The Academy of Physicians in Wound Healing (APWH) will present its first “APWH Outstanding Achievement Award “to Dr. Arthur Helfand on March 28, 2014 during the APWH 2014 Annual Physicians & Wounds Conference to be held at the Sheraton Downtown Philadelphia Hotel in Philadelphia, PA.

Dr. Helfand is a pioneer in wound care. Long before the terms “wound care” and “wound specialist” existed and long before training programs were developed for this field, Dr. Arthur Helfand was essentially practicing wound care by treating diabetic patients in his podiatric practice. He is a past president of the American Podiatric Medical Association and an inductee in the PM Podiatry Hall of Fame.

Dr. David Rizzo says that as people age, their nails become long and discolored and too thick to trim, harboring fungus and courting infection. He knocks on doors with an old-fashioned black leather medical bag and charges just $45 per visit. "I'm not trying to get rich."

He does not accept credit cards or insurance, and doesn't have medical insurance himself. Rizzo thinks we'd all be better off with a national healthcare program modeled after Kaiser.

FL Podiatrist Favors Legalization of Medical Marijuana
 
Tuesday night's workshop at the Witch's Brew Café in Palm Harbor was one of 16 planned until the November election, when Amendment 2 - the issue of legalizing medical marijuana - goes up for vote. In the crowd, along with those just wanting to talk, was a doctor who sees the benefit for his patients.

"I have MS patients who have had the opportunity to smoke it in the past, and claim marijuana is the only thing that gets them relief," said Dr. David Collom, a podiatrist. "I myself don't smoke it, but I'm frustrated we can't pursue what we can treat with it."

OH Podiatrist's MSRA Research Shows Benefits of Adding Oakin® Dressings 
 
February’s edition of the Journal of Wound Care includes a research paper “Healing Outcomes of MRSA-Infected Wounds with a Protocol Combining Oakin® Dressings with Elements of De-escalation Theory,” authored by Gene Graham, DPM, Columbus, Ohio. The high prevalence of chronic, MRSA infected wounds in Columbus drove the need for new protocols based on the latest research surrounding the treatment of MRSA-infected wounds.

The findings from Dr. Graham’s research resulted in a “nearly 80% wound closure within 90 days” when following the newly developed protocol. Dr. Graham has been invited to present his research at numerous podiatric and wound care conferences in 2014, including the upcoming APWH conference and Podiatry Institute conferences.

I have always admired how Dr. Markinson has put himself out there with his opinions in these posts with the intention of educating his fellow practitioners. Thank you for that. Unfortunately, his recent remarks are not accurate in terms of injectable steroids. First off, I practice in a multidisciplinary setting which includes three rheumatologists. Two of these doctors were university-based for many years. They use soluble steroids all the time. What about Celestone, a preparation combining a phosphate and acetate? But let me not dwell on hearsay. Let's turn to the literature for some help on this matter. There are countless articles written on this subject. Let me quote but just one:
 
"A short-acting solution, such as dexamethasone sodium phosphate (Decadron), is less irritating and less likely to cause a postinjection flare than a long-acting dexamethasone suspension. Many clinicians use injectables that combine short-acting compounds with long-acting suspensions (e.g., betamethasone sodium phosphate and acetate suspension), thereby obtaining the beneficial effects of both types of preparations."

In response to Dr Bijak's "slam" on my nitroglycerin paste protocol, I am familiar with his rude responses and judgmental behavior that is so typical for him. I will try not to take it personally. 
 
I am fairly certain in my description of my protocol using nitroglycerin paste that I presented both sides of the discussion using this particular modality. I mentioned that the medication had a dangerous potential and cited my experience of using it on a 90-year old patient, with low body weight. 
 
I had a similar event of hypovolemia while treating a patient when...

I have been using the smartdop 45 for a number of years with the software package. According to the rep from Koven, the software is appropriate for Medicare if you perform the tests properly and for the proper reasons and criteria. Koven has updated their software a number of times to conform to Medicare regulations. I read my results myself for both the single level and multiple levels. I feel I have enough knowledge from a number of lectures and learning sessions with my vascular surgeon, and experience to do so.
 
I certainly feel that most of us can read a single level ABI with pulse volume without help. I have the extra hardware to do a TBI and pulse volume, which conforms to Medicare guidelines. If you feel that you need the reading to be done by a vascular specialist, I'm sure that you can find a local guy who will be happy to do it. They only get about $12-20 to read the results, but they know that podiatrists are great sources of referrals and would love to have you send them all your vascular compromised patients. It's also a great way to develop a relationship with your local vascular specialists and get them on our side.

Recently, I performed hammertoe repair by proximal phalanx head resection on two toes at the outpatient surgical center. Today, I received the EOB from CIGNA. The allowed amount is $293.40 total ($195.60 for the first procedure, and $97.80 for the second procedure). What a slap in the face! I am now dealing with the usual redressings, etc. for free during the 90-day global period. I sure am glad to have a general podiatric practice, with only a portion devoted to surgery. I realize the procedures I did are not hi-tech and exotic, but I am sure the drastic reduction of surgical allowances has hit all the procedure codes.
