The Greater Greenbrier Chamber of Commerce will hold its Annual Dinner and Meeting on Apr. 13 at The Greenbrier Hotel. Business Leader of the Year honoree for 2018 will be Dr. Michael Adelman, president of West Virginia School of Osteopathic Medicine.

Dr. Adelman received his degree from the Pennsylvania College of Podiatric Medicine. Following a residency in podiatric surgery, Dr. Adelman went back for his degree in osteopathic medicine from the Des Moines College of Osteopathic Medicine and Surgery where he graduated with honors in family medicine, followed by a residency in proctology. He received his law degree from the University of Toledo in 2004.

Health Workers Endangered by Mini-C Arm Imaging
 
Radiation scatter from orthopedic hardware can harm the eyes of first assistants during mini-C arm fluoroscopy, results from a new study suggest. "I think the risk is significant," said Michael Groover, DO, from Grandview Medical Center in Dayton, OH. Surgeons and first assistants should use the mini-C arm in the vertical position as much as possible, and wear lead-lined glasses and other protective clothing, he told Medscape Medical News.
 
Orthopedic surgeons frequently use the mini-C arm fluoroscopy during the implantation of metal devices such as plates and retractors. Previous studies have measured radiation exposure from these procedures, and generally determined the levels to be safe.

Even at a tech conference, Martin Pricco, MD, president of Gould Medical Group in Central Valley, CA isn't afraid to name one of the major culprits of physician burnout. At Gould Medical Group, Pricco wasn't content to let EHRs get in the way of physician contentment. The group looked at five areas where they could reduce frustrations with the EHR:
 
2. Custom Interfaces: 
 
In this regard, it's imperative that EHR interfaces are made to enable specialty-specific functionality. "While IT would love one-size-fits-all standardized format that's easy to manage, my advice is don't do that. We had that for a long time; it became a major burden." He said they used a team of specialty docs, system analysts, EHR build experts, and vendor representatives to create interfaces that would optimize the EHR for the specialty doc. "The doctors were really engaged in the build of the EHR and their work flow. They were owners of this and it worked for them."

I had to chuckle when I read Dr. Luongo's posting about meeting a resident from the late 1990s and he referred to the recent meeting as "so many years later.

Last year, I met a newly arrived "resident" at a retirement facility where I provide care. He introduced himself and told me that he had taught at NYCPM. I was so delighted to get re-acquainted with vascular surgeon, Dr. Enrique Spiegler, who taught us at NYCPM. The last time I had seen him was in 1969.

We have our old x-ray films recycled and they will pay you for the silver content and provide you with a certificate of destruction. We use B/W Recycling – they will even cover the shipping costs to send x-rays to them as long as you have at least 50 lbs of films. We have used them three times so far over the years, with great service every time

With all do respect to Dr. Albright, if podiatrists do not grind mycotic toenails, then who will?  It is not as though I look forward to treating those "horse hoofs" like toenails in the nursing home or office setting, but the only way that I know how to give a patient relief from this condition is to grind their toenails. First of all, many folks cannot and should not take oral antifungals. Second, if the nail has been traumatized, the oral and topical antifungals won't work. The only treatment that will work every time is to grind those thick, fungal toenails with a burr on a Dremel. 
 
Yes, I do have a vacuum system in my office and I do wear a mask in the nursing home setting. However, although it may not be an appealing part of our profession to some, the relief and gratitude on that patient's face who can finally wear footgear for the first time (in a long time) without pain, makes it all worthwhile to me. It is what I was trained to do in school and in residency. It is my job!

After reading the comment posted by Estelle Albright, DPM, I can't help but agree and disagree with her post. First, I agree with her comment about the dangers of inhaling the dangerous dust particles caused by burring fungal toenails. The potential health issues this procedure can cause to podiatrists, their staff, and patients should always be considered. I have been using a nail dust extracting system that I manufacture for years and can honestly say, I do not suffer from any pulmonary issues nor do any of my staff, and I have had two assistants that were with me for 17 years. If you grind mycotic toenails, a vacuum system is a must. It would be very interesting to see the results of a poll question asking if podiatrists use vacuums when burring nails. My guess would be between 10-15%.
 
I most definitely disagree with Dr. Albright's statement about not needing to burr toenails because of the other options we have with topicals, oral medications, and double-action bone forceps. I don't think I'm going out on the limb by saying not too many patients would be satisfied walking out of any of our offices having their mycotic nails treated only with a double-action bone forceps,  especially if billing Medicare for the treatment of painful and symptomatic mycotic toenails.

Long ago and far away, Dan Vickers came to us. We are so much better for his being a part of all podiatric surgeons' lives than can be told here. He should be granted an honorary DPM if only because he dedicated his life to every podiatric surgeon alive as well as those who no longer are with us. I know Dan.from 1969 on. Dan has given, is giving, and will give all he has to those of us who devote our lives to our patients. We are his patients. Dan dedicated his life to all of us. Thank you Dan and most of all, to your family for allowing you to do so.
