NY Podiatrist Shares Insights on Laser Therapy and Regenerative Medicine
Dr. Tami Patzer, host of Optimal Health Radio, recently interviewed Davinder Bhela, DPM a board-certified podiatrist specializing in laser therapy and regenerative medicine. Dr. Bhela discussed the benefits of laser therapy in treating various foot and ankle conditions, including plantar fasciitis, neuropathy, toenail fungus, neuroma, tendon inflammation, and arthritis.

One of the most common conditions treated with laser therapy is plantar fasciitis, which causes heel pain and discomfort. Dr. Bhela highlighted how laser therapy can provide relief and complete healing within two weeks for acute cases, and 6-12 weeks for more chronic conditions. He also emphasized that laser therapy is a safe and non-invasive option for patients who prefer natural treatments and want to avoid medication or surgery.

An Augmented Vision of our Medical and Surgical Future, Today?
In a first-of-its-kind study titled “An Augmented Vision of Our Medical and Surgical Future, Today,” a collaborative team from the University of Southern California (USC), Italy, University of Texas Health Science Center, San Antonio, and the UK, led by Dr. David G. Armstrong, has published pioneering research on the use of Apple Vision Pro (AVP) in the operating room. This marks a significant milestone as the first documented instance of AVP’s application in surgical settings, specifically during limb preservation surgeries.

The manuscript is published in the Journal of Diabetes Science and Technology, which has published other groundbreaking works in the field.

Query: Recalcitrant Case of Posterior Tibial Tendonitis
I’m wondering if anyone has treated a recalcitrant case of posterior tibial tendonitis (found on MRI) of the slip to the medial cuneiform. It is tender just under the medial cuneiform. The patient wears HOKA shoes and custom orthotics, and was in a boot and using a knee scooter non-weight-bearing for 4 weeks. The main posterior tibial tendon is non-tender and the patient can do bilateral; heel raises without pain, but single leg heel raises hurt after a certain number of lifts. The patient is also seeing a physical therapist.

Query: Denial of Flexor Tenotomies
We are having difficulty receiving payment for a surgery. Here is the scenario: a diabetic presented with chronic pressure ulcers due to hammertoes. Flexor tenotomies were performed under local anesthesia in the office. We billed the patient’s Blue Cross Medicare Advantage PPO (advance checking for authorization was done and no authorization was needed).

The payer denied charges stating, "denied for exceeds number/frequency approved/allowed". We called Blue Cross MA and asked what this meant and were told they do not allow that procedure to be performed twice in one day. We asked them to look at the modifiers. They said they see the modifiers on the claim and said we could appeal. We appealed with a copy of the operative report. They are still denying the claim. Any suggestions or advice for the next step?

Check to see if your Medicare carrier has the following wording in their LCD: "Medicare will cover debridement of mycotic nails as an adjunct to pharmacologic treatment with a prescription antifungal agent indicated per its Food and Drug Administration (FDA) label for the treatment of fungal nail infections." If such wording is present, then pharmacologic treatment with a prescription antifungal agent is another part of the definitive treatment for mycotic toenails.
Such prescription treatment should be reimbursed with B35.1, but the carrier might disagree. Either you could appeal all the claims and go before an administrative law judge and perhaps win, or you might consider if your state society has an insurance committee that might intervene. The APMA could intervene on a national basis to clarify with CMS that a nationwide policy should be distributed to every local carrier that even if the debridement of mycotic toenails may not be treated, there is no statutory exclusion of the definitive diagnosis as well as treatment of mycotic toenails. For example, even the dermatoscopic findings of this infective process should be covered as an evaluation and management service. The ordering of alcian blue staining should also be covered regarding this infective process.

Let me start off by saying I am happy with my chosen profession. I have read the posts over the years about the profession and its frustrations. As a side note, I wish such discussions did not surface on public forums as I fear that this likely has had a negative impact on student recruitment, but I guess that is the way of the world in an era of social media. Why we should be happy with our chosen profession is a complex discussion and probably needs two parts. Here is Part 1.
What I find interesting is that people think that these claims of unhappiness, discord, frustration, the haves and have nots is unique to podiatry. I can assure you it is not. I have had many podiatry lives: Dean of a podiatry school, practice in an orthopedic group who managed professional sports teams, large institutional medicine, and as the first podiatrist in the orthopedic department at Stanford with an...
