FL Podiatrist Explains Why Feet Might Get Bigger With Age

Do feet really get bigger with age? Some strange facts end up being debunked, but this is certainly possible: "After years of wear and tear, tendons and ligaments in your feet may weaken. This can cause arches to flatten, which means feet get wider and longer. It won’t happen to everyone, though—people who are overweight, who get swollen feet or ankles, or who have certain medical conditions, like diabetes, are more prone. If it does happen, the average gain is about one shoe size by age 70 or 80," says Cary M. Zinkin, DPM, podiatric sports physician and spokesperson for the American Podiatric Medical Association.

David Alper, DPM was recently invited to be a part of a new National Advisory group focused on reviewing efforts towards professional membership services and growth for the American Diabetes Association. The Professional Membership Advisory Group will include professionals from an array of disciplines and backgrounds.

Dr. Alper has a 20 plus year relationship with the ADA, having served on several state, regional, and national committees. He currently is the president of the Northeast Region Advisory Board, and has a seat on the National Legislative and Regulatory Committee.

PODIATRISTS IN THE COMMUNITY
MO Podiatrist Virtually Assured of Spot on School Board
 
The Blair Oaks R-2 School District is not actually having an election for its Board of Education this April. Two incumbents ran unopposed, so the district isn't statutorily required to have a vote. Bill Duke, DPM is a podiatrist at Jefferson City Medical Group (JCMG) who has been in St, Louis for 12 years.

Dr. Duke has served two terms on the school board, and he's currently the vice president. He also cited the development of new school facilities and extra-curriculars as focal points for the district in the coming years. He's also on the parish council at St. Stanislaus Church, and has served on the finance committee at JCMG. In his spare time, he coaches football and baseball at Blair Oaks.

Irish Auditor General Cites Lack of Podiatrists Needed to Treat Diabetics
 
The lack of specialist podiatrists for people suffering from Type 2 diabetes in the Western Trust should be subject to an urgent Department of Health workforce plan by 2019, the Auditor General, Kieran Donnelly, has recommended. Mr. Donnelly said the shortfall in provision for local sufferers of a disease that affects almost six percent of adults in the North, needs to be addressed now.
 
He issued the warning in his newly-published ‘Type 2 diabetes prevention and care’ audit. In 2003, it was estimated that 76 podiatrists would be needed to meet demand in the North but only 36 podiatrists were in place, a shortfall of 40.

PRACTICE MANAGEMENT TIP OF THE DAY
Reducing Clinician Burnout in EHR-Related Areas - Part 1
 
Even at a tech conference, Martin Pricco, MD, president of Gould Medical Group in Central Valley, CA isn't afraid to name one of the major culprits of physician burnout. At Gould Medical Group, Pricco wasn't content to let EHRs get in the way of physician contentment. The group looked at five areas where they could reduce frustrations with the EHR:
 
1. Initial training: Pricco said research into his organization concluded that "if you get it right at the beginning, it persists over time." Training physicians properly on the EHR at the onset will lead to higher satisfaction. Best practices, he said, are six hours of onboard training. At Gould, Pricco said, they do four hours of classroom training and one week of at-the-elbow support. Another tip in initial training, he said, is to tailor training to an individual provider.

Query: Medicare/Medicaid Billing for Diabetic Shoes
 
My Medicaid DME supplier number recently came up for renewal. They are asking for a surety bond, and I’ve been quoted $1000 for the bond. Based upon demographics, it’s not worth it. So as it is right now, Medicaid will not process any of my DME claims unless I renew. So here’s the question: Let’s say I choose NOT to renew, and a patient with Medicare primary, Medicaid secondary comes in and I want to prescribe diabetic footgear. Since I’m not going to be billing Medicaid for the 20%, is there any problem collecting that from the patient (collecting up-front of course)?

Response: Not a problem. If the Medicaid information is on the claim to Medicare, Medicaid will not pay the 20% whether you are approved or not approved for DME. When you do this, please make sure the patient's Medicare deductible has been met.

RE: NJ Former Resident Meets Attending After Many Years

Every year, kids play their last games of Little League baseball at a week-long tournament in Cooperstown, NY. On my team’s fourth game, to my amazement, while meeting at home with the umpires, the opposing coach was very familiar looking. He then introduced himself as Dr. Steve Lemberger, one of my residents at St Clare’s/St Vincent's Hospital from the late '90s.  
 
What a coincidence, so many years later, here we are, meeting on the Cooperstown ball field representing our kids' 12U baseball teams the Maywood Hawks vs. the Freehold Fusion. We both had an incredible week, an experience for our boys they will never forget. It truly is a small world!

I am surprised from the survey that so many podiatrists are still grinding nails. To those podiatrists who are still grinding fungal toenails: Please consider the considerable risks to self and staff caused by nail grinding: lung granulomas, and fungal sinus or lung infections caused by aerosolization of fungal nail particles or spores. Also, consider the risk to staff for cleaning, disinfecting, and sterilization procedures of re-usable burrs.
 
I am of the opinion that we, as physicians, should instead treat the infection medically or surgically. There are many safe oral and topical antifungal meds available, as well as surgical treatment of fungal nails. If none of these are an option, consider double action bone cutters to debride thick nails. These do a very decent job on thick, hard nails and are easier on your hands. For ingrown edges, Miltex 40-22A ultrafine nail splitters are excellent for simple nail edge avulsions.
