NY Podiatrist Discusses How to Select Shoes for Wide Feet
 
How do you know if you actually have wide feet? According to Bruce Pinker, DPM, a board-certified podiatrist and foot surgeon of Progressive Foot Care in New York, if you feel pressure on the sides of your feet while wearing shoes, then you likely have wide feet. To confirm this, use a Brannock device, the metal contraption available at most shoe stores for determining your shoe size to measure the width of your feet.

“If one has an enlargement of the foot just behind the little toe or big toe, it’s a sign of a bunionette and/or a bunion. Both of these conditions can cause one’s feet to be wider," says Dr. Pinker. Brands like New Balance, Naturalizer, Brooks, and Vionic all offer lots of shoes in multiple widths, and some labels like Altra are specifically made with wider toe boxes, allowing toes to splay naturally. Choose wide, extra-wide, or extra-extra-wide options depending on your foot measurement. “The width should be snug, but not tight,” he says.

5 Things to Consider When Selling Your Practice
 
3. If you own the practice, prepare for two sales. 
 
Do you own your practice building or have ownership of your office space in an office condo? If so, you now have two sales to make. Here are your options:
 
• Sell the practice and lease the space. This overcomes resistance to ownership at this time. It also means that you are now the landlord. Following the sale, you could have your property in the hands of a property management company, who would collect rents and maintain the facility.
 
• Sell the practice and lease the space, with an option to buy at a future date (say five years). If the physician does not exercise the option, you can continue as a landlord or consider finding a buyer interested in investing in rentable space.
 
• Sell the practice and sell the space. If the practice buyer is not interested in ownership of the facility, consider finding a buyer. This makes sense if you are not planning to remain in the community.

Query: Alternative to Wisdom EMR?
 
I use WISDOM software from Cerner for my office management system. WISDOM will end at the end of 2022. Cerner is offering a new program, but I am wondering if anyone else is in this situation and if they have found a better alternative to what Cerner is offering. Any input will be appreciated.

There was a brief discussion last month regarding EMR and comments were made about Modernizing Medicine, Tracknet, etc. I also inquired as to whether or not there is some type of database that could be accessed to see what programs are available for podiatry, their fee structure, and features of the programs but as no one has responded, I'm assuming there is not.
 
This is unfortunate as those of us in private practice need to use programs in order to run our businesses. I started with DR software over three decades ago and then transitioned to ICS software/Sammy for the past 25 years and have been satisfied but...

RE: Coping with Inflation

We are facing an inflation the likes of which our country has not seen in forty years. As physicians, we will feel the pinch now more than forty years ago because of the restrictions by Medicare and insurance panels. 40 years ago, physicians could charge Medicare patients their fee, regardless of the approved amount. We have no means of simply raising our fees 10% or so to adjust, yet everything we consume in our practices costs more. I do not have an answer, but I do have a simple question: what are we supposed to do?

As Drs. Freedman, King, and Prikaszcikow have already elaborated, APMA has done much to mitigate the initial rulings on the NCCI edits applicable to nail debridement and paring of hyperkeratosis. These leaders have done an admirable job of reviewing what APMA has accomplished and continues to work on, in an attempt to resolve the NCCI edits which Dr. Kass correctly asserted are unfair.
 
Dr. Kass' accusations that APMA leadership has been inattentive is clearly wrong while attacking the very people who have been successful at resolving much of this issue. Had he been an APMA member, he would have received the communications all APMA members received to keep them up-to-date on this issue as well as others. 
 
The volunteer leadership and members of each and every APMA and NYSPMA committee which I have participated in over the past twenty years work very hard. They often sacrifice office hours, face arduous travel schedules while also sacrificing personal family time. They do this all for the betterment of our profession without any remuneration. It would be nice if these individuals were provided with a simple thank you instead of the unkind comments recently offered.

I’d like to thank Drs. Prikaszczikow and Freedman (Dr. King as well) for their honest response to my posting. They mention that “several years ago”, APMA recognized this bundling problem, and that over the past “several years” had discussions with medical directors from Anthem, Aetna, and other payers”.
 
I think the frustration on my part, as well as many of my colleagues, is we all recognize a problem existed for “several years”, and the sad truth is it still exists! After reading the response given, it is clear “podiatric leadership” has indeed “challenged the issue”. The problem is correction of the abusive bundling rule created by NCCI (not Medicare) which unfortunately still exists. I thank those who put their time in for the cause but have to wonder why what appears to be a simple problem has to take “several years” to be fixed. Elon Musk can fly people to space, build electric cars, dig tunnels underground, and have trains hover on electromagnetic rails, and we can’t change a bundling edit that is illogical? Something’s wrong!

High arched foot types (the rigid rearfoot, rigid forefoot functional foot types) only put excess weight on the outside of the foot in the rearfoot (a low rearfoot SERM-PERM Interval). In the forefoot, the rigid FFT has a plantarflexed and stiff first ray that results in too much weight and function on the inside of the foot (a low forefoot SERM-PERM Interval). They have medial forefoot pain, osteoarthritis of the 1st MP joint, and callus and wounds under the 1st metatarsal head confirming excess weight-bearing on the inner, not outer, side of the foot. High arched feet reflect lateral heel and medial forefoot dysfunction and lack the ability to perform tasks. They are poor shock absorbers and do not handle inclines and obstacles well. Summarily, they lack the ability to pronate the rearfoot and supinate the forefoot (their forefoot is pronated, not supinated). Click here to read reference.
 
For years, podiatry has focused its education and practice on reconstructive foot surgery as it has morphed and matured into a sophisticated surgical specialty with new terminology, expanded applications, and predictable outcomes. It accomplished this over time with a foundation of a three-year surgical residency and a practice focused on being a great surgeon. There was no longer room to remain expert in other fields of podiatric medicine such as biomechanics and human movement that have similarly morphed and matured over time as its practitioners have sacrificed surgical prowess to become an expert in like me. Am I the only podiatrist who cares?
