. "I see 18 to 20 patients a day with plantar fasciitis. Heel pain is epidemic," says Dr. Michael King, a podiatrist practicing in Fall River, MA, and past president of the American Podiatric Medical Association. He can only guess how many more cases are out there. "Some people just tolerate the pain and try to get by."

Plantar fasciitis has become a mark of modern life, but it's far from inevitable. According to King, heel pain could become a relative rarity, assuming we are willing to take better care of our feet. More than anything, King says, people with plantar fasciitis just have to give their feet a chance to recover on their own. He has a few simple guidelines. First of all: no going barefoot, and no flimsy flip-flops. (Flip-flops or sandals with arch support are fine.) He encourages patients to put on footwear with some arch support before even getting out of bed.

NY Podiatrist Travels to Haiti to Help Rebuild School

During a recent trip to Haiti, Dr. Patricia Nicholas took the time to spend with the 181 children attending the one-room schoolhouse until a school building can be erected.

Construction is ongoing. Much needed donations are needed to finish the project by October 2013 when the next school year starts in the small town of O’Rouck (Anse-a-Veau), Haiti.

AACPM Announces Release of Curricular Guide

The American Association of Colleges of Podiatric Medicine (AACPM) has announced the release of the first Curricular Guide for Podiatric Medical Education. A copy of the Curricular Guide for Podiatric Medical Education can be found on the Association’s website at aacpm.org.

AACPM Council of Faculties Chair Denise Freeman, DPM, MSE, stated that “the Council of Faculties Curriculum Guide is an AACPM initiative designed to reach a general consensus within the podiatric medical community on the skills, attitudes, and knowledge that graduating podiatric medical students should possess prior to starting residency training. The 300+ page Guide consists of program level learning objectives that podiatric school deans and faculty can use in reviewing and developing their educational programs.”

CPME Increases New Potential Residencies by 108

The Council on Podiatric Medical Education is focusing significant attention on facilitating increases in positions in approved podiatric residency programs. Following a thorough review of available clinical experiences, the Council identified programs with the clinical capacity to increase positions—resulting in a total of 108 new potential residency positions.

The Council has notified each of those programs authorized to add residents. While it is not expected that all of the offers to increase positions will be accepted, the Council is hopeful the positions that are accepted will help alleviate the residency position shortage.

Some Fear EHR Meaningful Use is Too Much, Too Soon for Doctors

Even before requirements for the second stage of the federal electronic health record incentive programs were finalized, many in the healthcare industry expressed concern that the promise of a bonus check was pushing some physicians to move too quickly with their transitions from paper. Their arguments were that by doing too much too quickly, work flow efficiencies and patient safety could decline, because physicians adopting for the first time would be too focused on checking things off a list of meaningful use objectives instead of easing into a new way of practicing medicine.

If rapid adoption is truly a cause for concern, a study in the January/February issue of Annals of Family Medicine shows which states would be playing catch-up the fastest. It found that when the meaningful use incentive program started, some states already had the majority of their family physicians using technology, according to administrative data from the American Board of Family Medicine. Utah had an adoption rate of 94.9%, but Florida, Illinois, and Michigan lagged far behind, with less than half using some form of electronic health record system.

QUERIES (CLINICAL)

Query: Suggestions for Painful Bunion

This AP, weight-bearing x-ray is the foot of a 50 year-old female. She is basically healthy with some osteopenia. The lateral view shows a calcaneal inclination of 25 degrees and normal talar declination. There is a mild anterior break in the cyma line. Her bunion is painful and progressively getting worse over the years.

On physical exam (using "C" arm), she does seem to have a very flexible met-cuni joint. The 1st MTPJ is trackbound with mild non-painful crepitus. There are no severe arthritic signs. After presenting her with conservative and surgical options, this lady wants surgical resolution. Suggestions are welcome.

In our area, we have several chiropractors aggressively advertising a "breakthrough" treatment for neuropathy utilizing intermittent pulsed-laser therapy (IPLT). There are impressive patient testimonials included in the ads. I do not have any experience with this modality, but any thoughts from colleagues would be appreciated.

I don't see any shame in the report that Dr. Warren Joseph is one of the highest paid consultants/speakers to the pharmaceutical industry. I have heard Dr. Joseph speak on several occasions and have found him to be an unbiased expert who promotes limb salvage and good antibiotic stewardship--both of these are more worthy of a nice salary in my opinion than most of the over-paid positions held by insurance company executives.
