Many Patients Have Misconceptions About Bunions: IA Podiatrist
 
Bradly Bussewitz, DPM, a foot and ankle fellowship-trained surgeon at Steindler Orthopedic Clinic in North Liberty, Iowa, sees anywhere from 20 to 40 patients with bunions each month. A lot of these patients think they’re forming extra bone from getting more calcium, not realizing it is their bones moving out of place.

Another common misconception is that bunions are caused by wearing heels or shoes that are too tight. While tight shoes can exacerbate the problem and contribute to discomfort, most of the time — other than after a traumatic injury to the foot — it is caused by genetics. “It’s not causing the bunion, it’s causing the bunion to happen much sooner in life than it otherwise would have without tight or restrictive footwear,” Dr. Bussewitz said.

A Houston physician is accused of secretly recording employees inside a bathroom at his own medical office. The victims’ attorney shared cellphone video of the hidden camera after staff discovered it. Authorities say local podiatrist Douglas Hansen, DPM has been arrested and charged.

2. Collect at the time of service
 
Every dollar you collect at the front desk is a dollar you don’t have to chase later. Point-of-care collection reduces accounts receivable, cuts billing costs, and minimizes the administrative burden of tracking down overdue balances. The AMA’s guidance on managing patient payments emphasizes that collecting co-pays, co-insurance, and known balances at the time of service is one of the most effective ways to maintain financial viability. Staff should be trained to ask for payment as a standard part of check-in or checkout, not as an afterthought.

5 Retirement Tax Traps to Watch in 2026
 
Even in retirement, some income sources can unexpectedly raise your federal and state tax bills. Here's how to avoid costly surprises.
 
2B. Note: You might have heard that the Trump/GOP 2025 tax and spending bill eliminates taxes on Social Security benefits. It does not. (The new law doesn't change the Social Security benefit tax formula or the IRS "combined income" thresholds.)
 
But the 2025 Trump tax bill contains a new tax deduction for older adults.
 
Senior Bonus Deduction key points:
  • Beginning with the 2025 tax year, retirees age 65-plus might be eligible for a “senior bonus” deduction of up to $6,000 ($12,000 for joint filers).
  • That's even if they take the standard deduction or itemize.
  • You must have a valid Social Security Number to claim the credit.
The new deduction can lower taxable income and thereby potentially reduce the portion of Social Security benefits subject to federal income tax. However, the benefit phases out for higher earners.

2.  What Is RED-S?
 
Relative Energy Deficiency in Sport (RED-S) is a broader, more accurate framework that recognizes that energy imbalance can affect all athletes—both men and women.
 
Issues with proper fueling are especially common in endurance sports, aesthetic sports, and weight-class sports, where pressure to maintain a certain physique or weight is often high. The older term, “Female Athlete Triad,” mistakenly suggested this problem was limited to female athletes and failed to capture the complex, multisystem nature of the condition. RED-S, on the other hand, acknowledges the full spectrum of consequences that arise from inadequate energy intake, including hormonal dysregulation, impaired bone health, cardiovascular effects, and a range of performance and recovery challenges.

Let’s say one owns a medical insurance company. Every year, I make more money by lowering reimbursements. Every year, the physicians continue to accept it. Why wouldn’t I continue to decrease reimbursements? Until physicians vote with their power of refusal, the trend will continue. There are a number of procedures I refuse to do secondary to reimbursement.

Dr. Hrywnak’s recent posting calls to mind the folk song The Old Grey Mare. Little did I appreciate this “Down in Alabama” song I sang as a child would be applicable to the transformation in medicine I have sadly witnessed over 50 years of practice. For the unfamiliar: “The old grey mare she ain’t what she used to be,  ain’t what she used to be, ain’t what she used to be. The old grey mare she ain’t what she used to be, many long years ago.” Substitute medical practice for mare.
 
Profit has replaced professionalism. Inhumanity has replaced humanity. Orwell has replaced Osler. Medicine has and will continue its dystopian journey through AI.

RE: Why have podiatrists’ surgical fees decreased over the past 10 years? (Sev Hrywnak, DPM)

"You should get out of that business!" Short story relating to Dr. Hrywnak's eloquent post. About 20 years ago, I was having a conversation with a good friend of mine in the construction business. I was telling him of all the struggles that doctors/surgeons go through to get not only a fair fee for services but one made in a timely fashion. He was appalled at the scenarios I shared with him over insurance denials and their manipulations. His answer was the quote above. He could not fathom the idea of not getting paid for services rendered. Here it is 20 years later, and reimbursements have deteriorated even more so and this is the way it will remain. Why? 
 
The late Dr. Neal Frankel, who was somewhat of a pioneer in practice management, once lectured to group of us at a division meeting. He told us that he was speaking to the CEO of a major U.S. health insurer who said and I quote, "Why should we pay podiatrists more when we know they'll work for less." And there is it in a nutshell. We've been rightly conditioned as physicians/surgeons to do the right thing when it comes to our patients. But at what cost to us and our families?
 
As it is with most business, you perform a service, submit a bill, and you're paid for that service. Not so in our line of work. A doctor's years of study, training, and patient care are looked on by insurance companies as basically meaningless today. It's all about their bottom line. One can't really blame them. They're in the "business" of making the management and shareholders happy. The public is basically ambivalent. Bottom line: podiatrists’ surgical fees have significantly decreased over the past 10 years. You, as a surgeon, have to decide if the liability risk is worth the reward.

1. Verify insurance before every visit
 
It sounds basic, but skipping eligibility verification remains one of the costliest mistakes a front desk can make. When a practice fails to confirm active coverage, network status, and patient responsibility ahead of time, the result is predictable: denied claims and revenue that vanishes into the re-work queue. 
 
The American Medical Association recommends that administrative staff contact the patient’s insurer to verify active policy, physician network status, procedure coverage, and prior authorization requirements before the appointment. Building this step into your scheduling workflow, not just your check-in workflow, gives your team time to resolve issues before the patient arrives.
