The annual PrePodiatryLife Women’s Month Scholarship was initiated by Elizabeth Ansert, DPM in 2021. It was created to inspire and recognize women podiatry medical students for their community service, health, and contribution at a national level. $250 cash prizes were awarded to two winners to support the creative written content in the field of podiatric medicine and healthcare.

PRACTICE MANAGEMENT TIP OF THE DAY
6 Questions to Answer Before Preparing 2020 Taxes - Part 3
 
As you begin to pull together your tax information for 2020, answer these 6 essential questions to potentially reduce your taxable income 2020 tax liability.
 
3.  Did you give any monetary gifts?
 
Each and every year you can gift a certain amount to anyone you want without having to deal with the gift tax. For 2020, that amount was $15,000. The $15,000 applies to each recipient, so if you donated $15,000 each to five of your children, you will have reduced your taxable estate by a total amount of $15,000 x 5 or $75,000. That’s not a tax deduction for this year but will reduce your heirs’ ultimate estate tax liability.
 
If you gave more than $15,000 in cash or assets (e.g. stocks, land, a new car) last year to any single person, you will need to file a gift tax return IRS Form 709 to be compliant with the IRS tax code. This doesn’t necessarily mean you’ll have to pay a gift tax this year, it simply satisfies the requirement to disclose the gift.

Query: Two Ulcers, Two Types of Codes
 
For a Medicare patient, if you debride ulcers on both legs, is it all right to bill an E/M code for one leg and a debridement code for the other leg? For example, you cite management on the right leg of the culture review, home health order updates, venous insufficiency management, dressing change (and by the way, you debride the ulcer). And on the left leg ulcer, you cite the process of debridement. In summary, I would bill E/M for right leg ulcer and associated conditions and bill CPT for debridement on the left leg ulcer. Would this pass audit?

Response: With the proper documentation, you should survive any audit. First off, you debrided both legs and you should bill for what you do. Remember that debridement is not an RT/LT situation but aggregate size of similar wounds and level of debridement.
 
That brings us to the E/M. -Culture review- Okay, reviewing unique test is one component of E/M selection. But what did you do? Continue with empirically started cephalexin? Changed/added/discontinued antibiotics per culture?
-Home health orders. What did you change? I assume the home health agency will be evaluating/treating both legs and not just the right side.
-Venous insufficiency management. What did you change? If these are venous ulcers, wasn’t that discussed day 1?
-Dressing change. Not payable anyway when a debridement is billed.
 
So, yes, you are treating “a stable chronic/acute illness” and, yes, you reviewed data so, in theory, you met two out of three E/M criteria. But remember that each procedure has a certain amount of E/M built into the fee allowance for the procedure. So was the work that you cite “significant and separately identifiable” from the E/M included in the procedure performed? This is where you will win or lose the audit.

QUERIES (NON-CLINICAL) - PART 1
Query: New Optum Fee
 
I have spoken to my billing department and have been informed that to print remittance statements from Optum, we now need to pay 0.05% of the amount reimbursed to us. Mind you, we can view them online, but they have eliminated the ability to print the statements. Optum is contracted with UnitedHealth Care. I have never heard of such a problem and cannot believe that a percent charge to print our statements from our computer would be possible. Has anyone else encountered this problem?

RE: Diagnosing Achilles Tendon Rupture During a Virtual Examination

The New England Journal of Medicine (February 27, 2021), in an article by Stephen Bent, MD and Martin J. O’Malley, MD, reported how a diagnosis of Achilles tendon rupture was made remotely in a 54-year-old man who sustained the injury while playing tennis. He felt a tearing sensation in his lower left leg and was unable to bear weight on that extremity since the injury. During video consultation, he was asked to lie in a prone position and flex his knees to a 90-degree angle and relax. Asymmetry of the feet including abnormal dorsiflexion of the left foot was apparent and suggestive of a rupture of the Achilles tendon.
 
Achilles Tendon Rupture Pre- and Intra-op
 
Since the exam was performed remotely, the examiner could not squeeze the calf to assess plantarflexion. During surgical intervention, Achilles' tendon rupture was confirmed and the tendon was repaired, followed by wearing a boot, gradual increase of weight-bearing, as well as 3 months of physical therapy. At the end of 5 months, the patient’s gait was normal and his strength improved.
