According to Conway Mclean, DPM, "Various methods of pain relief have been created but never brought to commercial use in this country. A few are gradually playing an increasing role in the treatment of America’s musculoskeletal pain as word spreads of their attributes and uses. One example of this phenomenon is PEMF therapy, short for pulsed electro-magnetic field. This refers to the application of a magnetic field which varies over time, i.e. it pulses."

UK Podiatrist, Former Elite Runner Interviewed on Pelvic Asymmetry
On his podcast Fit Foot U, Ben Pearl, DPM Interviewed former UK elite miler and podiatrist, Dr. Clifton Bradeley. Dr. Bradeley was a former elite track runner who was the first British runner to break the indoor 4 minute mile. Shortly thereafter, he fractured his pubic symphysis during an indoor meet which started his quest to understanding asymmetry in the pelvis and how this affects gait. In the process, he discovered he had a longer right leg. This results in more force through the right leg and a higher risk of injury. The following video clip is from the interview:

Bradeley encourages clinicians to get comfortable assessing the ASIS, PSIS, and sacral landmarks. He stated this is the key to discovering asymmetry. In his experience, a high anterior hip on the right often has a longer leg on the left. Bradeley continues his research in asymmetry, keeping a rigorous regimen that he credits to his training as an athlete.

OH Podiatrist Chairs First COVID Origin Hearing
Three years into the coronavirus pandemic, a new congressional committee set up by Republicans is trying to answer whether the virus came from a lab, or from an animal in China. “This question is fundamental to helping us predict and prevent future pandemics,” Ohio Rep. Brad Wenstrup, DPM (R, OH-2) said as he opened a hearing Wednesday.

Dr. Wenstrup, a former podiatrist who now represents southern Ohio as a Republican, is leading the effort as chair of the Select Subcommittee on the Coronavirus Pandemic. “The truth is we don’t know the origins of COVID-19 yet, for sure. We don’t have a smoking gun,” he said. So, Wenstrup called a series of medical experts in for an initial hearing to assess where things stand.

New Study Reveals Racial Disparities in Treatment and Outcomes for PAD Patients
Researchers at the University of Southern California (USC) have published a new study in the journal Advanced Therapeutics revealing significant disparities in the treatment and outcomes for patients with peripheral artery disease (PAD) based on race. The study included co-senior author David G. Armstrong, DPM, MD, PhD, Professor of Surgery at USC and the Director of the Southwestern Academic Limb Salvage Alliance (SALSA).
The study revealed that Black patients were younger on average but had a higher co-morbid burden, concomitant risk factors, and cardiovascular medication use at baseline. Although Black patients had a numerically higher prevalence of diagnostic testing, revascularization procedures, and medication use, they were more likely to receive medical therapy without a revascularization procedure. Additionally, Black patients with PAD had a higher incidence of major adverse limb events and cardiovascular events than White patients.

Query: Coding For Qutenza
We are looking at using Qutenza for diabetic neuropathy. The codes recommended by the manufacturer are J7336 and CPT 64640. What is the panel's opinion on what constitutes medical necessity and validity of the use of these codes?

Response: The J7336 is the correct code for Qutenza. I would not advocate or suggest use of CPT 64640. One may use an E/M for evaluation for the application of the patch as there is no application code yet.
Recently, NGS has come out to allow CPT 17999, the unlisted dermatology code for application. NGS does not have an LCD or LCA at this point addressing Qutenza. Billing per the NGS article on their website:
This unlisted code will analog to CPT 17360 for application but only in the NGS MAC so far. This is applied in units of 280 units per patch with allowance, typically, of 4 patches for a 30 minute application, every 3 months. A portal exists to verify if covered in your region.

RE: Discrimination in Third-Party Reimbursement for Podiatric Services

Dr. Moglia states, in the discussion of podiatrists seeking a PA degree, "The million dollar question is if the PA degree will allow better reimbursement from insurance companies who discriminate against podiatrists with lower reimbursements for the same services provided by an MD. It would also allow for a greater scope." He states something that many of us already experience with respect to unfair reimbursements. Nevertheless, discussion of reimbursements appear to be avoided by our professional organizations. Some disagree that lower reimbursements to podiatrists are actually the case.
One reason, I believe, for limited discussion on third-party reimbursements, has been the concern that we do not engage in collusion with respect to fees. That concern is overstated as we do not have the freedom to set fees and we are not attempting to collectively negotiate for increased fees in a specific geographic area. We are simply attempting to establish if we are being paid fairly and if there is systemic discrimination against DPMs with respect to third- party reimbursements.
It is likely that most of us have seen EOBs from other providers or from patients, so based on what I have seen, it appears that DPMs are being paid at a lessor rate than orthopedic surgeons for the same CPT codes. We are experiencing inflation -- increased supply costs, increased labor costs, and so on; yet we are contractually locked into fee schedules by third parties that are unfair and, I feel, discriminatory. Why aren't our national organizations and the leaders of our profession addressing this issue?
